Class II division 1 malocclusion with deepbite and gummy smile can be treated in several ways. Adequate intrusion and retraction of maxillary incisors may improve the smiling appearance as well as the facial profile. However, anterior torque control can be challenging. Herein, we report a patient with 5 mm gummy smile, where the final esthetic improvement was achieved by combined use of posterior and anterior miniscrews. Fixed maxillary molar distalization appliance is an effective and noncompliance way for space regain. But with the increased confidence on skeletal anchorage, maximum anchorage preparation was planned with miniscrew in recent years. In this article, we cited two clinical cases to introduce an intraoral noncompliance appliance for maxillary molar distalization. RAMDA (rapid molar distalizing appliance) assist with skeletal anchorage is our modified technique used in these cases. From superimposition, we found that average rate of distalization was 1-2 mm per month. However, distal tipping of molar and anterior anchorage loss were also noted when using miniscrew as indirect anchorage.

Overextrusion of maxillary molars usually results from early loss of antagonistic teeth due to caries or periodontitis. Conventional techniques for molar intrusion was used multiple teeth and extraoral devices, that depended highly on patient’s cooperation and usually result in extrusion of other teeth rather than posterior intrusion. This article reviews treatment consideration, force mechanics and side effects about miniscrews for intrusion of upper molars. We also present a clinical case to confirm miniscrew treat supra-erupted upper molars successfully.

Adult patients with skeletal Class III malocclusion may have several available treatment options: (1) a combination of orthodontic treatment and orthognathic surgery. (2) camouflaged orthodontic treatment with various extraction selections. In this case report, we use miniscrew anchorage to retract mandibular incisors and control labial tipping of maxillary incisors. Not only improving dental occlusion without extraction of teeth (except for wisdom teeth) but also achieving a balanced profile in only 14 months of treatment time.

An adult female patient aged 34 years and 8 months with Wilson’s disease, a rare autosomal recessive genetic disorder, demonstrated poor chewing ability and facial asymmetry which were worsened gradually since the disease attacked her 10 years ago. She was treated by surgical orthodontics with minimal presurgical orthodontic decompensation. We designed an atypical maxillary anterior subapical osteotomy to close the maxillary incisor pontic space, retract anterior segment of the maxilla, and maintain the dental midline simultaneously. A case was presented with an adult male patient diagnosed as skeletal class III and dental class I malocclusion, and complaining of his dental crowding. Undergone orthodontic related examinations and periodontal evaluations, the treatment plan was to combine atypical extraction with insertion of mini-implants into mandible, in order to facilitate control of anchorage. After the treatment, lateral profile and occlusion were both apparently improved. Discussions were made about usage of mini-implants in this case and other possible ways to treat the patient.

Mini-implant anchorage has been proven to be an effective therapeutic strategy in treating various kinds of malocclusions. The most common position for mini-implant anchorage is on the upper posterior area. It is not only because the clinical requirements in anchorage reinforcement in the upper arch but also because of the more favorable anatomical situations of attached gingiva and surrounding movable mucosa.
