Addressing Comorbidity Education Needs In Tristar Skyline’S Readmission Reduction Program, Savannah Carroll

This doctoral capstone experience was completed in partnership with TriStar Skyline Medical Center’s inpatient rehabilitation Readmission Reduction Program. This project was driven by a need to increase the provision of education for patients qualifying for this program to promote successful comorbidity management upon discharge to reduce avoidable readmissions. The primary focus of this project was to develop educational materials to promote an understanding of how to successfully manage Diabetes Mellitus, COPD, and CHF to reduce disease complications and avoidable readmissions.
