Unnecessary antibiotic prescriptions can raise healthcare costs and affect the quality of life due to their related side effects. Objectives: Aim to assess the impact of antibiotic prophylaxis (AP) on the prevalence of postoperative complications in patients undergoing simultaneous implant placement with lateral sinus floor augmentation. Materials and Methods: A randomized, triple-blind, placebo-controlled trial was conducted involving 30 healthy patients. Objective: To investigate the patient-reported ,clinical outcomes and efficacy of static computer-assisted sinus floor augmentation (SCA-SFA), compared to freehand approach. Material and methods: 40 patients underwent lateral sinus augmentation (20 freehand-SFA + 20 SCA-SFA). Patient-reported outcomes were recorded in Visual Analog Scale (VAS) on days 1-7, 14 and intra- and post-operative complications were recorded on week 2,4 after surgery. Chin profile plays a critical role in determining lower facial aesthetics, significantly affects perceptions of facial balance and attractiveness. Conventional assessment methods, which primarily rely on two-dimensional cephalometric norms, are limited by projection errors and a reduced capacity to accurately evaluate soft tissue contours. The development of three-dimensional facial scanning technology has facilitated more accurate assessment of facial morphology; nevertheless, aesthetic evaluation in acceptable aesthetic profile using 3-dimensional camera facial scan remains no evidence. The Effect Of Multimedia Information About Computer-Assisted And Freehand Dental Implant Surgery On Patients’ Knowledge, Anxiety, And Decision-Making, Veerawit Lukkanasomboon

This study aimed to evaluate the impact of multimedia and verbal information on patients' knowledge, anxiety, and decision-making regarding dental implant surgery, focusing on comparing computer-assisted and freehand techniques. This randomized controlled trial involved 100 patients divided into two groups: one receiving multimedia information (n=50) and the other receiving verbal information (n=50). Data were collected through self-administered questionnaires assessing patients' baseline information, knowledge, anxiety, and decision-making before and after receiving information about dental implant surgery techniques. Results showed significant increases in knowledge scores for both groups, with the multimedia group improving from 1.96 to 3.90 (p<0.001) and the verbal group from 2.0 to 3.98 (p<0.001). Anxiety levels decreased significantly in both groups, with reductions from 12.76 to 11.48 (p=0.001) in the multimedia group and from 13.92 to 12.16 (p<0.001) in the verbal group. The proportion of patients who decided to proceed with dental implant surgery increased from 70% to 78% in the multimedia group and from 54% to 66% in the verbal group. After receiving the information, more patients in both groups opted for computer-assisted implant surgery, though treatment costs remained a significant concern for some, leading to declines in surgery acceptance. Despite these findings, no significant differences were observed between the multimedia and verbal groups regarding knowledge improvements, anxiety reduction, or decision-making. In conclusion, multimedia and verbal information effectively enhanced patient knowledge and reduced anxiety about dental implant surgery, and both methods may influence patients' decisions regarding their preferred implant technique, with no significant differences between the two methods.

ที่มาและความสำคัญ: ภาวะกล้ามเนื้อลีบจากอายุที่เพิ่มขึ้น (Age-related sarcopenia) ส่งผลให้มวลกล้ามเนื้อและความแข็งแรงของลิ้นในผู้สูงอายุลดลง ซึ่งนำไปสู่ความบกพร่องในการกลืน (dysphagia) และเพิ่มความเสี่ยงต่อภาวะทุพโภชนาการ การสำลักปอดอักเสบ และการเสื่อมถอยของสมอง การออกกำลังกายเพิ่มความแข็งแรงของลิ้น (Tongue-strengthening exercises - TSEs) สามารถช่วยเพิ่มความแข็งแรง ความหนาของลิ้น และการทำงานของการกลืนในผู้สูงอายุที่สุขภาพดีได้ วัตถุประสงค์: เพื่อศึกษาผลของการออกกำลังกายลิ้นรูปแบบต่างๆ ต่อการเพิ่มความแข็งแรง ความทนทานของลิ้นและการกลืนในผู้สูงอายุวิธีการ: ทำการสุ่มอาสาสมัครผู้สูงอายุสุขภาพดีจำนวน 36 คน ที่มีอายุ 65 ปีขึ้นไป ซึ่งเข้ารับการถอนฟันแบบง่ายเพียงซี่เดียวที่ภาควิชาศัลยศาสตร์ช่องปากและแม็กซิลโลเฟเชียล ที่คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย จากนั้นสุ่มแบ่งเป็น 3 กลุ่ม ได้แก่ กลุ่มลิ้นห่อ (Tongue wrap) กลุ่มลิ้นมุมปาก (Tongue corner) และกลุ่มทาสีลิ้น (Tongue paint) โดยประเมินความแข็งแรงของลิ้น (Tongue strength) ความทนทานของลิ้น (Tongue endurance) และทดสอบความสามารถในการกลืนโดยการกลืนด้วยวิธี Repetitive Saliva Swallowing Test (RSST) ใน 2 ช่วงเวลา คือก่อนเริ่มฝึก (T0) และสัปดาห์ที่ 4 หลังการฝึก (T1) ผลการศึกษา: ในกลุ่มลิ้นทาสีพบว่าความแข็งแรงของลิ้นส่วนหน้า (Anterior Tongue Strength - ATS) และความแข็งแรงของลิ้นส่วนหลัง (Posterior Tongue Strength - PTS) เพิ่มขึ้นอย่างมีนัยสำคัญหลังการฝึก 4 สัปดาห์ (p < 0.05) โดยความแข็งแรงของลิ้นทั้งส่วนหน้าและส่วนหลังในเพศชายของกลุ่มทาสีลิ้นเพิ่มขึ้นอย่างมีนัยสำคัญ (p < 0.05) ส่วนความแข็งแรงของลิ้นทั้งส่วนหน้า ส่วนหลัง และความทนทานของลิ้นส่วนหน้าในกลุ่มลิ้นห่อและกลุ่มลิ้นมุมปากเพิ่มขึ้นจากค่าเริ่มต้น แต่ไม่มีนัยสำคัญทางสถิติ สรุป: การออกกำลังกายลิ้นด้วยการทาสีลิ้นอาจเป็นวิธีที่มีประสิทธิภาพสำหรับการเพิ่มความแข็งแรงของลิ้น

โรคติดต่อหลายโรครวมถึงโรคติดเชื้อไวรัสโคโรนา2019, วัณโรค และโรคไช้หวัดใหญ่สามารถแพร่กระจายการติดต่อได้ทางละอองชีวภาพจากผู้ป่วย การศึกษาเพื่อการหามาตรการจัดการที่เหมาะสมที่แต่เดิมใช้วิธีการตรวจหาการแพร่กระจายของจุลชีววิทยาและการปนเปื้อนของโลหิตถูกเปลี่ยนไปเป็นหลักการฝุ่นละอองขนาดเล็ก การรักษาทางทันตกรรมนับว่าเป็นส่วนหนึ่งของหัตถการที่ก่อให้เกิดละอองฝอยดังกล่าว แต่พบว่ามีการศึกษาที่เกี่ยวข้องกับศัลยกรรมช่องปากจำนวนน้อยมาก โดยส่วนใหญ่งานวิจัยที่เกี่ยวข้องในเรื่องของละอองชีวภาพที่เกิดจากทันตกรรมจะมุ่งไปที่การรักษาในทางทันตกรรมหัตถการและปริทันต์ งานวิจัยนี้มีจุดประสงค์คือต้องการที่จะใช้หลักการฝุ่นละอองขนาดเล็กในการวัดปริมาณของละอองชีวภาพที่เกิดขึ้นจากหนึ่งในกระบวนการศัลยกรรมช่องปากคือการตัดฟัน โดยงานวิจัยนี้จะเป็นไปในรูปแบบการทดลองตัดฟันในระบบปิดจากการใช้เครื่องมือ 3 ชนิด คือ 1.หัวกรอเร็ว 2.เครื่องพีโซอิเล็กทริค 3.หัวกรอไมโครมอเตอร์ เป็นเวลา 1 นาที และวัดปริมาณละอองชีวภาพที่เกิดขึ้นในรูปแบบของ ฝุ่นละออง PM1, PM2.5 และ PM10 ด้วยเครื่องวัดอนุภาคในอากาศ โดยผลการวิจัยพบว่าการใช้เครื่องมือทั้ง 3 ชนิดในการตัดฟันสามารถสร้างละอองชีวภาพขึ้นอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.001 ปริมาณละอองชีวภาพทั้ง PM1, PM2.5 และ PM10 สูงสุดเกิดจากการใช้หัวกรอเร็วตามมาด้วยการใช้เครื่องพีโซอิเล็กทริคและหัวกรอไมโครมอเตอร์ตามลำดับ โดย ณ เวลาที่ปริมาณละอองชีวภาพสูงสุดของแต่ละการใช้เครื่องมือทั้ง 3 ชนิดจะมีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.001 และการใช้หัวกรอเร็วจะต้องการเวลามากที่สุดโดยใช้เวลา 16 นาที 40 วินาที เพื่อให้ค่าละอองชีวภาพกลับสู่ค่าเริ่มต้น ในขณะที่การใช้เครื่องพีโซอิเล็กทริคใช้เวลา 13 นาที 20 วินาที และการใช้หัวกรอไมโครมอเตอร์จะใช้เวลา 8 นาที 20 วินาที นอกจากนี้จากการวิจัยพบว่า กระบวนการตัดฟันโดยการใช้หัวกรอไมโครมอเตอร์จะสร้างละอองชีวภาพน้อยที่สุดและไม่เกินข้อกำนดคุณภาพอากาศในอาคารขององค์การอนามัยโลก ขณะที่การใช้หัวกรอเร็วและเครื่องพีโซอิเล็กทริคจะสร้างละอองชีวภาพในปริมาณที่สูงกว่าข้อกำหนดคุณภาพอากาศในอาคารดังกล่าว การจัดการอากาศอย่างเหมาะสมควรจะถูกพิจารณามาใช้เพื่อป้องกันการติดต่อโรคติดเชื้อทางอากาศ โดยเฉพาะในระหว่างการใช้หัวกรอเร็วและเครื่องพีโซอิเล็กทริคในการตัดฟัน และควรจะมีการกำหนดมาตรฐานระดับปริมาณของละอองชีวภาพที่เหมาะสมในคลินิกทันตกรรม

Comparing Medium To Long-Term Esthetic, Clinical And Patient-Reported Outcomes Between Freehand And Computer-Assisted Dental Implant Placement, Sun Heng

Objective: The aim of the study was to compare medium to long-term clinical and patient reported outcomes between implants placed with Computer-assisted Implant Surgery (CAIS) and freehand protocols. Materials and Methods: Thirty dental implants in the anterior maxillary region with an average of 4 years after loading were assessed by means of Pink Esthetic Scores (PES), Marginal Bone Level (MBL), clinical and patient reported outcomes. Results: CAIS significantly outperformed freehand placement with regards to PES scores (p=0.011). Likewise, implants placed with CAIS showed significantly higher marginal bone levels (p<0.001). Bleeding on Probing, Probing depth and prevalence of mucositis did not differ between the groups, while no peri-implantitis was diagnosed. Conclusions: The use of CAIS lead to superior outcomes in terms of esthetics and marginal bone levels for implants placed in the aesthetic zone as observed in medium to long-term follow-up. No difference was however observed with regards to peri-implant mucosa inflammation.

Antiresorptive drugs are widely used to treat osteoporosis. A serious adverse effect of these drugs is medication-related osteonecrosis of the jaw (MRONJ). By informing and educating patients about MRONJ, providing proper dental referral, and monitoring the oral health of patients who receive antiresorptive agents, physicians can reduce the risk of MRONJ. We investigated the awareness and clinical practices of physicians in Thailand with regard to MRONJ. Aerosol-generating procedures with contaminated saliva may pose a risk of disease transmission during the COVID-19 pandemic. COVID-19 preventive measures in dental clinics have evolved in response to the pandemic situation. This research investigated the effect of this pandemic on Thai patients' knowledge, attitudes, and behavior toward dental treatment. The online survey was distributed in June 2022. Objectives: To study factors which influence a dentist’s decision to propose the Tooth Autotransplantation (AT). Material and methods: A cross-sectional study was conducted among 99 dentists between January and March 2021. A questionnaire comprised demographic characteristics, unprimed scenario, primed scenario, reasoning behind decisions, experience, and knowledge of AT. Objectives: To compare the accuracy of position and parallelity of two implants, using static and dynamic CAIS systems. Materials & Methods: 30 patients received two implants randomly allocated to 2 different CAIS systems. Optimal implant position and absolute parallelity was planned based on preoperative CBCT. Implants were placed using surgical guide (static CAIS, n = 15) and real time navigation (dynamic CAIS, n = 15). Implant 3-dimentional deviation and parallelity was calculated after surgery.
