The relationships between social network centrality, social construction of knowledge, and nurse practitioner competency among adult gerontology acute care nurse practitioner students participating in asynchronous online discussions were explored. Social network centralities (betweenness, in-degree, out-degree, closeness, and eigenvector) were determined through social network analysis. Social construction of knowledge was assessed by the Interaction Analysis Model (IAM). Nurse practitioner competency was evaluated using the Novice to Expert model. A retrospective exploration of an online discussion board from the College of Nursing at the University of New Mexico was conducted. The absence of quality assurance in training clinicians to perform pre-anesthesia consultations at a Canadian university is the Problem of Practice addressed in this Organizational Improvement Plan. This competency requires learners to apply their anesthesia knowledge to take medical histories; perform physical examinations; diagnose anesthetic risks; and generate anesthesia plans. Random chart audits of many learners identify deficiencies and suggest inconsistent training of this competency. This Organizational Improvement Plan analyzes the anesthesia program’s organizational context to be a complex adaptive system; organizational structure to be a hierarchy; and organizational state to be static. The Dominican health care system has many challenges to meet the legal and the population’s health care expectations (La Hoz, 2018; United States Agency for International Development, 2013). On entry into the nursing profession, new graduate nurses (NGNs) possess some knowledge and skills competency but self-report learning gaps in various areas. Experienced nurses have reported that new nurses do not always have knowledge and competency in essential nursing skills. Undergraduate nursing programs and organization orientation programs provide valuable foundational knowledge but are sometimes limited in providing multiple exposures to content. Lagging skills acquisition contributes to self-doubt, lack of confidence and high attrition among NGNs.
