Type 2 diabetes is a chronic metabolic disorder that requires complex care and involves a variety of self-management decisions on a regular basis to manage the symptoms and avoid complications. The prevalence of type 2 diabetes among the South Asian population is six times higher than their European and white American counterparts (Gujral et al. 2013). Due to the aging population and the high prevalence of chronic diseases such as diabetes and cardiovascular disease, in the US, millions of people suffer from chronic wounds secondary to these chronic conditions. Wound care treatment is very expensive, costing the US health care system approximately $10 billion annually. San Mateo Medical Center treats many patients with wounds. Healthcare providers are responsible for recognizing and assessing those individuals dealing with depression. Screening tools have been shown to be beneficial in assisting healthcare providers to identify those individuals meeting the depression criterion. The PHQ-9 is a self-administered screening tool that has shown to be reliable and valid in screening individuals for depression in most healthcare settings. The purpose of this paper is to discuss the quality improvement project, implementation of the post fall huddle in a long-term care facility. Falls are the most common problem in adults 65 years and older. Falls in this population can have devastating effects, often leading to significant changes in morbidity or death. Adults in long term care settings have an increased risk of falling and having a subsequent fall due to an acute illness, weakness, or confusion (CDC, 2015). Vaccinations are the most common painful needle procedure, with an estimated 12 billion injections given per year (CDC, 2019). The usual method for administration of vaccinations is through needle puncture, which is often painful. Children often report receiving a shot as one of the most feared and painful experiences (McMurtry et al., 2015). Obesity is a multifactorial, individualized, chronic disease that increases known risk factors for significant comorbidities and mortality. The primary purpose of this Evidence-Based practice project was to improve indicators of obesity (weight, BMI) among patients served by a free medical clinic, using a tailored multicomponent intervention. The secondary purpose was to determine if the weight loss intervention was associated with a decrease in blood pressure and depression symptoms. The intervention was guided by the Health Promotion Model by Nola Pender. The Johns Hopkins Nursing Evidence Based Practice Model (JHNEBP) guided implementation of the project. In 2015, approximately 30.3 million Americans, including 10.5% of the population of Minnesota, had been diagnosed with diabetes mellitus (American Diabetes Association, 2019). Increased risk of diabetic complications including neuropathy, retinopathy, nephropathy, cardiovascular disease, and chronic wounds has been associated with an elevated hemoglobin A1C (HgA1C). Formal diabetic education has been shown to be a cost-effective intervention to reduce HgA1C. The Office of Disease Prevention and Health Promotion published an objective within HealthyPeople 2020 that seeks to increase the percentage of diabetics receiving formal diabetic education in the United States from 46.9% of adults to 58.4%. The purpose of this study was to examine individuals’ perceptions of weight loss and weight loss methods. Respondents were employees and customers attending health/wellness events at local supermarkets in June, July, and August 2019. Participants completed a survey to assess their understanding and perceptions of weight loss and methods used to achieve weight loss. The majority of participants were white females, 60 years or older with a college degree and a Body Mass Index (BMI) classified as overweight or obese. While substantial practical, empirical, and theoretical contributions have been made toward the implementation of healthcare innovations, significantly less attention has been directed towards the sustainability of these interventions. For this reason, many healthcare innovations become unsustainable over time—yielding few long-term improvements, causing stakeholder disenchantment, and wasting valuable resources.
