The Effect Of Longitudinal Changes In Rn Specialty Certification Rates On Total Patient Fall Rates In Acute Care Hospitals, Diane Kay Boyle Phd, Rn, Faan
Diane Kay Boyle PhD, RN, FAAN
Background: Researchers have studied inpatient falls in relation to aspects of nurse staffing, focusing primarily on staffing levels and proportion of nursing care hours provided by registered nurses (RNs). Less attention has been paid to other nursing characteristics, such as RN national nursing specialty certification.
Objective: To examine the relationship over time between changes in RN national nursing specialty certification rates and changes in total patient fall rates at the patient care unit level.
Method: We used longitudinal data with standardized variable definitions across sites from the National Database of Nursing Quality Indicators®. The sample consisted of 7,583 units in …

Commercial Medical Transportation - The Better Alternative To Air Ambulance, Jime Carlo

From a patient's perspective, when there are at least a few days to make arrangements, commercial travel is a viable [and often superior] and cost effective alternative to air ambulance transport. When all the necessary pre-planning, clearances and pre-arranging are done by a third party, and there is a medical escort to navigate the logistics of travel and provide in-flight medical care, the entire transport experience is usually more positive for patients choosing commercial medical transport over a traditional air ambulance.

[Excerpt] An important element in president-elect Obama's economic stimulus proposal is his plan to invest a significant proportion of federal dollars in installing electronic medical records (EMR) in U.S. healthcare institutions. In emphasizing the need for EMR, Obama is following the advice of numerous healthcare experts who have pointed out that the healthcare sector lags behind other industries in the use of computer technology. They believe the widespread use of EMR would help reduce medical errors, control the costs of healthcare, and lead to significant improvements in the quality of care Americans receive. In this paper we present preliminary results …

Retention Of New Graduate Gen Y Nurses, Joyce K. Kutin
Joyce K Kutin RN, MSN, MOL
The purpose of this paper is to review the research of new residency programs on Generation Y retention and the quality implications that influence the organization. General orientation of all nurses upon hire has been the standard practice with preceptoring of senior nurses from the individual units. The process of each unit’s orientation and standards was different without respect to the individual nurse’s experience. Prior to the implementation of the Nurse Residency program for new graduate generation y nurses, the turnover rate within the first year was greater than 50 percent. Health care organizations and their leaders need to anticipate …

Evolving more than thirty years ago, the Diagnosis Related Groups (DRGs) covers a wide variety of applications that impact all health care segments throughout the United States. The DRGs are a patient classification scheme originally developed as a means of relating the type of patients a hospital treats (i.e., casemix) to the costs incurred by the hospitals.
The DRG terminology has experienced evolutionary growth through later generations: Major Comorbidities and Complications (MCC), Comorbidities and Complications (CC) and All Patient Refined Diagnosis Related Groups (APR DRG). This process directly impacts the reimbursement or payment re-distribution by Medicare and Medicaid.

The Bureau of Labor Statistics for the year 2000 stated that Baby Boomers (age thirty-six through fifty-four) represented fifty percent of the United States labor force while Generation X (age twenty to thirty-five) represented some thirty-three percent, a significant decline in workforce. This demographic, time bomb indicates the urgency for many organizations in developed countries to prepare for and cope with the imminent retirement of their aging workforce.

Leaders Of Today Are Challenged By Generation X Workforce Retention, Joyce K. Kutin
Joyce K Kutin RN, MSN, MOL
Many organizations today are comprised of a culturally diverse workforce. In addition to organizational diversity related to gender, ethnicity, race, or religious beliefs, there are also four different generations working side by side. Today’s workforce includes the Silent Generation, the Baby Boomers, Generation X, and Generation Y. The Silent Generation is known for their lifestyle approach of consistency, uniformity, conformity, law and order, hard work and playing by the rules. Baby Boomers were the first generation to grow up with television; they were the first to have broad access to news, issues, advertising, and a variety of programming. Generation X …

Reminder Systems To Reduce The Duration Of Indwelling Urinary Catheters: A Narrative Review, Thomas Blodgett

Catheter-associated urinary tract infection (CAUTI) is a common and costly problem for hospitalized patients. Policymakers have taken notice of the importance of these infections, and changes to the prospective payment rules of Medicare, Medicaid, and many additional third-party payers have been implemented to hold hospitals accountable for the delivery of poor quality health care services. As key members of the health care team, nurses must be prepared to utilize evidence-based practices to prevent CAUTI in hospitalized patients. This article describes several variable-technology interventions to remind clinicians to remove unnecessary urinary catheters and proposes potential roles for nursing informatics in the …
