Fairfield Medical Center is a vibrant healthcare organization focused on providing exceptional healthcare experiences to everyone it serves. Fairfield Medical Center encompasses Cancer care, diabetes managements, emergency care, Gastroenterology, Heartburn Care, Hospice Care, Infectious Disease, Maternity Care, Neurology, Obstetrics & Gynecology Care, and Orthopedic Care.

From a physical therapy standpoint, FMC operates two outpatient clinics, and one inpatient service at the hospital. There are over 40 therapists who will either stay primarily at outpatient and inpatient, and some float between the two depending on the volume of patients.

In attempts to make government fair and equitable, the process of any bill but especially the budget bill can be extremely convoluted and difficult to follow. I was able to learn how to navigate the system and understand the processes by which things happen at the statehouse.

The statehouse is filled with representatives of countless interest groups all fighting for their opinion of how things should be done. As demonstrated by this budget cycle, government can be a push and pull directed by politics, personal opinions, and countless other factors, which may result in nothing getting done.

Abstract

Problem Statement: The frequency and severity of hospital post-discharge events has become a national problem. The increase in readmission rates post-discharge has a negative impact on the patients overall morbidity and increases healthcare costs (Jack, 2012). Non-comprehensive discharge planning contributes to post-discharge events such as less than 30-day readmissions (Jack, 2012). Best practices for discharge planning should be utilized.
