Patient safety issues remain despite several strategies developed for their deterrence. While many safety initiatives bring about improvement, they are repeatedly unsustainable and short-lived. The index hospital’s goal was to build an organizational culture within a groundwork that improves teamwork and continuing healthcare team engagement. This study sought to obtain a better understanding of how leaders influence the culture of the organization to enhance performance. A post-positive qualitative research case study was conducted at a state agency in the Commonwealth of Virginia. During this study, 13 semi-structured interviews were conducted with agency employees who participated on a project to implement a smart phone fuel application. This fuel application automated specific manual processes related to their fuel management program. While the fuel application project implementation was finishing, the interviews were conducted. The purpose of this qualitative phenomenological study was to explore the experiences of 50 personnel from four Metro Washington D.C. area fire departments, 23 firefighters and 27 officers, at varying experience levels and ranks to determine if there was leadership training throughout a firefighter’s career that enhanced their leader development, improving teamwork needed to achieve operational objectives. Hofestede (1984) and Gray (1988) conducted studies and observations of the cultural dimensions and values that have contributed to culture and accounting research. National culture is broad in its influences, but affects the smallest aspects of society-even accounting. Accounting is also influenced by organizational culture, the overall environment in which a company functions.
