The purpose of this Masters project was to emphasize community psychosocial practice through collaboration with a local mental health organization by conducting a needs assessment, implementing a project at the community site, and evaluating the outcomes. Prior to collaboration with the organization, a literature review was conducted analyzing the role of occupational therapy working with individuals with serious mental illness. From this, it was found that occupational therapists can work with individuals with serious mental illness to engage in meaningful occupations, increase a sense of accomplishment, competence, and satisfaction, and participate in community living. The Medicare Inpatient Psychiatric Services (IPS) policy, originally developed in the 1960s and without any significant or meaningful updates since the 1980’s, is physician specific, does not reflect the current model of care for psychiatric inpatients, and is restrictive for psychiatric mental health nurse practitioner (PMHNP) practice. The policy is complex, consists of outdated regulations, and results in millions of dollars in compliance costs for Inpatient Psychiatric Facilities (IPFs) annually (NABH, 2019). Clinical psychologists are grossly underrepresented in treatment provision for individuals with serious mental illness (SMI; Roe, Yanos, Lysaker, 2006). Systemic (e.g., emergence of managed care) and training-based (e.g., limitations to SMI specific training) factors contribute to the establishment of this underrepresentation, while mental health stigma amongst psychologists may play a role in perpetuating it. This study explores the efficacy of delivering modified dialectical behavior therapy (m-DBT) to direct support staff who work with seriously mentally ill patients with neurocognitive deficits using remote web-based technology. The partnering facility for this study is a residential skilled-nursing facility that provides 24-hour care to individuals under conservatorship. Using a previously piloted m-DBT protocol, qualitative feedback was gathered from four participants with background training in psychology and varying experiences with DBT. Serious mental illnesses (SMIs) often have an onset early in life and go on to produce disability and functional impairment, which require ongoing supportive services and loss of productivity. Bipolar disorder (BD) and schizophrenia (SZ) are two of the most common SMIs. BD and SZ differ in their pattern of neurocognitive performance, which suggests differences in the severity of functional impairment between the illness groups. Within these psychiatric illness groups, subgroups may also be identified using cluster analysis. These chronic disorders are difficult to treat and often sabotaged by medication nonadherence. Long-acting injectable (LAI) medications are one strategy to combat medication nonadherence. Effectively providing treatment with LAI medications can be challenging at the clinic level.

Objectives: Reduce the number of days without medication by reducing the number of days between injection due date and injection administration for patients receiving LAI medications.

These higher rates are worsening in degree, despite known preventative strategies, such as physical health risk screening (HRS). This study evaluates the relationship of physical HRS with self-efficacy for health prevention behaviors, perception of level of risk of health consequences, and intention to follow up with medical care for identified health risks. Prevalence rates of posttraumatic stress disorder (PTSD) for individuals with serious mental illnesses (SMIs) are high, with an estimated 49% to 100% being exposed to potentially traumatic events. The consequences of these disorders are serious and lasting, with PTSD being the costliest of mental health disorders.
