Background: Fluid overload in patients requiring intensive care leads to increased costs for hospitals and patients, increased length of intensive care unit (ICU) and total hospital stay, ventilator days, acute kidney injuries, and mortality rates. Multiple studies have indicated that aggressive de-resuscitation with diuretics can decrease length of stay, ventilator days, organ injury, and mortality rates. A nurse-driven diuresis protocol utilizing intravenous (IV) push furosemide was introduced to the University of Kentucky Medicine ICU, but patients are still commonly still over-resuscitated with no plan to diurese until organ damage has occurred. Psychological distress is when patients experience anxiety, depression or post-traumatic stress disorder related to ICU admission. With the introduction of the COVID-19 pandemic, this psychological distress has become more of a problem with patients and current nursing practices are falling short in providing quality psychological care. Under-recognition by healthcare providers poses a barrier and contributes to poor patient outcomes, including increased morbidity and mortality.

Epidural analgesia (EA) is one inpatient pain management option prescribed by Acute Pain Services (APS) at VA Puget Sound Health Care Services (VAPSHCS). Historically, EA nursing care at VAPSHCS has been managed by surgical units. However, in the last two years, facility restructuring and budget constraints have led to reorganization and consolidation of units. The effect of a cardiac education program in terms of changes in knowledge, state anxiety and stress were examined for persons suffering a myocardial infarction for the first time. In addition, the relationship between stress and trait anxiety and changes in knowledge and state anxiety were examined. A sample of 20 patients were selected from two medium sized hospitals in Southeastern Virginia. Statistically significant differences at the .01 level were found between the means of the pretest and posttest scores of the experimental group for knowledge and state anxiety using the t test for dependent groups.
