End-stage renal disease (ESRD) is a prevalent condition among the elderly population in the United States. ESRD is the most feared stage of chronic kidney disease (CKD), as it is a terminal diagnosis requiring long-term management. A quazi-experimental, one-group, pretest/posttest study was conducted with a group of 25 hospice workers employed by a medium sized county hospice organization in the southeastern United States that was experiencing rapid personnel turn-over. Participants in the study included a doctor, a physician’s assistant, a nurse practitioner, a massage therapist, a grief counselor, a licensed practical nurse, a certified nursing assistant, two clergy, three administrative staff, three social workers, seven volunteers, eight registered nurses and one other. Rib fractures in the elderly population, 65 years and older, is more prevalent than any other population. Mortality increases with each year over the age of 65 and with each number of ribs fractured. Rib fractures are painful and can lead to a decreased respiratory effort. This results in complications such as pneumonia, respiratory depression, respiratory failure, Acute Respiratory Distress Syndrome (ARDS), and respiratory infections. Non-Invasive Positive Pressure Ventilation (NIPPV) assists with reducing the patient efforts to breathe while also ensuring spontaneous lung expansion. As hospice programs gain recognition, hospice referrals are gaining momentum. Yet for multiple reasons hospice referrals are challenging amongst rural healthcare providers (HCPs). For rural healthcare providers, lack of knowledge about hospice services and care contributes to a decrease in hospice referrals. The purpose of this capstone project was to develop an intervention to improve rural health care providers' behavioral intention to make hospice referral. This study was completed to help determine if nurses had a true understanding of how to provide pain management to patients in their care who were at the end of their life. A review of literature was completed and it revealed that nurses have a fear of opioid overdose and respiratory depression in their patients when providing proper pain management. In response to the Office of Inspector General's research report on atypical antipsychotic off label treatment of elderly dementia residents in long-term care, the Centers for Medicare and Medicaid Services developed a non-pharmacological intervention known as the Hand In Hand training tool. This projects focus was on training the direct care nursing staff with the Hand In Hand tool and evaluating for decrease in behavior and psychological symptoms in their patients. Constipation is a prevalent symptom in the hospice population, negatively effecting quality of life for patients. Constipation is frequently overlooked, undiagnosed, and untreated by healthcare providers. The role of the hospice nurse is ideal for managing the symptom of constipation with traditional and alternative therapies. Literature review reveals very limited research on constipation and hospice patients, and alternative therapies for constipation. A descriptive study was initiated to investigate the use of peppermint oil to reduce the nausea of the palliative care and hospice patient. Two local palliative care and hospice programs were used as sites for the study. Patients eighteen and older complaining of nausea were invited to participate in the study. The study was conducted for one and a half months. A total of eight patients consented to participate in the study with a mean age of seventy one years.
