UNLABELLED: Background Heart failure is one of the most prevalent chronic conditions in adults, leading to prolonged morbidity, repeated hospitalizations, and placing tremendous economic burden on the healthcare system. Heart failure patients discharged from rural hospitals, or primarily critical access hospitals, have higher 30-day readmission and mortality rates compared to patients discharged from urban hospitals. Self-management improves heart failure patients' health outcomes and reduces re-hospitalizations, but adherence to self-management guidelines is low. We propose a home based post-acute care service managed by advanced practice nurses to enhance patient activation and lead to the improvement of self-management adherence in heart failure …

There is a pressing need to retain dually eligible elderly Medicare and Medicaid beneficiaries in the community. The objective of this study was to examine median personal care services (PCS) hours, and how increasing PCS to the median (for those below), might delay nursing home placement (NHP) and save cost. Methods: Retrospective study of PCS hours, costs, and NHP in a statewide home and community based waiver program in the Midwest serving 6525 dually eligible Medicare-Medicaid beneficiaries aged 65 and older. Utilizing individualized exercise training programs that are inclusive of appropriate resistance training protocols can attenuate the deleterious effects of sarcopenia and enhance quality of life, functional capacity and also diminish its negative effect on independence. Considerations of how gender-related factors influence smoking first appeared over 20 years ago in the work of critical and feminist scholars. This scholarship highlighted the need to consider the social and cultural context of women's tobacco use and the relationships between smoking and gender inequity. Parallel research on men's smoking and masculinities has only recently emerged with some attention being given to gender influences on men's tobacco use. Since that time, a multidisciplinary literature addressing women and men's tobacco use has spanned the social, psychological and medical sciences. I review the current trend of international volunteer travel with a focus on advantages and disadvantages of short-term (1-2 week) medical volunteer trips and how cross-cultural communication can influence the effectiveness of volunteers. Advantages include making health care available to populations who normally do not have access to care. Disadvantages to trips include a lack of follow-up care and community dependence on outside aid. To increase the effectiveness of these trips, it is recommended that volunteers coordinate with full-time health care workers in target communities and utilize pre-departure training.

Background: There is a high potential for medication discrepancies to occur during patient care transitions. However, health professionals must find ways to reduce these and improve patient care, such as with medication reconciliation. This intervention is used to identify a patient’s most accurate medication list by comparing the medical record to another list obtained from the patient, hospital, or other provider. Misuse and overprescribing of antibiotics leads to a rise in pathogens that are resistant to the conventional drugs prescribed, causing increases in morbidity and mortality rates. Antibiotic resistance occurs when microbes still survive despite the presence of an antibiotic. The remaining microbes grow in strength and multiply until there is a population that are now resistant to treatment. Hemodialysis is a treatment process utilized by kidney patients to filter their blood. These patients are required to undergo an intensive regimen including hemodialysis treatments multiple times per week lasting 3-5 hours each. As a result of hemodialysis, patients experience decreased autonomy, freedom, and independence. An estimated 50% of these patients do not adhere to their treatment regimen. Patients on hemodialysis may be more compliant to their treatment if they are satisfied with the level of care and empathy they perceive from their healthcare providers. Medication nonadherence, defined as “the number of doses not taken or taken incorrectly that jeopardizes the patient’s therapeutic outcome,” is a major health problem with about 43% of the general population nonadherent to their medications. Medication nonadherence accounts for an estimated 125,000 deaths per year in the US, 33-69% of medication-related hospital readmissions, and an estimated $100 to $300 billion in direct and indirect medical costs. Vitamin K, a fat-soluble vitamin synthesized by plants, is found in large quantities within green leafy vegetables, such as collard, mustard, turnip and spinach. These greens are commonly cooked with fat to enhance flavor. The vitamin K in these greens can leach out during cooking into the liquid portion of the pot, which is referred to as potlicker. Vitamin K activates clotting factors (II, VII, IX, and X,), which can lead to the formation of a thromboembolism (blood clot). This article shares a unique model of collaboration between a school of nursing and a community business to offer an on-site occupational health clinical experience for RN to BSN students. This activity could be easily replicated by others to provide a learning opportunity for nursing students. Hospitals remain the primary site of employment for nurses. Background: Immigrant women may be regarded as a vulnerable population with respect to access and navigation of maternity care services. They may encounter difficulties when accessing culturally safe and appropriate maternity care, which may be further exacerbated by language difficulties and discriminatory practices or attitudes. This investigation seeks to understand how Aymara women navigate their sexual and reproductive health and rights (SRHR) in Chile’s intercultural health care model. Indigenous communities have their own practices that complicate the provision of sexual and reproductive health by requiring health care providers to be aware of two different worldviews and how they may conflict, as well as what is necessary to provide respectful care. However, an ethnically and culturally pertinent framework is vital to actually assuring successful SRHR provision, whose tenants include autonomous choice and care free of discrimination, coercion or violence.
