The goal of the dissertation is to undertake an analysis of substance use disorders that focuses on a public health ethics perspective. The ethical argument focuses upon justifying the use of harm reduction and is primarily concerned with the current opioid crisis. While substance abuse/misuse over the course of history has been identified as a public health concern, this dissertation presents substance use disorders over the course of the lifespan, examining various contexts in which it can affect daily living and health outcomes. In response, we created a consortium of recovery programs, tribal partners, social scientists, substance use disorders counselors, and treatment specialists to target prevention, treatment and recovery programming efforts in Tooele County, Utah.

In 2016, the CareLink New Mexico behavioral health homes program began enrolling Medicaid recipients with the goal of increasing care coordination, improving access to services, and decreasing long-term costs of care for adults with serious mental illness (SMI) and children with severe emotional disturbance (SED). To evaluate these aims, a retrospective interrupted time series study using Medicaid claims data was designed. First, a comparable subset of non-enrolled individuals was selected from the pool of Medicaid recipients with SMI or SED using propensity score matching. The use of alcohol and other harmful substances, such as cocaine, opioids, and marijuana during pregnancy has increased over the last decades (Sebastiani et al., 2018). Using these substances can be very harmful to the unborn baby and mother (Viteri et al., 2015). This fact sheet covers (1) the consequences of substance use and (2) the current recommendations for substance use during pregnancy.

The incarceration rates in the United States are the highest in the world. Within the U.S. prison population, mental illness is overrepresented as compared to the general population. The present study examined existing literature that researched the connection between the prison system and mental illness, and the potential solutions to this crisis. The studies looked at focused on psychiatric disorders, substance disorders, and trauma. They also focused on the intersection between race, mental illness, and the prison system, and the intersection between mental illness, gender, and the prison system. Abstract

The United States is in an opioid epidemic. The comitant use of opiates and benzodiazepines can ultimately result in death. This project consisted of a quality improvement approach addressing the continued need to educate prescribers on coprescribing benzodiazepines and opioids to the adult population. A qualitative and quantitative descriptive analysis was utilized for data collection. The results will assist in determining if accessing the Prescription Drug Monitoring Program (PDMP) and utilizing evidence-based guidelines before prescribing treatment compared to the nonutilization of the PDMP and evidence-based guidelines aid in the reduction of mortality and overdose rates within a three-month period. …

The Centers for Disease Control report that the rate of overdose deaths among women is rapidly increasing, with women experiencing a 532% increase in fatal opioid overdose deaths between 1999 and 2017, compared to a 355% increase for men. Among women, those in rural areas have disproportionately higher drug overdose death rates than women in urban areas (Mazure & Fiellin, 2018), indicating an opportunity for targeted rural public health interventions to slow overdose fatality increases among women. Increased rates of acute opioid poisoning and related emergency department (ED) visits in the United States have occurred at the same time as rural EDs face a number of resource constraints. Researchers at the Maine Rural Health Research Center conducted this study to gain insight about rural ED visits for acute opioid poisoning and how they compare with urban ED visits. The authors used data from the 2006 and 2013 Nationwide Emergency Department Sample to examine rural and urban opioid-related visits (ORVs) to EDs, including rate change over time, and the outcomes of these ED visits (treatment and release, inpatient …

Patients with opioid use disorder (OUD) frequently present to the emergency department (ED). Delays or barriers in referral to recovery programs from the ED may decrease successful linkage to ongoing care. Immediately following discharge is a particularly high-risk time for OUD patients as studies have shown that immediately following a non-fatal overdose there is an increase in death rates¹.
