Investigating The Relationship Between Unavoidable, Hospital Acquired Pressure Injury And Mortality: Is Unavoidable, Hospital Acquired Pressure Injury A Signal Of Increased One-Year Mortality Risk?, Eileen Jimenez

Theses & Dissertations

The purpose of this dissertation was to explore the strength of the relationship between unavoidable Hospital Acquired Pressure Injury (HAPI) and increased risk of one-year mortality for patients age 55 and older who were admitted to a hospital from 2015 to 2018. A quantitative, retrospective case-control study was designed to compare demographics, comorbid conditions, pressure injury risk, and living situation of those patients that developed unavoidable HAPI and hospitalized adults with similar physiological burdens that remained injury-free. The one-year mortality for the HAPI group was 27 of 48 (56%), and 26 of 47 patients (55%) for the Control group. The …

Malignant Hyperthermia, Kathryn Hernandez Jul 2020

Malignant Hyperthermia, Kathryn Hernandez

Nursing Student Class Projects (Formerly MSN)

Malignant hyperthermia (MH) is a condition in which the body goes into a hypermetabolic state affecting skeletal muscle in response to receiving inhaled anesthetics or the neuromuscular blocking agent, succinylcholine. MH is an autosomal dominant disorder that is most closely related to a defect in the ryanodine receptor. The defective receptor causes a rapid increase of intracellular calcium causing hyperthermia, tachycardia, tachypnea, hypercapnia, increased oxygen consumption, acidosis, hyperkalemia, muscle rigidity, and rhabdomyolysis. With the complexity of this disorder and its unique pathophysiological process it is crucial that all parts of the surgical team, especially anesthesia providers, understand how to treat …

Integrating Geriatric Assessment And Genetic Profiling To Personalize Therapy Selection In Older Adults With Acute Myeloid, Vijaya R. Bhatt May 2020

Integrating Geriatric Assessment And Genetic Profiling To Personalize Therapy Selection In Older Adults With Acute Myeloid, Vijaya R. Bhatt

Theses & Dissertations

Integrating geriatric assessment for patient profiling and genetic profiling of leukemic cells represents an innovative approach to personalize therapy selection in older adults with acute myeloid leukemia (AML). We report results of a pre-planned interim analysis of a pragmatic phase II trial that utilized this strategy to personalize therapy. Patients ≥60 years with a new diagnosis of AML underwent geriatric assessment prior to initiation of treatment. Geriatric assessment of physical function, cognitive function and comorbidity burden were used to determine fitness for chemotherapy. Patients with good or intermediate-risk AML received intensive chemotherapy such as anthracycline and cytarabine (7+3) if determined …

Resting Heart Rate And Its Effect On Cardiovascular Disease, Peter Tronset May 2020

Resting Heart Rate And Its Effect On Cardiovascular Disease, Peter Tronset

Physician Assistant Scholarly Project Papers

The purpose of the presented research and systematic review is to determine the associated impact resting heart rate has on development of cardiovascular disease. A literature review was conducted through electronic databases: PubMed, Clinical Key and Dynamed Plus. No limitation of timeframe was used when researching as few studies are available correlating resting heart rate and its associated development of cardiovascular disease. Research points to limitations of lowering resting heart rate due to adverse effects on blood pressure. Additionally, tachycardia is addressed through several methods including lifestyle modification and pharmacologic therapy. Comorbidities such as obesity, low body mass index (BMI), …

Modeling Heart Failure Predictive Mortality In Skilled Nursing Facilities, Stephen Charles Biondi Apr 2020

Modeling Heart Failure Predictive Mortality In Skilled Nursing Facilities, Stephen Charles Biondi

Dissertations (1934 -)

Problem: Approximately 25% of skilled nursing facility (SNF) patients are diagnosed with heart failure (HF). The heart failure mortality trajectory is non-linear and there are no useable mortality models from clinical records for use by healthcare providers in the SNF setting. A mortality risk model for patients with HF is important so that palliative care interventions may be offered to improve patients’ quality of life during the end stages of dying. Assessment items captured routinely by the SNF Resident Assessment Instrument –Minimum Data Set 3.0 (RAI-MDS) instrument may be useful in identifying those patients whose declining condition warrants discussion of …

Health Outcomes Associated With Gulf War Illness, Shannon K. Barth

Graduate Theses, Dissertations, and Problem Reports

Background:

Nearlythirty years after the 1990-1991 Persian Gulf War a definitive case definition for Gulf War Illness (GWI) is lacking. The objective is to explore health outcomes associated with GWI using currently accepted classifications in order to better understand GWI by examining pre-war medical conditions as predictors of probable GWI, mortality outcomes of veterans with probable GWI, and female-specific medical conditions among women with probable GWI. Additionally, we utilize an alternative GWI case definition.

Methods:

Data come from theDepartment of Veterans Affairs (VA)1995-1997 National Health Survey of Persian Gulf War Era Veterans, a population-based survey of deployed …

Examining Body Mass Index And Sepsis Mortality At One Year After Sepsis, Jamie D. Robinson Jan 2020

Examining Body Mass Index And Sepsis Mortality At One Year After Sepsis, Jamie D. Robinson

Theses and Dissertations

Introduction: Sepsis is a serious and life-threatening syndrome affecting 1.7 million Americans annually and resulting in approximately 270,000 deaths. An “obesity paradox” where obese individuals have lower sepsis mortality than their non-obese counterparts has been described. The problem is that the longevity of the effect is unknown, and few studies have examined the obesity paradox after 1-2 months post-diagnosis.

Methods: This retrospective cohort study examined clinical, demographic, and biomarker variables thought to affect sepsis mortality at three-time points: 30 days, 180 days, and at one year post-sepsis diagnosis in order to shed light on specific factors that might define …
