Literature Review On The Effectiveness Of Poc Lactate Tests For Post-Operative Sepsis Discharge, Blaire Adler, Paarth Jain, Nathan Rim, Rachel Monane

Phase 1

Background: Every year in the USA, there are at least two million surgery-related infections; around 11% are derived from decompression spinal cord surgery. Generally, later detection of sepsis leads to significant burdens on the healthcare system; this manifests financially, temporally, and in poorer patient outcomes. “SepsisCheckToGo” is a toolkit aimed to minimize the number of sepsis complications in spinal surgery patients postoperatively; the kit would include a pulse oximeter, thermometer, and fingertip POC lactate tests to detect early sepsis signs after discharge and before the follow-up visit (which would occur around one month after initial discharge).

Background: Baseball pitching is a dynamic movement where the lower extremities generate and sequentially transfer energy to the upper extremities to maximize ball velocity. The need for lower body muscular strength to produce adequate push-off and landing forces has been documented; however, the influence footwear and surface inclination has on muscle activation remains unknown.

Objectives: Determine how pitching in molded cleats and turf shoes from a pitching mound and flat ground affects stride-leg muscle activation in youth baseball pitchers while determining percent activation during each pitching phase.

Occupational therapy (OT) is an allied healthcare profession that is uniquely situated at the intersection of art and science. OT seeks to improve quality of life by addressing occupations, or daily activities that are meaningful and purposeful (Nelson, 2014). In order to accomplish these goals, occupational therapists (OTs) must use design thinking through the lens of evidence-based practice. This artistic creativity paired with well-researched scientific findings mimics the visual nature of this guide, which prioritizes this need for alternative representation of the sciences.

Breast cancer commonly metastasizes to bone, resulting in osteolytic lesions and poor patient quality of life. The bone extracellular matrix (ECM) plays a critical role in cancer cell metastasis by means of the physical and biochemical cues it provides to support cellular crosstalk. Current two-dimensional in-vitro models lack the spatial and biochemical complexities of the native ECM and do not fully recapitulate crosstalk that occurs between the tumor and endogenous stromal cells. Engineered models such as bone-on-a-chip, extramedullary bone, and bioreactors are presently used to model cellular crosstalk and bone-tumor cell interactions, but fall short of providing a bone-biomimetic microenvironment. …

A Comprehensive Analysis Of Balance, Symmetry, And Center Of Mass In The Gait Cycle Of Transfemoral Amputees, Kayla T. Etienne

The purpose of this thesis is to create a framework that assists in the transfemoral prosthesis fitting process by calculating balance and symmetry to quantify patient comfort with an understanding of bipedal locomotion and human anatomy. Three different software applications were used to compare (1) the body position during gait cycle, (2) the natural and amputee anatomies, (3) the natural and prosthetic legs, and (4) the equilibrium and torque movements of the hip, knee, and ankle joints. To understand the impact sports nutrition knowledge has on the risk for developing LEA in Female Cross Country Runners.

Background

The International Olympic Committee introduced the concept of Relative Energy Deficiency in Sports (RED-S) to accurately encompass the condition previously known as the Female Athlete Triad. LEA is the root cause of RED-S. (Mountjoy et al., 2014). The purpose of this study was to measure sports nutrition knowledge and the impact knowledge level has on the susceptibility to develop LEA in female cross country runners.

Background: Designing South Philadelphia’s first permanent healthcare facility dedicated to refugee health presents a unique opportunity to integrate cultural sensitivity with principles of community health. Design thinking (human-centered design) is a promising strategy to address health and social justice concerns through the development of innovative products and services that prioritize population needs. Background: Inhalers are a commonplace in American health care and deliver crucial drugs to patients with COPD and asthma. Inhaler use has been shown to be unsatisfactory among patients resulting in ineffective medication delivery. The goal of this project was to improve the inhaler design for increased effectiveness and ease of use.

Background: Foley catheter-associated UTI’s (CAUTI’s) represent a driving cause of cystitis, bacteremia and sepsis in the hospital setting. Accordingly, the CDC has identified prolonged indwelling time as the foremost risk factor for CAUTI’s. However, the fundamental design of the Foley has remained unchanged for over eighty years. Hospital-specific protocols do currently exist for the removal of infection-prone catheters; however, in practice it remains commonplace for catheters to be ignored, increasing the risk of life-threatening infection. Waste anesthetic gases (WAGs) are associated with spontaneous miscarriages in pregnant persons, an increased risk of congenital abnormalities, hepatotoxicity, neurotoxicity, and cognitive impairment. Through monitoring anesthesiologists we found levels of WAGs to be 5-10x the current standard inside the OR. Currently, no solutions exist for the mitigation of WAG release. Remora is a solution to removing WAGs that fits on top of existing anesthesia masks. A flexible skirt is joined to a rigid ring, which deforms under hand pressure to facilitate effective hand-to-mask placement. Project Background: In the United States, 2% of the population will experience bone spurs, which will not resolve on their own.1 Of people over 60 who experience a bone spur, 40% will require medical attention.2 The soft tissue visualization appears to be difficult with imaging, leaving increased chance of poor balancing of the knee replacement. Osteophyte structure continues to be difficult to visualize in a 3D way and without their proper removal, can lead to increased rate of revision surgery. Background: Excessive auditory stimulation can have negative effects on the growth and development of newborn babies. The American Academy of Pediatrics states that newborns should not be exposed to sounds in excess of 45dB while they are in the hospital, however noise levels in NICUs across the country are often between 50-100dB. A design project was conducted to develop an intervention that could reduce infant exposure to excessive noise.

Background: It is estimated that about 1 in 10 Americans use inhalers to deliver inhaled drugs for respiratory disease such as COPD and asthma. Some studies have shown that up to 84% of people use their inhaler incorrectly, which can cause long term problems and acute flareups. Therefore, improving the inhaler design to allow for ease of use was the main focus of our project.

Background: 3D printing has recently emerged as an effective, cost-efficient tool for healthcare innovation. We propose the fabrication of 3D printed, patient-specific liver models as a pre-surgical planning and communication tool for liver resection surgery.

Methods: Creation of the model began with the segmentation of the patient's abdominal CT scan, where specific sections of their anatomy, including the blood vessels (portal and hepatic systems), gallbladder, and tumor (when applicable), were digitally segmented. Background: Catheter associated urinary tract infections (CAUTI) are a major source of patient morbidity and mortality in healthcare settings and have a significant financial impact on the healthcare system. Many hospitals have clinical protocols in place to continually reassess the necessity of indwelling foley catheters and remove them if their use is no longer warranted. However, it is still common for indwelling foley catheters to be overlooked and remain in place, even after meeting criteria for removal. Project Background: Osteophytes are a common problem, affecting 2% of the United States population. For many elderly people, these osteophytes will cause them to seek medical attention. Due to the 2-D nature of MRIs and CT scans, it can be difficult to gain a complete understanding of the complicated soft tissue structures surrounding the joint when performing a Total Knee Arthroplasty(TKA). Without proper removal of osteophytes and correct soft tissue balancing, there is an increased rate of revisional surgery.
