Procedural And Clinical Outcomes Of Dedicated Venous Stents In Acute And Chronic Venous Obstruction: A Retrospective Single Center Study, Guangming Tan, Madeleine G Yan, Bryan Py Yan
Journal of the Hong Kong College of Cardiology
Background Endovenous angioplasty with stenting is an accepted treatment for patient with venous obstruction. Dedicated venous stents have been developed to improve the efficacy and safety of the venoplasty procedure. We report the procedural as well as clinical outcomes of these new generation stents in the treatment of acute and chronic venous obstruction. Method This is a single centre retrospective cohort study of patients who received venous stenting for either acute or chronic venous obstruction. Baseline demographics, procedural details, clinical outcomes including 30 day all-cause mortality, 90 day primary patency rate, and change in Villalta score were reported. Result 41 …

Recurrent Cyanotic Spells In An Octogenarian, Chin Tong Kwok, Chun Kit Wong
Recurrent Cyanotic Spells In An Octogenarian, Chin Tong Kwok, Chun Kit Wong
Journal of the Hong Kong College of Cardiology
We report rare case of an octogenarian who, after years of diagnostic delay, was found to have a patent foramen ovale, and due to an ascending aortic aneurysm that anatomically facilitated right-to-left shunt, presenting with life-threatening cyanotic attacks. The characteristic orthodeoxia-platypnea syndrome and high alveolar arterial gradient hint to the diagnosis. She was eventually treated successfully with urgent percutaneous PFO closure. Routine ordering of a transthoracic echocardiogram, without agitated saline injection under provocation maneuvers, may not be sufficient to rule out shunts.

The Value Of 3d Vena Contracta Area In A Patient With Degenerative Mitral Regurgitation And Multiple Jets, Shing Ching, Chiu Sun Yue
Journal of the Hong Kong College of Cardiology
Quantification of degenerative mitral regurgitation with multiple jets in atrial fibrillation is challenging with two dimensional techniques and may underestimate the severity. We report the feasibility and advantages of vena contracta area in three dimensional transesophageal echocardiography in evaluation of such a patient.

Journal of the Hong Kong College of Cardiology
Immune checkpoint inhibitors (ICIs), though revolutionizing cancer care, are well known to cause a diverse array of autoimmune side effects termed immune-related adverse events (IRAEs). ICI Myocarditis is a rare manifestation of IRAE associated with high mortality. We report two cases of fulminant ICI myocarditis refractory to potent immunosuppressants. Our first patient received pembrolizumab for lymphoma, small cell lung cancer and renal cell carcinoma. After 13 doses of ICIs, he developed fulminant myocarditis with concomitant lower-motor neuron type facial nerve palsy. Despite receiving triple immunosuppressive therapies with pulse steroid, mycophenolate mofetil and basiliximab, his cardiac condition progressively worsened and eventually …

Background
The ‘inflamm-ageing’ phenomenon may worsen the prognosis of heart diseases including infective endocarditis (IE), while surgery may reduce inflammatory burden by direct removal of bacterial vegetation. This study aims to characterize the epidemiological and clinical features of surgical intervention for IE in Hong Kong in the past two decades.
Methods
All patients with incident IE from 2000-2019 in a territory-wide database were identified. We studied the trends in age-specific incidence of IE and identified the most significant contributors to 1-year all-cause death using the population attributable fraction (PAF). A propensity score approach was used to evaluate the benefits of …

Indicators Of Cardiac Autonomic Dysfunction Associated With Chronic Hyperglycemia: Implications On Vascular Surrogates And Heart Failure, Yap-Hang Chan, Bernard Man-Yung Cheung, Hung-Fat Tse
Journal of the Hong Kong College of Cardiology
Background: Limited data exist on the role of chronic hyperglycemia-related cardiac autonomic dysfunction in incident heart failure (HF). We investigated the relations of resting heart rate (HR) and PR interval with chronic hyperglycemia, and effects on new-onset HF.
Methods: We prospectively studied 550 patients with prior coronary disease, ischemic stroke and/ or type 2 diabetes. Incident HF was ascertained. Heart rate-PR interval dissociation was defined as counter-physiological co-occurrence of increased PR interval (>mean; 173.3ms) and increased resting HR (>mean; 64.4 beats/min).
Results: Over 63 ±11 months, 6.7% patients developed new-onset HF. Resting HR³75bpm was independently predictive of new-onset …

Clinical Features And Prognosis Of Chinese Childhood Pulmonary Arterial Hypertension-Related Gene Mutation Carriers, Zhuoyuan Xu, Hongsheng Zhang, Chen Zhang, Qiangqiang Li, Hong Gu
Journal of the Hong Kong College of Cardiology
Background: The impact of PAH-related gene mutations on the clinical phenotype are not yet fully understood, especially in childhood.
Methods: Retrospective study of 114 pediatric Chinese Han population whose onset of PAH was between 3 months and 18 years of age with IPAH were performed. Whole exome sequencing results, clinical characteristics, treatments and outcomes were recorded in all patients.
Results: Thirty-one BMPR2, nine ACVRL1 and 33 other PAH-related gene mutation carriers were detected in this cohort. The median age at diagnosis of PAH was 5.9 (3.4,10.3) years. 54 (47.4%) patients were female. 80 patients including 59 mutation carriers underwent right …

Implantable Cardioverter Defibrillator: Clinical Performance Of Dual Chamber Versus Single Chamber Devices, Katherine Fan, Kathy Lee, Elaine Chau, Alex Yip
Journal of the Hong Kong College of Cardiology
Implantable cardioverter defibrillator (ICD) has been accepted as primary therapy for patients with life-threatening ventricular tachyarrhythmias. ICDs with dual chamber sensing and pacing capabilities are now available for better discrimination between supraventricular and ventricular arrhythmias.

Right Atrial Mass With Inflow Obstruction In A Patient With Igg4-Related Disease, Shek Joyce, Wu Lok Yi, Wong Chun Kit, Lu Jianlin, Lam Wai Lung William, Mak Wai Man Vivien, Chan Ngai Yin
Journal of the Hong Kong College of Cardiology
Cardiac tumour is a rare cause of heart failure. We report a case of right atrial mass found in a patient with known immunoglobulinG4-related disease (IgG4-RD), who presented with right heart failure, renal impairment and thrombocytopenia. Multimodal investigations were performed to reach the diagnosis of diffuse large B-cell lymphoma with cardiac involvement. Chemotherapy was initiated in an attempt for disease control. We highlight the potential link between IgG4-related disease and lymphoma.

Myocarditis and Pericarditis have many virological and immunological causes. This article described the SARS-CoV involvement to the heart in children and adolescents, also the entity Multisystem Inflammatory Syndrome in Children (MIS-C), a rare but severe consequence of SARS-CoV2 infection. Hong Kong case definition of MIS-C was listed. The consensus from overseas experts was that MIS-C should be managed by multidisciplinary team with a structured long-term follow-up to monitor the functioning of various organs and ascertain the prognosis. The side effect mRNA vaccination causing myocarditis and pericarditis is also concern of paediatricians worldwide. Hong Kong published local epidemiology of acute myocarditis …
