A 60 year-old male patient who presented with lumbago, fever, local tenderness over lumbar spine and associated with paravertebral muscle spasm, was prescribed antibiotic therapy 12 days after the onset of initial symptom under the impression of septic lumbar discitis, which was then verified by MRI and microbiology within one week after admission. Early diagnosis and adequate antibiotic therapy gained a favorable result, although the patient needed anterior inlerbody fusion for symptomatic retrolisthesis of L3 on L4 at the sixth week after admission.

Sexual dysfunction Wth erectile disability is a common complication in spinal cord injured males. Usually their erectile dysfunction are neurogenic in originn. We have begun to apply intra-cavernous injection of prostaglandin E1(PGE1) for management of erectile dysfunction in spinal cord injuries since 1991 and encouraging results have been achieved. We report here a case of chronic spinal cord injury with vasogenic erectile dysfunction who does not respond to intracaver-nous PGE1 injection. Defining Success: The Politics Of Evaluation In Alcohol And Drug Abuse Treatment Programs, James L. Wolk, David J. Hartmann, William P. Sullivan
The Journal of Sociology & Social Welfare
Alcohol and drug abuse treatment programs must respond to several important stakeholders or beneficiaries of services who have an investment in how success is defined. Utilizing data from recent statewide studies of treatment outcomes of alcohol and drug abuse services, this paper concludes that a strict adherence to an abstinence-only model of success, rigidly adopted by many in the treatment industry is counterproductive. Multiple measures of success are essential to fully understand and assess a changing model of intervention in the chemical dependency field.

Clinically, we were occasionally asked to evaluate peripheral nerve injuries in patients whose local cutaneous conditions, such as burn wound, crushing wound, skin graft, and gauze overlying limb, were not suitable for percutaneous electrical stimulation or needle EMG. Could the magnetic stimulation be used in these conditions? How much potential deviation could be produced with this technique when the limbs were covered by gauze? The purpose of this study is to discuss the long term changes of renal function in traumatic spinal cord injured patients. This study was carried out on 45 male cases who suffered from complete traumatic spinal cord injury. As a result of injury 17 cases were quadriplegic, 28 were paraplegic. The mean age of these cases was 41.5 years (range from 21 to 60 years). The purpose of this study is to investigate the effect of elbow position on segmental motor nerve conduction velocity (MNCV) of ulnar nerve and compound motor action potential (CMAP) pick up on abductor digiti minimi. The study was performed on 38 ulnar nerves of 19 healthy persons under 4 different joint angles (FE: full extension, F45: flexion 45°, F90: flexion 90°, FF: full flexion). This study was taken to determine the cardiovascular response after isokinetic endurance exercise among different extremities (Upper vs Lower), speeds (High-180°sec vs Low-60°sec), Types (Concentric vs Eccentric). The subjects (N=80) used KinCorn isokinetic dynamometer to perform isokinetic endurance exercise. Systolic & Diastolic pressure, rate pressure product and change ratio were measured at rest and after exercise.Among the lower limb isokinetic endurance exercise, the high speed eccentric contraction group had the significant cardiovascular response; among the upper limb, the low speed concentric had the significant cardiovascular response. A total of 50 patients, who had developed hemiplcgia or hcmiparesis as a result of a cerebral vascular disease, were collected for electrophysiologic study. All of these patients were free of peripheral nerve disorder or shoulder joint pathology.MNCV, SNCV, F latencies of bil median nerves and concentric needle eiectromyographic examination were done. A female of age 30 years developed a muscular weakness in proximal extremities during 2 years and responded poorly to steroid treatment. Creatine kinase in isoenzymic MM form was markedly elevated according to a serum muscle enzyme test. Electromyography examination showed a myopathic feature. Histopathological tests in muscle biopsy specimens revealed prominent cytoplasmic vacuoles rimmed by basophilic granules under space optical microscopy, and abundant intranuclear filamentous inclusions in electron microscopy. These clinical features and athological especially electron microscopic findings are considered to be of paramount importance for the diagnosis of inclusion body myositis.

A case of 4 years 8 months old girl who had the brain insult was followed till 8 years 1 month old. Precocious puberty with hyperphagia was noted. Results of endocrine testing revealed normal pituitary function, but increased to pubertal level of gonadotropins. Radiological study confirmed posttramatic encephalomalacia at right hemisphere including right ventromedial hypothalamus. This report discussed the medical course of the case including rehabilitation management.

การศึกษาอุบัติการของโรคกระดูกอ่อนในสุนัขพันธุ์ใหญ่ ที่มารับบริการตรวจรักษาและ ฉีดวัคซีนที่โรงพยาบาลสัตว์ คณะสัตวแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย จากสุนัขที่ตรวจ 31 ตัว พบเป็นโรคกระดูกอ่อน 21 ตัว (67.7%) ในจำนวนนี้เป็นโรค nutritional secondary hyperparathyroidism (NSH) 18 ตัว (85.7%) และ retained enchondral cartilage core ที่กระดูก ulna 3 ตัว (14.3%) พบมากที่สุดในพันธุ์ร้อตไวเลอร์ 8 ตัว (38.1%) สุนัข ป่วยอายุน้อยกว่า 6 เดือน 15 ตัว (71.4%) ระหว่าง 6-12 เดือน 4 ตัว (19.0%) และมาก กว่า 12 เดือน 2 ตัว (9.5%) เป็นเพศผู้ 15 ตัว (71.4%) และเพศเมีย 6 ตัว (28.6%) การ รักษาโรค NSH ในสุนัข 4 ตัวด้วยสารประกอบแคลเซี่ยมคาร์บอเนต เปรียบเทียบกับการ ใช้สารประกอบที่มีแคลเซี่ยมต่อฟอสฟอรัส อัตราส่วน 4 ต่อ 1 ในสุนัข 3 ตัว พบว่าสาร ประกอบแคลเซี่ยมฟอสฟอรัสไม่สามารถปรับอัตราส่วนแคลเซี่ยมต่อฟอสฟอรัสในซีรั่ม ในขณะที่แคลเซี่ยมคาร์บอเนตสามารถปรับอัตราส่วนของแคลเซี่ยมต่อฟอสฟอรัสในซีรั่ม ให้สูงอย่างเห็นได้ชัดภายหลังรักษา 2 สัปดาห์ ซึ่งแตกต่างจากก่อนการรักษาอย่างมีนัยสำคัญ ทางสถิติที่สัปดาห์ที่ 3 และ 4

ได้วัดระดับของเอนไซม์โซลีนเอสเทอเรสในซีรั่มสัตว์ 10 ชนิด โดยการวัดสีที่เกิดจาก ปฏิกริยาที่มีอะซีทิลไธโอโพลีน (acetylthiocholine) เป็น substrate พบระดับของเอนไซม์ ในสัตว์แต่ละชนิดแตกต่างกัน ลิงจะมีระดับเอนไซม์สูงสุด (3,038.67 ± 516.97 U/L) รองลงมาคือม้า (2,202.86 ± 516.81 U/L) และสุนัข (1,562.85 ± 388.07 U/L) สัตว์ เคี้ยวเอื้องประเภทแกะ โค กระบือ จะมีเอนไซม์ในระดับต่ำกว่า 300 U/L ส่วนสัตว์ที่มีระดับเอนไซม์ปานกลาง ได้แก่ เป็ด ไก่ สุกร และกระต่าย

The purpose of this study was to evaluate the effect of quadriceps isokinetic eccentric trainingprogram at the strength of both concentric and eccentric strengths, as well as for the different eccentric training positions. Fifty-two normal subjects (26 male and 26 female), aged 21-32 year, were randomly assigned to a control group and two treatment groups. The first treatment group (n=18) trained in the sitting position and the second treatment group (n=18) trained in the supine position. One hundred and fifty-six stroke patients were investigated to analyze the changes of time or frequency of performing social activities between pre-stroke and post-stroke stages. Modified (Frenchay activities index) questionnaire was used and subjects were devided into three groups by the Barthel scores. Within- subjects Wilcoxon signed-rank test was used to analyze the collected data. The results showed social activities were grossly disrupted in most patients after stroke especially in group I, group II (ADL-dependent groups), while group III (ADL-independent group) was primarily limited in outdoors social activities. The facial expression of functional recovery in patients with peripheral facial paisy/paraiysis on rehabili-tation therapy was investigated. Thirty-four cases of fa-cial palsy were collected from January 1986 to Decem-ber 1991 in the Department of Physicai Medicine and Rehabilitation of Tri-Service General Hospital. Twenty-five cases (74%) were classified as idiopathic and the remaining 9 cases (26%) were as organic facial palsy.The rehabilitation program included daily superficial heat, oii massage, electrical stimulation and active exer-cise of the affected muscles was prescribed and instituted completely. Renal failure was once the leading cause of death in patients with chronic spinal cord lesions. In western countries, dramatic improvements in urologic care enhancing patient survival have occurred in the last 25 years. Nonetheless, urological complications continue to be the major morbidity in spinal cord injured patients. The purpose of our investigation was to survey the urological complications in patients suffered from spinal cord injuries (SCI) for more than 2 years in Taiwan. The majority of tuberculosis bacillus infection is in the lung. Although the incidence of infection in the skeletal system is low, spinal tuberculosis is on important cause of hack pain in the Rehabilitation Outpatient Department because it is the most skeletal site affected.During the past year, we managed three cases of spinal tuberculosis. The characteristics of these patients were 1) backache over the thoracolum-bar junction with knocking pain, 2) fair response to analgesics and physical modalities and Paras-pinal abscess noted on MRI (Magnetic Resonance Image) examination. Pulmonary embolism (PE) is a life-threatening disease with variable manifestations and thus early diagnosis is difficult among the spinal cord injury (SCI) patients. There are still no clinical reports or statistical data about the PE in SCI patients in Taiwan. However, the incidence of PE in SCI patients was higher than that in general population according to the reports in western countries, and early prevention, diagnosis and treatment were documented to reduce the morbidity and mortality.
