Resolving Echoes From Dentistry’S Past, Christopher J. Smiley Dds

This editorial discusses a 1939 concept of dental disease prevention and the resistance it faced from some in the dental profession due to potential economic self-interest. It draws parallels to current issues in the dental field, such as the Centers for Medicare & Medicaid Services (CMS) rules on dental coverage. The CMS rules link dental services to medical outcomes, opening the door for future expansion. However, the editorial highlights gaps in coverage, particularly for cancer patients and those with diabetes. The new CMS rule signifies progress in integrating oral health into healthcare, echoing a goal set in 1939.

This 10-Minute EBD column explores the potential association between neoplasms and the risk of peri-implantitis in patients with dental implants. A case study involving a patient with a history of an ossifying fibroma raises questions about the impact of neoplasms on implant success. The literature search, conducted on PubMed, reveals relevant insights. The evidence suggests that ossifying fibromas, particularly with augmentation, may pose a risk for peri-implantitis. However, the direct causative relationship between neoplasms and peri-implant pathology remains unclear. Other risk factors such as periodontitis, foreign bodies, and augmentation procedures are also discussed in the context of implant complications.

As the gateway to the respiratory system, the oral cavity can play an essential role in SARS-CoV-2 invasion and transmission. Besides high expression of angiotensin‐converting enzyme 2 (ACE2), the oral cavity can harbor viral particles in saliva, gingival crevicular fluid, and the periodontal pocket. Microbial and inflammatory associations closely link periodontitis and COVID-19. In this review, we summarize the possible oral manifestations of COVID‐19, its association with periodontal diseases, the plausible underlying mechanisms of microbial and inflammatory crosstalk between COVID‐19 and periodontal diseases, and mitigation protocols during periodontal treatment.

Narrow diameter implants and mini-implants are preferred by dentists for restoration of missing teeth on narrow ridges. However, such implants are associated with long-term complications including soft tissue deficiencies. This case report describes a novel approach using a connective tissue graft to resolve peri-implant mucositis. A 66-year old male patient presented with inflammation and abscess in relation to a mini-implant, at the upper left incisor, on the facial aspect. Deficiencies of keratinized tissue and width of attached gingiva were observed. Surgical management was performed using subepithelial connective tissue. Papillon-LeFevre Syndrome (PLS) is a rare autosomal recessive disease, associated with palmar-plantar hyperkeratosis and premature loss of both deciduous and permanent teeth. Treatment of this disease continues to be a challenge to the medical and dental professions. The following is a case report of two siblings of Chinese origin, diagnosed with Papillon-LeFevre syndrome and follows them for 10 subsequent years.

This study underscores the complexity of esthetic crown lengthening, emphasizing the need for tailored strategies to ensure optimal outcomes.
