An ever-growing body of evidence supports the efficacy of integrated care as a treatment approach; however, less is known about what specific components of integrated care are most effective. This is especially true of warm handoffs, which are an often discussed but understudied process in integrated care. A total of 246 patient charts were reviewed to determine if type of referral (warm handoff or traditional) increased the likelihood of follow-up with behavioral health services as well as factors that might impact this relationship. There were no significant differences between type of referral and likelihood of follow-up with behavioral health services. …

Primary care providers have become the front line of treatment for mental health in the United States. Group interventions have been argued to be an effective way to treat more patients with fewer resources, which could reduce the burden of psychopathology on primary care settings. Group therapy faces many barriers to successful implementation in primary care, including site constraints, provider perceptions, population needs, and recruiting difficulties. A survey was developed to assess primary care providers’ perspectives on these areas and distributed via online survey to practitioners in Appalachia; 28 providers responded. Postpartum depression (PPD) is a prevalent, complex illness impacting 10% to 20% of mothers and their families. Treatments for PPD, such as medication and psychotherapy, are effective at reducing the severity of symptoms and generally improving quality of life for new mothers and their families. Unfortunately, many mothers with PPD go unrecognized due to a lack of standardized screening methods. Further, mothers regularly encounter barriers to accessible, evidence-based follow-up care to treatment even when symptoms of PPD are detected. Suicidal behavior, including ideation and attempts, is a significant public health problem. Due to the complexity of suicidal behavior, it is necessary to consider an array of factors that could serve as risk and protective factors. Previous research has shown that deficits in social problemsolving ability are associated with increased risk for suicidal ideation and attempts; conversely, problem solving strengths are associated with reduced risk. This dissertation project, consisting of 3 individual manuscripts, was designed to explore the relationship between social problemsolving ability and suicidal behavior in low-income primary care patients. Between one quarter and one third of all primary care patients meet criteria for at least one mental disorder. However, many women are not screened for mental disorders as recommended by the literature. Some studies suggest even lower rates of screening and treatment in rural areas. This researcher sought to identify barriers and facilitators to mental and behavioral health screening and treatment for women in primary care and solicit feedback on how to ameliorate perceived barriers. Objective: To examine the prevalence of psychosocial concerns presenting in 3 pediatric primary care clinics serving rural Appalachia. The association between pediatric weight status and psychosocial concerns was examined. Methods: Caregivers of children presenting in one of 3 pediatric primary care facilities completed a demographic questionnaire and the Pediatric Symptom Checklist. Results: Approximately 21.4% of children were identified as having a clinically significant behavioral health concern. Among children with available weight status data, ~53% of the children were identified as overweight. Children identified having significant behavioral concerns were more likely to have a lower weight status than those with nonsignificant …
