However, these questionnaires often employ negatively worded items that can potentially worsen patients' distress. Ten to 20% of individuals who sustain a concussion continue to experience symptoms outside the typical window of recovery. Pre-morbid and post-injury anxiety are risk-factors for persistent post-concussion symptoms (PCS). However, mechanisms linking anxiety and persistent PCS are unclear. The fear-avoidance model of disability could add clarity to associations between anxiety and persistent PCS. This study examined if factors of the fear-avoidance model (e.g., catastrophic thinking, fear of symptoms, anxiety sensitivity) would mediate the association between persistent PCS and maladaptive coping responses (e.g., avoidance, limiting activities) following concussion. Gratitude interventions are emerging as a method to improve physical health outcomes; however, gratitude’s effect on pain perception and the autonomic response is understudied. Recent research suggests the perigenual anterior cingulate cortex is activated in gratitude interventions and is also involved in the perception of pain and the activation of the sympathetic nervous system. In this study, we investigated the effect of state gratitude induction on pain through the use of the cold pressor method. The findings presented in this dissertation are part of the bigger SYMBIOME project which aims to use the biopsychosocial model of pain to develop a prognostic clinical phenotype for people that experience musculoskeletal (MSK) trauma. Chapter 2 presents an exploratory analysis to assess the relationships between genetic polymorphisms and pain severity and interference. Early childhood trauma was also explored as a moderator between genetic polymorphisms and pain outcomes. For pain severity, major allele carriers (A/A and G/A) of FKBP5 rs9394314 reported significantly higher scores than minor allele carriers (G/G). Post-9/11 veterans are more likely to experience elevated levels of pain and poor sleep quality compared to their civilian counterparts; however, little is known about how beliefs related to veterans’ pain impact their sleep and how levels of pain acceptance influence the sleep-pain association. Chronic pain is a global public health problem, affecting 10-25% of the population. Mindfulness is an effective treatment but requires consistency. Because of its benefit, it is important to examine obstacles to mindfulness practice. In order to determine if negative attitudes toward mindfulness are related to non-adherence, 748 adults with chronic pain were recruited to fill out a series of questionnaires assessing treatment adherence and attitudes toward mindfulness. We found that positive attitudes toward mindfulness predicted reduced adherence. However, those who had more positive feelings toward mindfulness made more attempts at the therapy.
