Timely identification is crucial for effective intervention and improved outcomes. Atrial fibrillation, also known as A-Fib or AF, is a cardiovascular abnormality that is characterized by an abnormal heart rhythm or muscle contractions in the atria. These abnormalities are a result of disorganized electrical impulses. AF has various causes including, but not limited to, hypertension (HTN), heart surgery, thyroid disorders, heart defects, and acute alcohol ingestion. Persons with AF are often asymptomatic; however, some patients report heart palpitations, chest tightness, shortness of breath, and lightheadedness. A review of literature was conducted in order to determine the availability of risk factors, patient education, and treatment of AF.

Early diagnosis can prevent further comorbidities such as heart disease, stroke, and chronic kidney disease. The purpose of this quality improvement project was to use evidence-based practices to determine if personalized education on disease self-management would lead to improved treatment adherence in a residential facility. The target population consisted of four elderly female patients who had been diagnosed with congestive heart failure (CHF) and were struggling with the self-management of their disease. Contributing barriers to effective CHF self-management were determined using a root cause analysis, and included a lack of educational templates, declining cognitive ability of the residents, and no electronic health record (EHR). Background: Obesity is a preventable epidemic and costs the United States $200 billion annually. The leading causes of death are linked to obesity.

Local Problem: In a chart review, 53% of patients at a northeast Louisiana primary care clinic were obese. However, none were offered comprehensive lifestyle intervention. The aim was to increase the percentage of patients with BMI < 30 kg/m2 to 60% over 8 weeks.

Background: Without proper education, heart failure (HF) is very difficult to manage. A lack of education and proper management of HF can lead to frequent hospital readmissions, decreased quality of life, and early morbidity and mortality.

Objective: Through literature review, this paper aims to identify the benefits of implementing telehealth education for HF patients in order to decrease hospital readmissions.
