Background: Diabetes Mellitus (DM) is the 7th leading cause of death in the United States (HHS ODPHP, 2019). It is also the leading cause of chronic disease complications such as adult-onset blindness, kidney failure, and lower-limb amputations (HHS ODPHP, 2019). It is recommended that all people with diabetes should participate in diabetes self-management education (ADA, 2018). Primary care practices that do not utilize electronic medical records (EMR) could pose difficulty in adhering to clinical guidelines for diabetic patients. Diabetes flow sheets are a one page document that includes current practice guideline recommendations for easy access of results to promote comprehensive care. The main objective of this project was to promote adherence to diabetes guidelines with the use of a diabetes flow sheet for providers that do not utilize an EMR.
