In 2019, the United Nations Industrial Development Organization (UNIDO) launched the “Promotion of Bio-Ethanol as Alternative Clean Fuel for Cooking in the United Republic of Tanzania” project with an aim to promote and distribute 500,000 bioethanol stoves in Dar es Salaam by 2024. The “Proof of Concept” Phase I of the project aimed to sell 110,000 stoves and establish a strong retail market for ethanol fuel. In this study, we conduct an implementation science analysis of Phase I of the project, describing the roll-out of this market-based bioethanol stove program. Indoor air pollution from gas stove combustion remains a public health concern, given links to adverse cardiorespiratory health effects, yet few studies have characterized or compared the air quality impacts of different gas-based cooking fuels. We investigated kitchen-level concentrations of nitrogen dioxide (NO2) and fine particulate matter (PM2.5) concentrations in four homes in Central Pennsylvania that used natural gas and/or biogas fueled stoves. We conducted time-resolved kitchen monitoring and assessed pollutant concentrations during cooking and non-cooking periods. The adaptability of the central nervous system has been revealed in several model systems. Of particular interest to central nervous system-injured individuals is the ability for neural components to be modified for regain of function. In both types of neurotrauma, traumatic brain injury and spinal cord injury, the primary parasympathetic control to the gastrointestinal tract, the vagus nerve, remains anatomically intact. However, individuals with traumatic brain injury or spinal cord injury are highly susceptible to gastrointestinal dysfunctions. Such gastrointestinal dysfunctions attribute to higher morbidity and mortality following traumatic brain injury and spinal cord injury. While the vagal efferent output remains …

Cooking and heating with solid fuels results in high levels of household air pollutants, including particulate matter (PM); however, limited data exist for size fractions smaller than PM2.5 (diameter less than 2.5 μm). We collected 24-h time-resolved measurements of PM2.5 (n = 27) and particle number concentrations (PNC, average diameter 10–700 nm) (n = 44; 24 with paired PM2.5 and PNC) in homes with wood-burning traditional and Justa (i.e., with an engineered combustion chamber and chimney) cookstoves in rural Honduras.

Growing evidence links household air pollution exposure from biomass-burning cookstoves to cardiometabolic disease risk. Few randomized controlled interventions of cookstoves (biomass or otherwise) have quantitatively characterized changes in exposure and indicators of cardiometabolic health, a growing and understudied burden in low- and middle-income countries (LMICs). Ideally, the solution is to transition households to clean cooking, such as with electric or liquefied petroleum gas stoves; however, those unable to afford or to access these options will continue to burn biomass for the foreseeable future. Household air pollution is estimated to be responsible for nearly three million premature deaths annually. Measuring fractional exhaled nitric oxide (FeNO) may improve the limited understanding of the association of household air pollution and airway inflammation. We evaluated the cross-sectional association of FeNO with exposure to household air pollution (24-h average kitchen and personal fine particulate matter and black carbon; stove type) among 139 women in rural Honduras using traditional stoves or cleaner-burning Justastoves. We additionally evaluated interaction by age. Results were generally consistent with a null association; we did not observe a consistent pattern for interaction by age. Evidence …

Growing evidence links household air pollution exposure from biomass cookstoves with elevated blood pressure. We assessed cross‐sectional associations of 24‐hour mean concentrations of personal and kitchen fine particulate matter (PM2.5), black carbon (BC), and stove type with blood pressure, adjusting for confounders, among 147 women using traditional or cleaner‐burning Justa stoves in Honduras. We investigated effect modification by age and body mass index. Traditional stove users had mean (standard deviation) personal and kitchen 24‐hour PM2.5 concentrations of 126 μg/m3 (77) and 360 μg/m3 (374), while Justa stove users’ exposures were 66 μg/m3 (38) and 137 μg/m3(194), respectively. Household air pollution from biomass cookstoves is estimated to be responsible for more than two and a half million premature deaths annually, primarily in low and middle‐income countries where cardiometabolic disorders, such as Type II Diabetes, are increasing. Growing evidence supports a link between ambient air pollution and diabetes, but evidence for household air pollution is limited. Background: This work sought to identify characteristics differing between those with and without muscle cramping during a 161-km ultramarathon.
Methods: In this observational study, race participants underwent body weight measurements before, during, and after the race; completed a post-race questionnaire about muscle cramping and “near” cramping (controllable, not reaching full-blown cramping), drinking strategies, and use of sodium supplementation during four race segments; and underwent a post-race blood draw for determination of serum sodium and blood creatine kinase (CK) concentrations.

Background: Since the landmark study conducted by Huggins and Hodges in 1941, a failure to distinguish between the role of testosterone in prostate cancer development and progression has led to the prevailing opinion that high levels of testosterone increase the risk of prostate cancer. To date, this claim remains unproven.
Presentation of the Hypothesis: We present a novel dynamic mode of the relationship between testosterone and prostate cancer by hypothesizing that the magnitude of age-related declines in testosterone, rather than a static level of testosterone measured at a single point, may trigger and promote the development of prostate cancer.

The third International Exercise-Associated Hyponatremia (EAH) Consensus Development Conference convened in Carlsbad, California in February 2015 with a panel of 17 international experts. The delegates represented 4 countries and 9 medical and scientific sub-specialties pertaining to athletic training, exercise physiology, sports medicine, water/sodium metabolism, and body fluid homeostasis. The mitochondrial protein deacetylase sirtuin (SIRT) 3 may mediate exercise training-induced increases in mitochondrial biogenesis and improvements in reactive oxygen species (ROS) handling. We determined the requirement of AMP-activated protein kinase (AMPK) for exercise training-induced increases in skeletal muscle abundance of SIRT3 and other mitochondrial proteins. Exercise training for 6.5 weeks increased SIRT3 (p < 0.01) and superoxide dismutase 2 (MnSOD; p < 0.05) protein abundance in quadriceps muscle of wild-type (WT; n = 13–15), but not AMPK α2 kinase dead (KD; n = 12–13) mice. We also observed a strong trend for increased MnSOD abundance in exercise-trained skeletal muscle of healthy humans (p = 0.051; n = 6). To further elucidate a role for …

Both hyponatremia and osteopenia separately have been well documented in endurance athletes. Although bone has been shown to act as a “sodium reservoir” to buffer severe plasma sodium derangements in animals, recent data have suggested a similar function in humans. We aimed to explore if acute changes in bone mineral content were associated with changes in plasma sodium concentration in runners participating in a 161 km mountain footrace. Purpose: Noninfectious comorbidities such as cardiovascular diseases have become increasingly prevalent and occur earlier in life in persons with HIV infection. Despite the emerging body of literature linking environmental exposures to chronic disease outcomes in the general population, the impacts of environmental exposures have received little attention in HIV-infected population. High-fat meal consumption alters the circulating cytokine profile and contributes to cardiometabolic diseases. A prior bout of exercise can ameliorate the triglyceride response to a high-fat meal, but the interactive effects of exercise and high-fat meals on cytokines that mediate cardiometabolic risk are not fully understood. Context: Gastrointestinal (GI) distress is common during ultrarunning.
Purpose: To determine if race diet is related to GI distress in a 161-km ultramarathon.
Methods: Fifteen (10 male, 5 female) consenting runners in the Javelina Jundred (6.5 loops on a desert trail) participated. Body mass was measured immediately pre-race and after each loop. Runners reported if they had nausea, vomiting, abdominal cramps, and/or diarrhea after each loop. Subjects were interviewed after each loop to record food, fluid, and electrolyte consumption. Race diets were analyzed using Nutritionist Pro.

Purpose: To determine the effects of 6wk of supplementation with fish oil (FO) on blood pressure and the morning salivary cortisol concentration in normotensive adults.
Methods: Testing was performed following an overnight fast. Subjects (n=40; 35+/-13y, mean+/-SD) rested supine for 40 min, at which time blood pressure and heart rate were measured. Saliva was collected and analyzed for cortisol. Subjects were then randomly assigned to either: 4g/d of Safflower Oil (SO); pr 4g/d of FO supplying 1,600mg/d eicosapentaenoic acid and 800mg/d docosahexaenoic acid. Testing was repeated following 6wk treatment.

In social epidemiology, an individual's neighborhood is considered to be an important determinant of health behaviors, mediators, and outcomes. Consequently, when investigating health disparities, researchers may wish to adjust for confounding by unmeasured neighborhood factors, such as local availability of health facilities or cultural predispositions. With a simple random sample and a binary outcome, a conditional logistic regression analysis that treats individuals within a neighborhood as a matched set is a natural method to use. Purpose:To determine the incidence of exercise-associated hyponatremia (EAH), the associated biochemical measurements and risk factors for EAH, and whether there is an association between postrace blood sodium concentration ([Na+]) and changes in body mass among participants in the 2009 Western States Endurance Run, a 161-km mountain trail run.
Methods: Change in body mass, postrace [Na+], and blood creatine phosphokinase (CPK) concentration, and selected runner characteristics were evaluated among consenting competitors.

Background: Incorporation of fish oil (FO) into the diet of rodents has been shown to result in positive changes in bone health. Currently it is poorly understood if FO has the same effects on bone health in humans. The purpose of this study was to determine the effects of supplemental FO on levels of urinary N-terminal cross-linked telopeptide (NTx), which is a marker of bone breakdown, and how this is related to the morning levels of salivary cortisol and urinary excretion of interleukin 6 (IL-6).

The purpose of this study was to examine pain perception during and following two separate 30-min bouts of exercise above and below the Lactate Threshold (LT). Pain Threshold (PT) and Pain Intensity (PI) were monitored during (15 and 30 min) and after exercise (15 and 30 min into recovery) using a Cold Pressor Test (CPT) and Visual Analog Scale (VAS) for pain of the non-dominant hand. Significant differences in PT scores were found both during and after exercise conditions. Background: To determine the effects of supplemental fish oil (FO) on resting metabolic rate (RMR), body composition, and cortisol production in healthy adults.
Methods: A total of 44 men and women (34 ± 13y, mean+SD) participated in the study. All testing was performed first thing in the morning following an overnight fast. Baseline measurements of RMR were measured using indirect calorimetry using a facemask, and body composition was measured using air displacement plethysmography. In approaching the study of racial discrimination and health, the neighborhood and individual-level antecedents of perceived discrimination need further exploration. We investigated the relationship between neighborhood and individual-level socioeconomic position (SEP), neighborhood racial composition, and perceived racial discrimination in a cohort of African-American and White women age 40-79 from Connecticut, USA.
Design. The logistic regression analysis included 1249 women (39% African- American and 61% White). Neighborhood-level SEP and racial composition were determined using 1990 census tract information. Individual-level SEP indicators included income, education, and occupation. Perceived racial discrimination was measured as lifetime experience in seven situations.

In recent years, numerous studies have established a connection between blood pressure and nocioception. While this connection is well documented in the literature, its underlying physiological mechanisms have yet to be elucidated. Much attention has focused on the relationship between cardiovascular regulatory centers and nocioception, yet the intricacies of this relationship have not been fully explored. Exercise-associated hyponatremia (EAH) is hyponatremia that occurs greater than or equal to twenty four hours after prolonged physical activity. It is a potentially serious complication of marathons, triathlons, and ultradistance events, and can occur in hot and cold environments Clear evidence indicates that EAH is a dilutional hyponatremia caused by excessive fluid consumption and the inappropriate release of arginine vasopressin. Cerebral and pulmonary edema can cause serious signs and symptoms, including altered mental status, respiratory distress, seizures, coma, and death. Rapid diagnosis and urgent treatment with hypertonic saline is necessary to prevent severe complications or death. Prevention is based on …

Background: Birth weight has been identified as a birth-related factor associated with the risk of breast cancer. However, the evidence is inconsistent.
Methods: To investigate the association between birth weight and breast cancer, we conducted a meta-analysis of published studies between 1996 and 2008. Eighteen studies encompassing 16,424 breast cancer cases were included in the meta-analysis. Data were combined using a fixed-effect or random-effect model depending on the heterogeneity across studies.
Results: Women with their own birth weight >4000 g or 8.5 lb had a higher risk for developing breast cancer than those with birth weight(OR¼1.20, 95% CI 1.08, 1.34). …

Background: The purpose of this study was to determine the effects of an acute oral dose of 3 mg/kg of Rhodiola rosea (R. rosea) on endurance exercise performance, mood, and cognitive function.

Ponderal somatograms assessed body compositions in four groups of Division III collegiate football players: offensive line (OL), defensive line (DL), offensive backs (OB), and defensive backs (DB). Ponderal somatograms evaluate body size and shape by converting muscular (shoulders, chest, biceps, forearm, thigh, and calf) and nonmuscular (abdomen, hips knee, ankle, and wrist) girths into ponderal equivalent (PE) values. Anthropometric measurements, including stature, body mass, girths, and percent body fat by densitometry were collected in 82 players (22 OL, 12 DL, 20 OB, and 28 DB) during preseason camp. Background: In this study we assessed whether a liquid carbohydrate-protein (C+P) supplement (0.8 g/kg C; 0.4 g/kg P) ingested early during recovery from a cycling time trial could enhance a subsequent 60 min effort on the same day vs. an isoenergetic liquid carbohydrate (CHO) supplement (1.2 g/kg).

Background: The purpose of this study was to determine the effects of an acute dose of 5 mg/kg of caffeine on perceived pain of the quadriceps during a sustained submaximal isometric contraction.
