Zambia’s HIV prevalence is estimated at 11% among adults aged 15–49 years and 1% among children younger than 15 years. An estimated 10% of Zambia’s population is at high risk of being orphaned or vulnerable due to the HIV epidemic. The Zambia Family (ZAMFAM) project aims to improve the care and resilience of vulnerable populations while supporting HIV epidemic control. ZAMFAM used a case management approach that tracks beneficiaries from identification to graduation. The ZAMFAM program aims to improve the care and resilience of orphans and vulnerable children (OVC) as well as young people living with HIV in Zambia by providing support and protection and by strengthening the capacity of children, families, and the community to better manage the socioeconomic shocks of the HIV/AIDS epidemic. A case study approach was employed to understand actors and perceptions, and to document best practices by the program. This brief summarizes the case study findings. There are substantial gaps in essential services and numerous missed opportunities to engage children in care and provide effective HIV treatment. Although not new, family-centered care models that provide HIV services comprehensively to families as a unit, rather than providing separate services to children and adults, have the potential to address family needs and break down social, physical, and emotional barriers to accessing care. The family-centered care study (FAM-CARE) aims to contribute to our understanding of the role of family-centered care models (FCCM) in improving pediatric HIV outcomes in a sub-Saharan African setting.

The Population Council has a longstanding partnership with indigenous communities in rural Guatemala through the Abriendo Oportunidades program. In compliance with COVID-19 prevention protocols, as of mid-August 2020 government restrictions allowed for inter-district mobility at the national level, classes remain suspended, and businesses were allowed to operate. To better understand indigenous knowledge, perspectives, and evolution of needs related to the COVID-19 pandemic in Guatemala, we conducted a second round of phone interviews with 144 key informants in ten municipalities. Pakistan's family planning (FP) program remains more or less exclusively aimed at women, yet evidence from both international as well as programming from within Pakistan suggests that including men in FP could improve results. Given the large youth cohort entering reproductive life in Pakistan, there are compelling grounds for also including younger men and adolescent boys, especially those about to wed, within the country's reproductive health (RH) and FP strategies.

Although sexual violence against children is increasingly prevalent in Kenya, documented and tested interventions for proactively identifying child survivors and providing them with care are currently nonexistent. This study aimed to determine the feasibility of screening children for SV in both school and health facility settings, and of providing child survivors identified through this process with access to care. The four-component intervention, implemented from January through April 2017, involved parent dialogues; student sensitization; provider training to administer a child-friendly screening tool and respond to child survivors; and SV screening, paired with SV service provision. In Haiti, where an estimated 3,000 children and youth are receiving antiretroviral therapy (ART), Caris Foundation, with funding from the United States President’s Emergency Plan for AIDS Relief (PEPFAR) through the United States Agency for International Development (USAID), provides hospital-linked psychosocial support groups, known as Kids Clubs, for young people ages nine and older. The clubs are intended to help ensure that HIV-positive children and adolescents stay in contact with health services, and serve as a conduit for delivery of other services, including health messages and health products. This brief presents responses from the first round of data collection among adults, adolescents, and community leaders in indigenous municipalities of Chiapas and Yucatan, Mexico, June 1–30, 2020. In total, 295 people from 17 municipalities were surveyed in Yucatan, and 83 people from 15 municipalities were surveyed in Chiapas. Adults and adolescents in both states identified the groups most at risk of suffering complications. Food insecurity may be a consequence of the pandemic. Regarding livelihoods, 51% of women and 63% of men in Yucatan have lost their work and/or their primary source of income since the start of the pandemic, …

Female genital mutilation/cutting (FGM/C) continues to be a widespread practice in Egypt. According to the 2014 Egypt Demographic and Health Survey, the prevalence of FGM/C was 92 percent among ever-married women aged 15–49. However, Egypt continues to witness a drastic surge in the medicalization of FGM/C, with 74 percent of women aged 19 years and younger circumcised by medical practitioners, compared to 55 percent in 1995. One of the significant implications is a food shortage in households and a reduction in food intake. This nutrition shock may hit females harder than males due to the deep-rooted gendered culture prevalent in India. Drawing on data from two rounds of COVID-19 knowledge, attitudes, and practices (KAP) telephone surveys conducted in Bihar, India, this research brief sheds light on the socioeconomic differentials and changes over time in the adoption of COVID-19 preventive measures. Though the adoption of preventive measures has increased, around two-fifths of males and one-quarter of females reported that they were currently practicing all four standard preventive measures. The better educated, wealthier, and those belonging to privileged castes were more likely than others to practice these measures, as were those who perceived moderate or high risk of COVID-19 in their neighborhood. COVID-19 and steps to break the chain of viral transmission led to certain economic challenges that triggered loss in livelihoods and income and constraints on financial resources in Bihar, India. Often, these effects were disproportionately experienced among the most marginalized. Female genital mutilation/cutting (FGM/C) is considered a harmful cultural practice and a form of violence against women. Kenya has an FGM/C prevalence rate of 21 percent, but levels vary by ethnic group. Kenya has adopted a robust legal framework for the prohibition of FGM/C, including passage of the Prohibition of FGM Act in 2011, a detailed law that criminalizes carrying out FGM/C and associated offenses. Kenya relies on the human rights–based approach and criminalization in its legal framework for the prohibition of this practice. Despite extensive legal provisions, Kenya has prosecuted fewer than 100 cases, raising concerns with the law’s …

Economic hardships caused by the nationwide lockdown to prevent the spread of COVID-19 have been reported across India. Social protection measures have been announced by the state and central governments to ease these hardships. The key objectives of this research brief are to examine the awareness and the social protection measures received during the COVID-19 lockdown in Bihar, and the socioeconomic differentials in receiving these measures. The majority of respondents (64%) reported a family member losing their job/income-earning opportunity and lacking resources to survive for long. The Sustainable Development Goals target the elimination of all forms of harmful practices, including female genital mutilation/cutting (FGM/C) by 2030. Kenya has made progress in addressing the elimination of FGM/C through ratification of global instruments and enactment of the Prohibition of FGM Act in 2011 and other supportive laws. The Act created the foundation for the establishment of the Anti-FGM Board, which provides policy leadership and coordination of anti-FGM/C programs in Kenya. More recently, the President of Kenya issued a decree for accelerated elimination of FGM/C by 2022. This synthesis report highlights how evidence generated over the last five years …

The first COVID-19 positive patient in Bangladesh was reported on March 8, 2020. The advent of COVID-19 triggered the closure of all educational institutions. A nationwide “holiday” was declared on March 26, 2020, and a ban was implemented on all passenger travel by water, rail, and air. Domestic work is characterized by excessive working hours, extremely low pay or no pay at all, and conditions that make the profession exploitive in many cases. Evidence is emerging that domestic work is a feeder profession for sex work. One study of more than 2,000 commercial sex workers in Ethiopia found that 42 percent were former domestic workers. However, little is known about transitions to commercial sex work or commercial sexual exploitation in Tanzania. This technical report presents results of the baseline Behavioral Sentinel Surveillance survey by Breakthrough RESEARCH in Nigeria in September and October 2019. From 2019 to 2021, baseline, midline, and endline surveys will assess the effectiveness of Breakthrough ACTION integrated social and behavior change activities for malaria, family planning, and maternal, newborn and child health plus nutrition in Kebbi and Sokoto in comparison to Breakthrough ACTION's exclusively malaria activities in Zamfara state. Malaria activities were initiated in Zamfara in October 2019. This document pulls from a recent review of social and behavior change (SBC)-focused literature and highlights eight promising practices regarding how to integrate gender considerations in health and non-health SBC interventions. The practices are operationalized at different levels of the Socio-Ecological Model for Change and are mutually reinforcing.
