The etiologies of skeletal Class III malocclusion include excessive growth of mandible, deficient growth of maxilla, and the combination of both. Anterior openbite may be due to skeletal, dental, habitual, or functional problems. Mini-implant can be a reliable anchorage for maxillary molar intrusion, which facilitate anterior openbite closure by counterclockwise rotation of the mandible. However, the mandibular counterclockwise rotation may increase antero-posterior jaw discrepancy in Class III malocclusion. Therefore, surgical orthodontics is often required in the treatment of Class III malocclusion with anterior openbite. Facial asymmetry affects more frequently on the lower face than the upper face. Significant facial asymmetry compromises both occlusal function and facial esthetics. The etiology of facial asymmetry may be skeletal, dental, and soft tissue components of dentofacial structures. Correct diagnosis and appropriate treatment planning are very important in the management of the orthodontic patients with the chief complaint of facial asymmetry. Facial asymmetry in lower face can be clearly detected through the examination on chin deviation and asymmetry of bilateral gonial angle regions. Severe root resorption with excessive tooth mobility may compromise the capability of the teeth to sustain occlusal loading, and possibly lead to extraction. Apical external root resorption (AERR) is considered as one of the risks associated with orthodontic treatment. Some clinical factors have been shown to correlate with orthodontic treatment, whereas the determinants for predicting AERR consistently were not found. Therefore, orthodontists must monitor the amount of root resorption during the course of orthodontic treatment. Multiple idiopathic root resorption (MIRR) is a rare condition. Adult patients with skeletal Class III malocclusion may have several available treatment options: (1) a combination of orthodontic treatment and orthognathic surgery. (2) camouflaged orthodontic treatment with various extraction selections. In this case report, we use miniscrew anchorage to retract mandibular incisors and control labial tipping of maxillary incisors. Not only improving dental occlusion without extraction of teeth (except for wisdom teeth) but also achieving a balanced profile in only 14 months of treatment time.

Class III malocclusion with mandibular protrusion and lateral displacement are commonly observed at orthodontic clinic. If patients don’t accept orthognathic surgery and belong to mild to moderate malocclusion, non-surgery treatment will be available. This was a 25-year-old female; her chief complaint was anterior crossbite with anterior spacing and mandibular protrusion. The diagnosis was dental and skeletal Class III malocclusion with mandibular prognathism. The patient was successfully treated with fixed orthodontic appliance by modified Multiloop Edgewise Archwire (MEAW) Technique and asymmetric elastics. The Class III malocclusion is the most difficult anomalies to understand. The purpose of this study was to investigate the differences in morphological characteristics of Class III malocclusion patient who can properly treated by orthodontic treatment only or combined orthognathic surgery. To find the most important morphological factors those influence the treatment planning and predict probability of orthognathic surgery. The pre-treatment lateral cephalograms of two groups of 80 subjects (40 orthodontic and 40 surgical) with Class III malocclusion were analyzed. Anterior crossbite is a common clinical problem that requires early diagnosis and proper treatment. Here we report on a pair of siblings with anterior crossbite. They have a similar dental and facial profile at the start of treatment, but developed different types of dental facial patterns on follow-up. Our report highlights that it is difficult to precisely predict the facial growth simply based on patient’s family history and the data of cephalometric analysis. Young patients with growth potential should be monitored continuously after orthodontic treatment.
