Enteric Absorption Of Ciprofloxacin During Tube Feeding In The Critically Ill, Stephen M. Cohn, Mark D. Sawyer, Gerard A. Burns, Concertina Tolomeo, Kerry A. Milner

To determine the pharmacokinetic properties of ciprofloxacin in the critically ill, we studied seven mechanically ventilated patients with pneumonia during entcral feedings. Subjects received ciprofloxacin 750 mg every 12 h via nasogastric tube and serial serum drug concentrations were measured after the first and fourth dose. After the initial dose, the maximum serum concentration ranged from 1.24–3.06 mg/L, and the area under the time curve from 0–12 h ranged from 3.2–19.65 mg.h/L. Similar levels were noted after dose four. Gastrointestinal absorption of ciprofloxacin in tube fed critically ill patients was decreased, but well above MIC values for many pathogenic bacteria.

A Comparison Of The Ovulation Method With The Cue Ovulation Predictor In Determining The Fertile Period, Richard Fehring

The purpose of this study was to compare the CUE Ovulation Predictor with the ovulation method in determining the fertile period. Eleven regularly ovulating women measured their salivary and vaginal electrical resistance (ER) with the CUE, observed their cervical-vaginal mucus, and measured their urine for a luteinizing hormone (LH) surge on a daily basis. Data from 21 menstrual cycles showed no statistical difference (T= 0.33, p= 0.63) between the CUE fertile period, which ranged from 5 to 10 days (mean = 6.7 days, SD = 1.6), and the fertile period of the ovulation method, which ranged from 4 to 9 …

PURPOSE: The purpose of this study was to examine the relationship between the diagonal earlobe crease and the incidence of coronary artery disease.

PATIENTS AND METHODS: A descriptive, correlational, retrospective and concurrent approach was utilized in examining 61 patients, all with a cardiac catheterization within the last six months. All were examined for the presence of diagonal earlobe crease, presence of coronary artery disease, total blood cholesterol levels, age, sex, and past medical histories. Analyses included the Fischer's exact test, regression analysis, and multiple logistic regression analysis.

Explication Of The Construct Of Shared Care And The Prevention Of Pressure Ulcers In Home Health Care, Margaret Sebern

The purpose of this investigation was to render a more complete understanding of subjective perceptions of pressure ulcers from the perspective of family dyads, and to study the effect of these subjective experiences on preventive behaviors and pressure ulcer outcomes. A naturalistic inquiry, combined with objective measures, was used. Twenty-one dyads participated in four in-depth interviews to explore how they mentally represented and responded to the risk of pressure ulcers. Through the process of concept development, a lay representation of pressure ulcers was developed. This process produced a new concept, identified as “shared care,” that explained how the dyads interaction …

Exploring The Boundaries Of Pediatric Practice: Nurse Stories Related To Relationships, Joan P. Totka

Pediatric nurses struggle to find the right level of involvement with children and families. The purpose of this study was to illuminate nurses' struggles and insights as they learned to find the interpersonal boundaries of their own practice. The phenomenological method of Heideggerian hermeneutics was used to analyze data from audiotaped and transcribed single interviews of five pediatric nurses. The analysis of the transcriptions involved multiple stages of interpretation using a research team. Relation Of Health Related Hardiness To Health Perception And Psychosocial Adaptation In Adult Hispanics With Chronic Hepatitis C, Darlene M. Boytell

The purpose of this study was to investigate the role of hardiness in health perception and psychosocial adaptation in adult hispanics with chronic hepatitis C (n = 32). The Health Related Hardiness Scale and the Psychosocial Adaptation to Illness Scale were administered to 32 adult hispanics diagnosed with chronic heptitis C at a gastroenterology center. The results indicate that a comparison of subjects with low and high hardiness scores did not reveal significant differences on any of the PAIS domains (health care orientation, sexual relationships, psychological distress, vocational, domestic and social environments). The Role Of Group Activity Participation In Depression Among Institutionalized Elderly, Amparita L. Cabrera

The role of group activity participation in depression among a group of residents (N=65), age 80 and older, in a nursing home was examined using the framework of Roy's Adaptation Theory and Nolen-Hoeksema's Response Style Theory of Depression. Roy views depression as a maladaptation. Nolen-Hoeksema views group activity participation as a therapeutic distraction to break depressed moods and thus allow for positive adaptation.

This study utilized data from medical records, group activity attendance, and self-report questionnaires. Perceptions Of Nurses And Clients Related To Empathic Interactions, Patricia Christine Calhoun

The definition of empathy is generally agreed upon, although its precise nature and characteristics within a nurse-client relationship are unclear. The perceptions of nurses and clients related to empathic interactions were examined using hospitalized clients and their direct care nurses. A phenomenological approach was chosen for research design and analysis.

Data for the study were collected by interviewing the nurses and clients individually. The interviews were recorded and transcribed. The data were analyzed using the seven step Colaizzi method.

The findings of the study indicate that nurses and clients have a multi-dimensional view of empathy. There is agreement that there …

Coronary artery disease (CAD) remains a leading cause of mortality and morbidity in the United States. CAD can be a long term, recurrent health problem placing a heavy burden on the afflicted individual. This paper is posted to assist graduate nursing students and others who are interested in my early PNI theory development work. The paper documents development and initial testing of a midrange theory of the effects of various nursing interventions on the psychoneuroimmunological (PNI) network. The hypothesized effects of interventions on various immune factors and subsequent health outcomes are explored, with special interest given to the use of PNI theory in nursing. The purpose of this qualitative study was to investigate the anger of individuals who have provided direct patient care to loved ones who are living with or who have died from AIDS. The sample consisted of eight individuals who are currently involved in an AIDS service organization in northeast Tennessee. Data were collected using semistructured taped interviews. At the beginning of the study, two group interviews were conducted. Subsequently, each of the participants was interviewed privately. Knowledge Of Risk Behaviors Of Persons Seeking Hiv Antibody Testing At A Community Site, R.L. Sowell, B. Seals, Kenneth D. Phillips

People seeking HIV antibody testing at a community-based AIDS service organization (N = 342) were asked to complete a self- report questionnaire to determine knowledge of HIV transmission, source(s) of HIV-related knowledge, and recent risk behaviors. The overall knowledge level of risk for transmitting HIV associated with 16 related behaviors was relatively high. Yet, 69% of the participants reported engaging in unsafe sex during the previous six months. Poorly informed subjects tended to overestimate their level of knowledge. In a multiple regression analysis, knowledge, age, and gender were the only significant predictors of engaging in unsafe sex in this cohort. …

Until recently, reverse transcriptase inhibitors have been the mainstay for treating HIV infection. Now, three protease inhibitors have been approved. Early evidence suggests that triple therapy delays the progression of HIV infection. An understanding of these new drugs and treatment strategies is imperative for clinicians. This article (1) reviews HIV's structure and replication process, (2) discusses currently approved reverse transcriptase and protease inhibitors, (3) describes the antiretroviral drugs" modes of action, (4) discusses important nursing implications for monitoring clients on these drugs, and (5) presents current scientific evidence regarding the effectiveness of combination therapy strategies.
