Purpose: The purpose of this evidence-based practice Doctor of Nursing Practice (DNP) project was to review research-based studies on Nola Pender’s Health Promotion Model for weight loss and implement the evidence. The goal was to increase patient self-efficacy, weight-loss, and health promoting behaviors such as improved nutrition and increased physical activity among obese women in a weight loss clinic.

Lifestyle intervention is the first line treatment for obesity. Yet, young men are underrepresented in lifestyle interventions, which is posited to be the result of unmet needs and overall low concern for weight management. The aims of this dissertation are to:

Papers 1 and 2: Test the preliminary efficacy, feasibility, and acceptability of a self-guided lifestyle intervention for young men.

Paper 3: Explore the behavioral and physiological factors associated with changes in adiposity among young men and women enrolled in a more intensive lifestyle intervention.

Background: Obesity among adults is characterized by a body mass index (BMI) of 30 or higher. The impairment and disability resulting from obesity indicates a significant health problem that is often seen in primary care settings. Research indicates that cognitive behavioral therapy (CBT) is an underutilized, but extremely beneficial intervention that may support individuals in achieving weight loss. Adults suffering from obesity with or without comorbid conditions such as psychiatric disorders (e.g. anxiety and depression) have benefitted from CBT.

Obesity is a common medical condition associated with negative health and social outcomes. Obesity has a primary malleable behavioral cause, eating more calories than are metabolized. While metabolic rate is malleable with exercise, eating can more quickly add calories than exercising can subtract them. In the past, behavioral weight-loss treatment studies relied on multi-component package interventions that have shown reliable patterns of participant weight-loss during treatment and weight-regain in follow-up. Those findings could be conceptualized as an ABA withdrawal design, eating behavior returns to baseline after the prosthetic contingencies of the treatment study are withdrawn. Past research has indicated a significant relationship between physical fitness and standardized test scores; however, the relationship between physical fitness and other aspects of school performance has yet to be empirically examined in a population specifically composed of middle school girls. This study examined several factors that contribute to school success, such as classroom behavior, attendance, and grades, in relation to physical fitness among a group of adolescent girls. The purpose of the CHANGE (Coaching towards Healthy Actions Naturally through Goal-related Empowerment) Program was to compare the effectiveness of an interactive versus prescriptive 12-week telephone-based behavioural intervention on the psychological and physiological profiles of university students with obesity. Motivational Interviewing administered using Co-Active Life Coaching (MI-via-CALC) and a structured lifestyle treatment following the LEARN (Lifestyle, Exercise, Attitudes, Relationships, Nutrition) Program for Weight Management were examined.

Depression and obesity are significant public health problems that adversely affect the health and quality of life of women in the United States, particularly female African American welfare recipients transitioning to work. African American women are twice as likely to experience depressive symptoms as women of other races. Fifty percent of African American women are obese. The prevalence of depression and depressive symptoms and obesity is higher among African American women who receive welfare benefits.
