How To Prevent Perioperative Myocardial Injury: The Conundrum Continues, Jian-Zhong Sun, David Maguire
How To Prevent Perioperative Myocardial Injury: The Conundrum Continues, Jian-Zhong Sun, David Maguire
Department of Anesthesiology Faculty Papers
BACKGROUND: Perioperative myocardial injury (PMI) remains a major cause of perioperative morbidity and mortality but clinical strategies to prevent PMI are still uncertain.
METHODS AND RESULTS: We comprehensively searched PubMed for major research articles concerning clinical strategies to prevent PMI. The key findings are as follows: (1) the American College of Cardiology/American Heart Association guideline update for perioperative cardiovascular evaluation for noncardiac surgery is very useful to stratify cardiac risk preoperatively; (2) cardiac troponin has emerged as a biomarker to diagnose postoperative PMI and to predict clinical outcomes; (3) coronary revascularization before noncardiac surgery probably would provide cardiac protection in …

Glycaemic Index, Glycaemic Load And Ovarian Cancer Risk: A Prospective Cohort, Stephanie A. Navarro Silvera, Meera Jain, Geoffrey R. Howe, Anthony B. Miller, Thomas E. Rohan
Department of Public Health Scholarship and Creative Works
Background
There is some evidence that plasma insulin levels might influence ovarian cancer risk. Glyacemic index (GI) and glycaemic load (GL) are measures that allow the carbohydrate content of individual foods to be classified according to their postprandial glycaemic effects and hence their effects on circulating insulin levels. Therefore, we examined ovarian cancer risk in association with GI and GL, and intake of dietary carbohydrate and sugar. Methods
The study was conducted in a prospective cohort of 49 613 Canadian women enrolled in the National Breast Screening Study (NBSS) who completed a self-administered food-frequency questionnaire (FFQ) between 1980 and 1985. …

Does ‘Excessive’ Anticoagulation Predispose To Periprosthetic Infection?, Javad Parvizi, Elie Ghanem, Ashish Joshi, Peter F. Sharkey, William J. Hozack, Richard H. Rothman
Department of Orthopaedic Surgery Faculty Papers
Background
Although persistent drainage and hematoma formation are recognized risk factors for the development of periprosthetic infection, it is not known if excess anticoagulation is a predisposing factor.
Methods
We conducted a 2 to 1 case-control study with 78 cases who underwent revision for septic failure. The controls underwent the same index procedure but did not develop consequent infection. Patient comorbidities, medications, intraoperative, and postoperative factors were compared.
Results
Postoperative wound complications including development of hematoma and wound drainage were significant risk factors for periprosthetic infection. A mean international normalized ratio of greater than 1.5 was found to be more …

Posttraumatic Stress Disorder And Associated Risk Factors In Canadian Peacekeeping Veterans With Health-Related Disabilities, Don Richardson, James A. Naifeh, Jon D Elhai
MacDonald Franklin OSI Research Centre
Objectives:
This study investigates posttraumatic stress disorder (PTSD) and its associated risk factors in a random, national, Canadian sample of United Nations peacekeeping veterans with service-related disabilities.
Methods:
Participants included 1016 male veterans (age < 65 years) who served in the Canadian Forces from 1990 to 1999 and were selected from a larger random sample of 1968 veterans who voluntarily and anonymously completed a general health survey conducted by Veterans Affairs Canada in 1999. Survey instruments included the PTSD Checklist-Military Version (PCL-M), Center for Epidemiological Studies-Depression Scale (CES-D), and questionnaires regarding life events during the past year, current stressors, sociodemographic characteristics, and military history.
Results:
We found that rates of probable PTSD (PCL-M score > 50) among veterans were 10.92% for veterans deployed once and 14.84% for those deployed more than once. The rates of probable clinical depression (CES-D score > 16) were 30.35% for veterans deployed once and 32.62% for those deployed more than once. We found that, in multivariate analyses, probable PTSD rates and PTSD severity were associated with younger …

Tazeen S Ali Dr
This study was aimed at identifying practices during the menstrual, partum and postpartum periods posing possible risk factors contributing towards secondary infertility in women of a selected population in Karachi, Pakistan. A matched case-control study was conducted from April 2003 to March 2004. Four hundred cases were selected from five infertility clinics affiliated with tertiary-care hospitals, and 400 age-matched controls were recruited from the neighborhood of each case. After taking written consents, trained interviewers conducted interviews using a pretested structured questionnaire. Factors found to be independently associated with secondary infertility were: previous delivery at an unclean place (adjusted odds ratio …
Understanding Sleep Quality And Waking Cervico-Thoracic Symptoms, Susan J. Gordon, Karen A. Grimmer, Patricia Trott
Understanding Sleep Quality And Waking Cervico-Thoracic Symptoms, Susan J. Gordon, Karen A. Grimmer, Patricia Trott
Internet Journal of Allied Health Sciences and Practice
Purpose: Using data from an epidemiological study described elsewhere (Gordon et al 2002 & 2007), multivariate logistic regression models were constructed to explore the association between sleep position, factors related to sleep quality, and the prevalence of waking symptoms (cervical spine pain and stiffness, headache, and aching in scapulae or arm regions). Method: A causal model was developed in which putative exposures were tested for their association with sleep quality, which was considered as an antecedent cause of waking cervico-thoracic symptoms. Results: Factors which significantly constrained sleep quality were identified as the presence of a medical condition, past history of …

We used long-term data collected for up to 10 yr (1994–2004) at 23 trapping arrays (i.e., webs and grids) in Arizona, Colorado, Montana, and New Mexico to examine demographic factors known or suspected to be associated with risk of infection with Sin Nombre virus (SNV) in its natural host, the deer mouse (Peromyscus maniculatus). Gender, age (mass), wounds or scars, season, and local relative population densities were statistically associated with the period prevalence of antibody (used as a marker of infection) to SNV in host populations. Nevertheless, antibody prevalence and some of the risk factors associated with antibody …

Turkish Journal of Medical Sciences
Aim: Community-acquired infections caused by extended-spectrum beta-lactamase (ESBL)-producing bacteria are an emerging problem. Digestive tract colonization is a prerequisite for infections by ESBL-producing microorganisms. The aim of this study was to determine the prevalence of and risk factors for fecal carriage of ESBL-producing Escherichia coli (E. coli) or Klebsiella spp. in the community. Materials and Methods: A total of 928 stool samples admitted to the laboratory during a four-month period were included in the study. Samples were diluted in saline and cultured in two EMB agar plates supplemented with either 1 µg/ml cefotaxime or 1 µg/ml ceftazidime. All isolates that …
The Prediction Of Antenatal And Postnatal Depression In A Sample Of Western Australian Women, Debbie A. Lien
The Prediction Of Antenatal And Postnatal Depression In A Sample Of Western Australian Women, Debbie A. Lien
Theses: Doctorates and Masters
In Australia, the Edinburgh Postnatal Depression Scale (EPDS; Cox, Holden & Sagovsky, 1987) has been increasingly used to screen for antenatal depression prior to its evaluation on a sample of Australian pregnant women. Also, the identification of predictors associated with antenatal depression has been neglected relative to the research focus on postpartum depression. An aim of the study was to evaluate the antenatal screening properties of the EPDS against diagnoses of major depression with the Mini International Neuropsychiatric Interview (MINI; Sheehan eta!., 1998). The aims were also to develop predictive models of risk factors associated with antenatal depression as measured …
