This doctoral capstone was an exploration of the specialty practice area of seating and mobility through partnerships with Numotion and TriStar Skyline Medical Center. A variety of projects were completed including educational materials for a wheelchair clinic, a loaner wheelchair tracking spreadsheet, educational materials for a Belmont Occupational therapy course, a literature search on pediatric power mobility, and protocol spreadsheets for an FDA research study. Throughout this capstone, I learned the skills necessary for a wheelchair evaluation, developed interprofessional communication skills, and gained an understanding of the lived experience of end users.

The United States (U.S.) population is aging resulting in an increase in Skilled Nursing Home (SNF) residents. A larger proportion of SNF residents rely on wheelchairs for mobility. Currently there are no studies that determine the processes, which therapists utilize to correctly fit their clients for wheelchairs. Poor fitting wheelchairs can have a negative impact on the residents’ ability to participate in occupational activities. Residents may also suffer physical or psychological consequences from an ill-fitting wheelchair. Therapists in SNFs often have no established criteria in providing wheelchairs for their clients. The purpose of this project was to gain a deeper understanding of the lived experience of individuals with disabilities and to identify what accessibility barriers may exist in community settings. Individuals with disabilities may face increased environmental barriers that influence their ability to successfully access and fully participate in community settings. This project attempts to provide community accessibility resources for individuals with disabilities in order to make community outings as successful as possible for all persons.

Manual wheelchair (MWC) users with spinal cord injury (SCI) rely heavily on their upperextremities to complete daily occupations. Due to repetitive shoulder use during wheelchair mobility and propulsion, MWC users are at greater risk of shoulder pain and shoulder pathology, and thus decreased independence, and lower quality of life. The relative fit of the wheelchair and its parameters are critical and can further impact the user’s propulsion biomechanics. Occupational therapists receive basic education on seating and wheeled mobility, but additional education is needed for best practice. This study is a series of three studies aimed at exploring wheelchair service delivery in a department of physical and occupational therapists dedicated to wheelchair seating. The three studies utilized a retrospective analysis to investigate the clinical documentation written by the practicing clinicians between 2007 and 2017. The first study aimed to describe the wheelchair and accessory recommendations made in the 11-year period, in relationship to age and gender. The majority of recommendations in the dedicated seating department were for complex wheelchairs. The biomechanical demands of the upper extremities (UEs) during pediatric manual wheelchair (MWC) use have not been fully explored. Children who use MWCs for mobility engage in a range of functional activities that may place large biomechanical demands on the UEs leading to a high risk on overuse injuries. This study aims to analyze the kinematics and kinetics of pediatric manual wheelchair use during propulsion, starting, stopping and weight relief tasks. Development Of A Model Of Participation Of Community-Based, Discretionary Activities By People Who Use Wheelchairs, Anita Perr

This cross-sectional research analyzed an existing data set of 302 wheelchair users to identify the psychosocial predictors of participation in community-based, discretionary activities. Two defining elements of participation were studied: the extent of participation and satisfaction with participation. Descriptive analyses of the participants' demographic information and portions of four assessments were completed first. Regression analyses were then used to systematically eliminate potential covariates until the significant psychosocial covariates of the extent of and satisfaction with participation were identified. The Beach Wheelchair, Sandcrawler, is a machine intended to help give people with disabilities the full beach experience. The non-profit organization Bridge II Sports sponsored this project, and tasked us with developing the design and fabricating a prototype. This chair has several features including a linkage mechanism that raises and lowers a chair relative to an external frame, hand crank drive with Gates Carbondrive belt and sprockets, buoyant Wheeleez balloon tires, and much more.
