Local Anesthetic Systemic Toxicity: Last Protocol Development, Implementation, And Evaluation For Healthcare Providers, Rachel Vaughn
Doctor of Nursing Practice Projects
LAST is an uncommon but deadly complication of local anesthetic administration. Healthcare facilities benefit from protocol implementation to help streamline best evidence-based practice. The American Society of Regional Anesthesia and Pain Medicine warrants a LAST emergency protocol for any healthcare facility practicing regional anesthesia. Healthcare personnel encountering patients at risk for LAST must understand the symptoms and treatment of LAST to improve response time and patient outcomes. The project implemented a LAST protocol at a rural hospital in Southern Illinois in July 2023. The investigator held an educational service to discuss the new LAST protocol and implementation. The educational service …

Cancer is the leading cause of death for incarcerated individuals, prostate cancer awareness and screening guidelines are essential areas of focus for healthcare professionals (Puglisi et al., 2021). Prostate cancer screenings recommend that males between the age of 55-69 be informed of evidence-based guidelines and offered these screenings as a part of routine health maintenance (USPSTF, 2018). In 2019, the Illinois Department of Corrections (IDOC) was ordered to follow these guidelines for incarcerated individuals (Raba, 2021). As of 2021, the IDOC had yet to implement these screenings for incarcerated individuals within the state facilities. Two separate one-hour educational sessions regarding …

Enhanced Recovery After Surgery (Eras) Protocol For Bariatric Surgery, Krista Bunch
Doctor of Nursing Practice Projects
Bariatric surgery has been utilized in treating severe obesity and offers several benefits, such as sustainable weight loss, treatment of various metabolic comorbidities, and improved life expectancy (Sundbom et al., 2017). With these benefits and procedural improvements, there has been a marked increase in the demand for bariatric surgery and increased interest in shifting some of these procedures to an ambulatory care setting (Stenberg et al., 2022). However, postoperative complications such as pain, nausea, and inability to tolerate oral intake can cause patients to experience an increased length of stay in the hospital and suboptimal outcomes (King et al., 2018). …
Short Term Medical Mission Gerd And Ulcer Treatment Guideline, Robin Risky
Short Term Medical Mission Gerd And Ulcer Treatment Guideline, Robin Risky
Doctor of Nursing Practice Projects
The United States conducts over 6,000 international medical mission trips each year. The short-term nature of these trips, unfamiliarity with the patient population, language barriers, limited resources, and inadequate staff training are just some of the factors that make it challenging to provide quality care. One way to avoid doing harm is to follow evidence-based guidelines when treating common conditions. This quality improvement project sought to provide an evidence-based protocol for short-term medical missions (STMMs), guiding providers and pharmacy staff on the treatment of GERD and peptic ulcer. This project was implemented in a rural health clinic at the Land …
Implementation Of A Tele-Triage Protocol In An Emergency Department Setting, Megan Tuetken, Sierra Hamlin, Sydney Spaulding
Implementation Of A Tele-Triage Protocol In An Emergency Department Setting, Megan Tuetken, Sierra Hamlin, Sydney Spaulding
Doctor of Nursing Practice Projects
Nationwide, the volume of emergency department (ED) visits has steadily increased over the last several decades. This steady increase in volume frequently causes ED crowding and considerable delays in patient evaluation and treatment. Recently, teletriage has been used in emergency departments to evaluate less emergent patient needs while freeing up in-house providers to care for more critical patients. The goal of this DNP project was to develop a protocol for the utilization of teletriage in an Emergency Department in a level 1 trauma center in central Illinois using nurse practitioners to assess patients in the waiting room and order appropriate …

Human immunodeficiency virus (HIV) is an incurable condition that continues to be a public health threat. Approximately 38 million people live with the disease worldwide, and one out of seven are unaware they have the disease. Pre-exposure prophylaxis (PrEP) is a medication that can be prescribed to individuals at risk of acquiring HIV. PrEP can be prescribed by any healthcare provider who deems a patient at risk for HIV. However, a primary care practice in central Illinois previously had no protocol for initiation of PrEP. This quality improvement (QI) project aimed to implement a PrEP protocol in the department of …

Improving Depression Surveillance At The We Care Clinic, Kaitlin Springer, Emma Hanney
Doctor of Nursing Practice Projects
Depression is one of the most underdiagnosed and untreated disorders affecting more than 16 million people in the United States each year (Henry et al., 2020). Unmanaged depression can lead to emotional suffering, decreased productivity, reduced income, impaired relationships, and an increased risk for comorbidities (Sinisalchi et al., 2020). It can affect anyone, however, individuals who are non-white, female, undereducated, disabled, pregnant or postpartum, have low socioeconomic status, or lack social support are at an increased risk (United States Preventative Services Task Force, 2019). The Southern Illinois University Edwardsville We Care Clinic provides primary care to minority patient populations who …
Development Of A Protocol For Perioperative Management Of The Breastfeeding Mother, Kayla Nyseth, Kelsey Park
Development Of A Protocol For Perioperative Management Of The Breastfeeding Mother, Kayla Nyseth, Kelsey Park
Doctor of Nursing Practice Projects
Facilitating breastfeeding is a public health priority, as disruption in lactation is associated with adverse health outcomes for the breastfeeding mother and child (ACOG, 2016). The need to take medication, maternal illness, and hospitalization are among the most common reasons women cite for ceasing breastfeeding before their projected goal (Odom, Li, Scanlon, Perrine, & Grummer-Strawn, 2013). Providing a healthcare team that encourages breastfeeding during the perioperative period can minimize interruptions in breastfeeding and optimize the benefits for both the mother and infant (Simon, Carabetta, Rieth & Barnett, 2018). Unfortunately, many anesthesia providers lack confidence related to breastfeeding and drug transfer. …

Developing A Ketamine Infusion Protocol To Treat Complex Regional Pain Syndrome, Nikolas M. Kozul
Doctor of Nursing Practice Projects
Complex Regional Pain Syndrome (CRPS) has been studied under different names dating back to the American Civil War. Despite being studied for over 100 years, CRPS is still not a very well understood disease process and there is no consensus on the best way to treat it (O’Connell, Wand, McAuley, Marston, & Moseley, 2016). One treatment that has shown to be effective in reducing pain scores in patients with chronic CRPS is a multiple day infusion of ketamine (Sigtermans et al., 2009). The purpose of this doctor of nursing practice project was to develop a ketamine infusion protocol for anesthesia …

Sugammadex Protocol, Brenna M. Thomas
Doctor of Nursing Practice Projects
Sugammadex is a new medication used for the reversal of muscle relaxation induced by rocuronium, vecuronium, and pancuronium. Sugammadex is available for use at St. Anthony’s Memorial Hospital in Effingham, IL. However, they did not have a protocol regarding the use of this medication. A sugammadex protocol was written for the operating room (OR) staff after a thorough, evidence based literature review was completed. The OR staff included certified registered nurse anesthetists, anesthesiologists, registered nurses, and pharmacy. The protocol was implemented and evaluated with a pretest, an educational PowerPoint regarding sugammadex, and a posttest. A paired samples t test was …
