OBJECTIVES: The purpose of this study was to determine the feasibility, safety and diagnostic accuracy of dobutamine stress echocardiography (DSE) for evaluating posttransplant coronary artery disease (TxCAD) in children, and to determine the frequency of selected cardiac events after normal or abnormal DSE.

BACKGROUND: Posttransplant coronary artery disease is the most common cause of graft loss (late death or retransplantation) after cardiac transplantation (CTx) in children. Coronary angiography, routinely performed to screen for TxCAD, is an invasive procedure with limited sensitivity. In 1918, in the Crimea, the adolescent Vladimir Nabokov devised a new pastime: "parodizing a biographic approach" by narrating his own actions aloud. In this self-conscious "game," he orchestrated changes in grammatical person, gender, and tense in order to transform his present experiences into a third-person past, as remembered by a female friend in an imaginary future. Staging his own biography in this fashion allowed Nabokov to resolve the inherent conflict between his life and his art. The purpose of this study was to determine the effectiveness of a modularized therapeutic recreation self-esteem program (that included outdoor adventure based components) provided for adolescents in a residential treatment center. Data were also collected on demographic variables including age, gender, and ethnicity. Subjects consisted of 112 adolescent patients ranging from ages 12 to 17, with an average length of stay of nine months to one year.

Secular Trends In The Incidence Of Anorexia Nervosa: Integrative Review Of Population-Based Studies, Kevin M. Gorey

OBJECTIVE AND METHOD: Aggregating across retrospective cohort samples, this integrative review synthesizes the findings of 12 cumulative incidence studies (45 hypotheses) on anorexia nervosa secular trends.

RESULTS: (1) The female/male anorexia incidence rate ratio was estimated to be 8.20, 18.46 versus 2.25 cases per 100,000 per year, p < .05; (2) female teenagers experienced anorexia at a rate fivefold greater than other women, 50.82 versus 10.37 incident cases per 100,000 per year, p < .001; (3) no secular trend or change in the incidence of anorexia was observed among teenagers, while a near threefold increase was observed over the past 40 years among women in their 20s and 30s, 6.28 (1950-1964) versus 17.70 (1980-1992) cases per 100,000 per year, p < .05; and (4) the two cohort characteristics of age, and the age by year interaction accounted for nearly two thirds of the variability among anorexia incidence estimates, R2 = .614, F(2,27) = 21.49, p < .001. After the two factors of age and the Age x Year interaction were accounted for, none of the other study characteristics, including study year(s), were found to be significantly associated with anorexia incidence, that is, a main effect of time was not observed.

DISCUSSION: The integrative evidence across the population-based epidemiologic studies covering 40 years in this field suggests strongly that, overall, the incidence of anorexia nervosa, particularly among those very young women at greatest risk of experiencing it, has not increased significantly. OBJECTIVES: This study examines the impact of the condom availability program in New York City public high schools by comparing rates of sexual activity and condom use for New York students and similar students in Chicago.

METHODS: A total of 7119 students from 12 randomly selected New York schools and 5738 students from 10 Chicago schools participated in a cross-sectional survey.

RESULTS: New York students, compared with Chicago students, reported equal rates of sexual activity but higher rates of condom use at last intercourse (odds ratio [OR] = 1.36). For higher-risk students (those with three or more sexual partners in …

A countywide survey in Uinta County, Wyoming, was conducted from December 1992 through March 1993. A stratified random sampling method was used in which 647 surveys were distributed with an overall response rate of 59.5%. This study employed analysis of variance (ANOVA) to address four research questions contending that the independent variables o f education, socioeconomic status, employment, and family stat us would differentially account for variation in the dependent variables, which were responses to survey items indicating the "appropriate age" to teach children about human reproduction, consequences of sexual activity, birth control methods, HIV/AIDS prevention, and sexually transmitted diseases. …

The Prevalence Of Child Sexual Abuse: Integrative Review Adjustment For Potential Response And Measurement Biases, Kevin M. Gorey

This integrative review synthesizes the finding of 16 cross-sectional surveys (25 hypotheses) on the prevalence of child abuse among nonclinical, North American samples. It is essentially a research literature on sexual abuse; only one of the studies assessed physical abuse, and there has not yet been a single study of prevalent child emotional abuse nor neglect. The following summative inferences were made: (1) response rates diminished significantly over time, M = 68% prior to 1985 and M = 49% for more recent surveys, p < .05; (2) unadjusted estimates of the prevalent experience among women and men of childhood sexual abuse was 22.3% and 8.5%, respectively; (3) study response rates and child abuse operational definitions together accounted for half of the observed variability in their abuse prevalence estimates, R2 = .500, p < .05; (4) female and male child sexual abuse prevalence estimates adjusted for response rates (60% or more) were respectively, 16.8% and 7.9%, and adjusted for operational definitions (excluding the broadest, noncontact category) they were 14.5% and 7.2%; (5) after adjustment for response rates and definitions, the prevalence of child sexual abuse was not found to vary significantly over the three decades reviewed. Given the large human costs, both personal and social, of child abuse, and the identified gap in the requisite knowledge needed to steer effective preventive and treatment interventions, it is time to invest in a large, methodologically rigorous, population-based study of child abuse which, if it does nothing else, spares no expense in ensuring very high participation.

BACKGROUND: This study addresses the yield and clinical impact of computed tomography (CT) imaging in otherwise asymptomatic patients with stage III melanoma metastatic to the regional nodes.

METHODS: The database from the University of Michigan Mutlidisciplinary Melanoma Clinic was reviewed and identified 127 asymptomatic patients with stage III melanoma (regional nodal disease) who received CT scans of the head, chest, abdomen, and/or pelvis. Scans were confirmed as true positive, false positive, and normal.

Adolescent sexual offenders (n = 27) were compared to non-sexually offending juvenile delinquents (n = 23) on familial, sexual and social variables. The two groups were demographically similar and did not differ significantly on a measure of perceived family environment. It was hypothesized that the two groups would be similar on familial and social variables; that is, both displaying similar levels of perceived dysfunction. It was further hypothesized that the two groups would differ significantly on measured behavioral and cognitive constructs defined as sexual deviance. The purpose of this study was to determine the attitudes of youth toward each of their individual grandparents. The subjects were students ranging in age from 12-19 years with the majority being between 14-17 years of age. These students were enrolled in a release time religious class that allowed them one hour a day off campus to take a seminary course.

The Relationship Between Perceived Parenting Styles And Adolescent Locus Of Control And Self-Concept, Lisa Ann Mcclun

Relationships between adolescents' perceptions of their parents' responsiveness and demandingness, adolescents' locus of control orientation, and adolescents' self-concept ratings were investigated. Subjects included 198 students from a middle school in northern Utah. Subjects were given the Nowicki-Strickland Internal-External Control Scale for Children, the Harter Self-Perception Profile for Adolescents, and the Perceived Parenting Style Survey.

Results indicated that subjects who perceived their parents as being authoritative had significantly (p < .001) more internal locus of control scores than subjects who reported either the permissive or authoritarian styles. Also, self-concept scores were significantly higher (p < .001) for the authoritative group than the authoritarian group on the Self-Perception Profile for Adolescents subscales of Scholastic Competence, Social Acceptance, Physical Appearance, Behavioral Conduct, Close Friendship, and Global Self Worth. The permissive group reported significantly lower scores (p < .05) on the subscales of Scholastic Competence and Behavioral Conduct. There was also a significant negative correlation (p < .001) between locus of control scores and the subscale scores on the Self-Perception Profile for Adolescents.

This study investigated the quality and quantity of sexual communication between teens and parents in relation to the teens' sexual intentions. Perceptions about communication were examined of parents and adolescents of both genders separately. Both same- and cross-gender effects of parental communication on adolescent sexual intent ions were explored. Approximately 290 families (target teen and parent) were measured in pre- and posttests. Parents perceived higher quality communication with teens than teens did, but not necessarily more frequent sexual communication. It is posited that self-conception disparity is the amount of difference between an individual's ideal self-conception and his or her real self-conception. Such a postulation arises directly from the literature wherein the self-concept is conceptualized as a multitude of self-conceptions an individual has. During the evaluative phase (i.e., the comparison of the ideal self-conception against the real self-conception), an image (self-image) of one's self is evoked. Demographic, historical, psychometric, and clinical data were obtained from the psychiatric files of all patients manifesting schizophrenic symptomatology who were hospitalized in an adolescent psychiatric facility during a five year period (N= 71). Factor analysis of the usable data resulted in three interpretable factors, which included: (1) aggressive behavior; (2) disturbed family functioning; and, (3) thought disorder. Age of first hospitalization correlated positively with factor three.

A predicted association between family relations and cognitive development and the emergence of adolescent egocentrism was explored in this study. A sample of seventh grade boys (n=131) and girls (n=120) completed Elkind and Bowen's Imaginary Audience Scale (a measure of egocentrism) and selected items from Heilbrun's Parent-Child Interaction Rating Scale and Schaefer's Parent-Behavior Inventory. A modified version of Lawson's Classroom Test of Formal Operations was used to measure cognitive development.
