Recent reports cite estimates of more than two million children in the United States who are currently living in kinship care arrangements; 10% or approximately 200,000 of these relationships are children in foster care. Much kinship care is done by midlife and older persons who are finding themselves assuming new responsibilities associated with parenting their grandchildren, typically for a period of two years or more. This is a social phenomenon that cuts across all socioeconomic, racial, and ethnic groups. Some of the most important basic services provided by any level of government are the primary responsibility of municipalities, and urban managers devote more time and attention in making sure that these services are delivered. The availability of public services in any local jurisdiction usually depends on location, history, laws and regulations. Some local governments use alternative service delivery such as public-private partnerships, intergovernmental agreements, and contracting out or privatization. Enacted in 1997, the State Children’s Health Insurance Program (SCHIP) represented the largest expansion of U.S. public health care coverage since the passage of Medicare and Medicaid 32 years earlier. Although the program has recently been reauthorized, there remains a considerable lack of thorough and well-designed evaluations of the program. In this study, we use school attendance as a measure of the program’s impact. Utilizing state-level data and the use of fixed-effects regression techniques, we conclude that SCHIP has had a positive and significant effect on state average daily attendance rates, as measured by both SCHIP participation and eligibility rates. …

Unpredictable yet frequently occurring exception situations pervade clinical care. Handling them properly often requires aberrant actions temporarily departing from normal practice. In this study, the authors investigated several exception-handling procedures provided in an electronic health records system for facilitating clinical documentation, which the authors refer to as ‘data entry exit strategies.’ Through a longitudinal analysis of computer-recorded usage data, the authors found that (1) utilization of the exit strategies was not affected by postimplementation system maturity or patient visit volume, suggesting clinicians' needs to ‘exit’ unwanted situations are persistent; and (2) clinician type and gender are strong predictors of exit-strategy …

This paper reviews out-of-pocket (OOP) medical expenditure measures collected in the Health and Retirement Study (HRS). Medical expenditures are an important cost of poor health. Medical expenditure measures are important for understanding retirement decisions, financial preparation for retirement, and predicting the consequences of health care reform, particularly Medicare reform. Despite the comprehensiveness of the HRS, there are always limitations to what can be learned from population interviews.
