Heart failure is a chronic health problem. Heart failure is costly for society, resulting in high morbidity and mortality which accounts for large public spending on this disease. Heart failure (HF) management is complex and requires coordination between patients, families, and their health team members. Self-management (SM) of HF is an important component of chronic disease management and, when done well, aids in preventing HF exacerbations and unnecessary hospitalizations. Although the subpopulation of chronically unsheltered homeless women is a small percentage of the overall homeless population, this vulnerable group of women is very high risk for poor health status and negative health outcomes. Unsheltered homeless women are significantly higher risk for assault, physical/mental disorders, and substance/ alcohol abuse compared to women residing in shelters or transitional housing. Advances in treatment technology and the importance of obtaining normoglycemia in order to prevent or delay complications associated with Type 1 Diabetes Mellitus (T1DM) has shifted much of the emphasis of diabetes self-management (SM) onto the adolescent and his or her family. The primary responsibility for managing T1DM in childhood is with the parent whereas during adolescence, increasing levels of responsibility for SM are transferred to the adolescent.
