From Isolation To Inclusion: Embracing Local Perspectives In Examining The Treatment Model Of Care For Aboriginal Persons Affected By Tuberculosis Or Leprosy In The Kimberley Region, North Western Australia, Stefanie Jane Oliver

In the remote Kimberley region of North Western Australia, tuberculosis (TB) and leprosy continue to affect a small number of Aboriginal people, despite historical efforts to eliminate either disease. Treatment, predominantly antibiotic therapy, is a principal therapeutic intervention used to cure TB and leprosy and halt infection transmission. Decisions made around treatment therefore impact not only the individual person affected, but also their families and communities. The well-worn models of Directly Observed Therapy (DOT) and case management are used nationally to assist treatment continuity and completion. Accurately defining disease distributions and calculating disease risk is an important step in the control and prevention of diseases. This study used geographical information systems and remote sensing technologies within the MaxEnt ecological niche modeling program to create predictive risk maps for leprosy and Schistosomiasis in Brazil and Chagas disease in both Brazil and Bolivia. New disease cases were compiled for leprosy, Schistosomiasis, and Chagas disease from the Brazilian ministry of Health for 2001 to 2009 and the data was stratified to a 10,000 population for each municipality. Bolivian Chagas prevalence rates were calculated from 2007 to 2009 survey data. …

Complete sequencing and annotation of the M. leprae genome has provided new information related to proteins constituting its hypothetical proteome. Since M. leprae can not be grown in vitro, novel approaches are needed to determine which proteins are expressed during infection and whether these proteins are related to pathogenesis. Secreted proteins represent a distinct group of protein with respect to their structure and function, contribution to virulence and are of particular importance for vaccine development because they are often immunogenic and have the potential to be recognized early in infection.
