Purpose: Home visits, once a popular but now uncommon form of health care delivery, are on the rise. Few studies have focused on the value the experience brings to resident physicians and their patients.
Methods: A 6-month pilot was conducted with 11 residents who participated in 32 home visits with 11 patients. Patient and resident experiences were captured through a survey following the home visits.
Results: In all, 100% of patients and a majority of residents were very interested in being a part of and incorporating future home visits, respectively. Every patient in the survey said that the visits resulted …

Severe stress reactions in female patients presenting for annual pelvic examination may occur for a variety of reasons, including previous sexual assault, post-traumatic stress disorder, and /or intimate partner violence. The clinician's approach to care delivery is critical for establishing trusting relationships with female patients exhibiting these types of reactions. A case study is used to describe assessment and communication techniques to improve care delivery.

High-risk alcohol use occurs on college campuses across the United States. A 2008 Core Alcohol and Drug Survey performed on the campus of Indiana University-Purdue University Fort Wayne (IPFW) in northeast Indiana, found an annual prevalence rate of alcohol consumption by students of 82.3%. A commitment to reduce high-risk alcohol use by IPFW students is supported by IPFW leadership in congruence with Healthy Campus objectives. A translational research project using the Alcohol Use Disorders Identification Test (AUDIT) was implemented at IPFW’s nurse-managed health and wellness clinic in order to increase student awareness of high-risk drinking behaviors, administer risk-reducing interventions, and …

A variety of barriers contribute to delays in accessing health care in Indiana. Medically underserved areas and health professional shortage areas further potentiate barriers creating additional delays. Reduction in barriers to access is critical for promoting optimal health for Hoosiers. Advanced practice nurses may be the answer for reducing access dilemmas by developing nurse-managed health centers and by maintaining active employment in Indiana's Community Health Centers.

The aim of this integrative literature review is to explore and discuss palliative care placement within the trajectory of heart failure in the end stage process. After an extensive search through 200 peer-reviewed studies published from 2009-2013 in the following databases: CINAHL, Academic Search Elite, Health Source Consumer Source Edition, Health Source: Nursing/Academic Edition, MEDLINE, Academic Collection (EBSCOhost), seven articles meeting the constraints were chosen.. Psychological Distress In African American Grandmothrs Raising Grandchildren: The Contribution Of Child Behavior Problems, Physical Health, And Family Resources, Susan Kelley, Deborah Whitley, Peter Campos

Diminished psychological health has been identified among caregiving grandmothers. The intent of this investigation was to examine psychological distress levels, as well as their predictors, in a sample of 480 caregiving African American grandmothers, mean age 56 years. Almost 40% (39.8%) of participants had clinically elevated psychological distress scores. Results of hierarchical multiple regression indicated that internalizing and externalizing child behavior problems, poor grandmother physical health, younger age of grandmother, and lack of family resources predicted 31% of the variance in psychological distress. Results provide direction for nursing interventions aimed at enhancing the psychological well-being of caregiving grandmothers.

The HIV epidemic in the United States has disproportionately affected young racial/ethnic minority men who have sex with men (YMSM). However, HIV testing rates among young men of color remain low. Within this sample of racial/ethnic minority YMSM (n = 363), the first HIV test was a median of 2 years after men who have sex with men sexual debut. Individuals with less than 1 year between their first negative and first positive HIV test were significantly more likely to identify the reason for their first negative test as being sick (OR = 2.99; 95 % CI 1.23-7.27). Recent findings highlight the continued rise in cases of HIV infection among racial/ethnic minority young men who have sex with men (YMSM). In adults, disclosure of HIV status has been associated with decreased sexual risk behaviors but this has not been explored among YMSM. In this study of 362 HIV-infected racial/ethnic minority YMSM, rates of disclosure were high, with almost all disclosing their status to at least one person at baseline. African American Caregiving Grandmothers: Results Of An Intervention To Improve Health Indicators And Health Promotion Behaviors, Susan Kelley, Deborah M. Whitley, Peter Campos

The purpose of this study was to examine the efficacy of an intervention to improve the health of grandmothers raising grandchildren. A pre- and post-test design was employed with 504 African American grandmother participants. The intervention included monthly home-based visitation by registered nurses (RNs) and social workers, participation in support groups and parenting classes, referrals for legal services, and early intervention services for children with special needs. The Health Risk Appraisal was used to assess health indicators and health promotion behaviors. A comparison of pre- and post-test scores indicated significant (p < .002) changes in the desired direction for a number of health indicators and health promotion behaviors, including blood pressure, annual routine cancer screenings, frequency of weekly exercise, and improved dietary intake, as well as participants’ perception of their health and life satisfaction. No improvements were observed in the proportion of participants who were obese or overweight.

Bullying is a form of violence characterized as an aggressive behavior that is unprovoked and intended to cause harm. Prior studies have found that lesbian, gay, bisexual, and transgender (LGBT) youth experience high levels of bullying related to their sexuality and this harassment can lead to engagement in risk behaviors, depression, and suicide. The average reported age of sexual debut for youth in the United States is 14.4 years, with approximately 7% reporting their sexual debut prior to age 13. While the research literature on sexual debut for youth addresses gender and ethnic differences (with males and African-American youth experiencing earlier sexual debut), there is limited data regarding factors associated with sexual debut for young men who have sex with men (YMSM). Early sexual debut poses potential health risks, such as contracting HIV with an increased risk of unprotected intercourse.
