This project provides convincing empirical evidence on the impact of quality of care on contraceptive use and fertility behavior through analysis of a unique longitudinal data set from rural Bangladesh. The findings from this project add important new and methodologically rigorous evidence to the emerging literature on the implications of service quality on contraceptive behavior. The results reaffirm the importance of female fieldworker outreach—both in terms of quality and the number of visits—on contraceptive use dynamics in rural Bangladesh. Adolescents, who make up about 25 percent of the population of Bangladesh, have limited access to reproductive health (RH) information and services. With age at marriage increasing, young people are vulnerable to a variety of RH risks, including unwanted pregnancy and sexually transmitted infections (STIs). In 1999, FRONTIERS tested the feasibility, impact, and cost of several interventions to improve the RH of adolescents. The intervention took place in two experimental urban sites, while a third control site received prevailing services. Over the last 30 years, Bangladesh’s national family planning (FP) program has mainly concentrated its promotional efforts on women’s adoption of FP. While the government policy has been effective in influencing women to accept contraceptive methods, men’s role in FP has been completely neglected. Similarly, male RH services are hardly available at Health and Family Welfare Centres (HFWCs). This report describes a project aimed at integrating male RH services within the existing government female-focused health-care-delivery system. Marriage is an event of great social and economic significance in most societies. Despite the centrality of marriage in an individual’s life history, the literature on marriage patterns pays little attention to men. This paper examines trends in schooling, age, and assets at marriage for both men and women, and spousal differences in these variables in six countries-Bangladesh, Ethiopia, Guatemala, Mexico, the Philippines, and South Africa-using comparable data sets and methodologies.

This paper describes two initiatives that have used experimental studies to guide the development of community-based health and family planning programs. In Bangladesh and Ghana, factorial experiments were implemented in stages. An exploratory phase developed a service system for community-based health care; an experimental phase assessed the demographic impact of the system; a replication phase examined the transferability of the experimental program to a non-research setting; and a scaling-up phase facilitated the extension of the new system to the national health care program. All stages were guided by research, with questions, mechanisms, and outcomes shifting as the process developed. Large-scale …

Power, Knowledge And Childbirth Practices : An Ethnographic Exploration In Bangladesh, Kaosar Afsana

The intent of this study is to explore why and in what ways rural, poor women in Bangladesh adhere to indigenous birth practices and resist cosmopolitan obstetric care. To understand the complexities of childbirth, a multidimensional framework encompassing culture, gender, socio-economic, political economy and historical perspectives is used. I used ethnographic methods to have deeper understanding of childbirth practices predominantly from women's voices, but strengthened by multiple other voices and my observational experiences. I gathered information in Apurbabari village, the adjacent Thana Health Complex and the Medical College Hospital using in-depth interviews and participant observation, in particular.
