The purpose of this study is to review and utilize components of the Certified Community Behavioral Health Clinic (CCBHC) model to create an outpatient redesign model for behavioral health (without duplicating it). This was accomplished by recommending greater use of, and comprehensive integration of, Peer Support Specialists within the current behavioral health continuum of care by increasing funding for the CCBHC demonstration program through legislative expansion by using Medicaid eligibility. Problem: Transition patients at the end-of-life are those patients who are not mentally ready or physically eligible for hospice thus not enrolled in a Palliative or Hospice Program. Only one-third of the Transition patient census had completed and filed advance care plans. Healthcare providers are obligated to perform life-saving measures unless documented otherwise. A lack of advance care planning can create complicated scenarios and cause discordant care incidents.

Problem: Loneliness and social isolation, both of which can have serious detrimental effects on health and well-being, have become more prevalent during the recent COVID-19 pandemic, particularly in the older adult population. During part of the past year, older adults were strongly encouraged to follow strict quarantine measures because of their vulnerability to the virus. During the COVID-19 pandemic, universities and colleges had to quickly transition their courses from in-person to online. This switch to online learning heavily impacted community-engaged learning (CEL) courses because it meant that internships with community partners had to become remote as well as the coursework. Problem: Widespread opioid misuse has led to the opioid epidemic that claims 130 deaths per day due to an opioid overdose (CDC, 2018). The opioid epidemic has plagued the United States for three decades and has been considered a public health emergency since 2017 (HHS, 2017). Rural communities are disproportionately affected by this problem and in 2014, the rate of opioid overdose deaths was 45% higher in rural communities (SAMHSA, 2018).

Purpose: This qualitative study is to identify programing that encourages increased self-sufficiency in SNAP recipients. The goal was to identify themes related to successful self-sufficiency programming at federally-funded health centers/health programs (HCHPs). The study also investigated the role of social determinants of health in relation to achieving self-sufficiency.

Method: A series of 45-minute to 1-hour-long interviews were conducted to explore the perspectives of seven total staff at seven federally-funded HCHPs across the U.S. western region. An accumulation of stress and lack of coping resources can contribute to a higher incidence of mental and physical health disparities. Homeless adults are one of the most vulnerable subgroups who utilize Enterprise Resource Center, a peer-run day-center in Marin County. Because stress from multiple factors was identified in this transient and high-risk population, an intervention for stress reduction and management was explored. The findings from the needs assessment suggested that there was a need for an integrated approach for stress reduction and management. SIFs are spaces where people who inject drugs (PWID) can bring their pre-obtained drugs in a hygienic space and use them under supervision. Evolving throughout the last century, Gonorrhea has become a superbug, becoming difficult to treat. As the second most commonly reported notifiable disease, gonorrhea rates have been increasing, despite efforts in prevention and treatment. Populations at risk are mainly MSM and FSW. In 2015, men’s gonorrhea rates in the United States were higher compared to women, at 140.9 and 107.2 per 100,000 population, respectively.
