The Effect Of Heart Failure Education On Reducing Readmissions, Mayola Lara Villarruel
The Effect Of Heart Failure Education On Reducing Readmissions, Mayola Lara Villarruel
Evidence-Based Practice Project Reports
Management of heart failure is a significant financial challenge for the health care industry, costing approximately $33.2 billion annually. Common reasons for preventable heart failure readmissions include inadequate discharge education and lack of self-care and health management activities. Education at discharge is a vital component of improving heart failure outcomes. Following a review of the literature, high quality evidence supports that heart failure education should focus on medication adherence, sodium and fluid restriction, daily weights, activity tolerance, identification of deteriorating signs and symptoms of heart failure, and smoking cessation. The purpose of this EBP project was to reduce heart failure …
Contributing Factors To Hospital Readmissions, Susan M. Amalfitano
Contributing Factors To Hospital Readmissions, Susan M. Amalfitano
Master's Theses, Dissertations, Graduate Research and Major Papers Overview
Aims and objectives. To investigate the elderly patient’s perspective about the reasons for discharge to home being unsuccessful, resulting in rehospitalization. Background. Elderly patients have a high rate of readmission to the hospital within 30 days of discharge. Starting in October of 2013, hospitals having a high rate of readmission of patients with heart failure (HF), myocardial infarction (MI), and pneumonia (PNA) will face financial penalties. Evidence indicated that by utilizing risk prediction tools and incorporating transition interventions the risk of a hospital readmission may be reduced. Design. A qualitative, descriptive design was used. Methods. In 2013, a student investigator …
Booking The Appointment: A Strategy To Reduce Readmissions In Heart Failure Patients, Lindsey Moran
Booking The Appointment: A Strategy To Reduce Readmissions In Heart Failure Patients, Lindsey Moran
Master's Theses, Dissertations, Graduate Research and Major Papers Overview
Heart failure (HF) is a chronic condition that is the cause for many hospitalizations in the United States. Hospital readmission is a common problem in many chronic conditions, especially heart failure. The purpose of this research was to determine if scheduling a follow-up appointment with a primary care provider (PCP) or cardiologist prior to hospital discharge decreases 30-day readmission rates in patients with a primary diagnosis of HF. A quasi-experimental, two-group study was performed at The Miriam Hospital, a 247-bed acute care hospital, with a sample of 60 patients. A retrospective chart audit was performed to determine if 30-day readmission …
The Use Of Motivational Interview Intervention In Discharge Teaching Of Patients With Heart Failure, Jason O'Brien
The Use Of Motivational Interview Intervention In Discharge Teaching Of Patients With Heart Failure, Jason O'Brien
Jason O'Brien
It is widely recognized in healthcare that there are many different behavioral choices a patient can make that can have an impact on their health and disease management outcomes. One of the more prevalent diseases that requires consistent compliance is heart failure. The literature suggests that the different areas of management are often something the layperson in the home can have control over, provided they have the confidence and conviction to do so. There is a call for a new way of teaching these patients upon hospital discharge as the old way of lecturing instruction is not effective. It has …
Early Home Visits By A Registered Nurse Care Manager With Heart Failure Patients, Dana Davis Blake
Early Home Visits By A Registered Nurse Care Manager With Heart Failure Patients, Dana Davis Blake
Nursing Theses and Capstone Projects
Strategies focused on 30 days in the life of a patient with heart failure will have limited impact on the burden that heart failure will have nationally or individually. The broader landscape of readmission risk underscores the need for a more comprehensive approach to heart failure management. Care management with a registered nurse demonstrated efficiently coordinate care. Home visits address the peaks of risk in the post-discharge transition and palliative phase while providing longitudinal support. The purpose of this study is to substantiate that early home visits and telephone followup with a registered nurse after discharge from the hospital will …
Intergrative Review Of Palliative Care In End Stage Heart Failure, Joyce K. Kutin
Intergrative Review Of Palliative Care In End Stage Heart Failure, Joyce K. Kutin
Joyce K Kutin RN, MSN, MOL
The aim of this integrative literature review is to explore and discuss palliative care placement within the trajectory of heart failure in the end stage process. After an extensive search through 200 peer-reviewed studies published from 2009-2013 in the following databases: CINAHL, Academic Search Elite, Health Source Consumer Source Edition, Health Source: Nursing/Academic Edition, MEDLINE, Academic Collection (EBSCOhost), seven articles meeting the constraints were chosen.. Common themes of these studies concern symptom management, medication administration, and decision-making tools for assessing patient centered needs and future research regarding effective implementation of palliative care integration in end stage heart failure patients. Nurses …
