Those receiving medical care are at risk for developing serious hospital-acquired conditions (HACs), which include infections (HAIs) and other harm while receiving medical care. Pay for performance programs initiated by the Centers for Medicare and Medicaid Services (CMS), and private payers, linking reimbursements to quality of care, have created financial incentives for hospitals to address these HACs. The most common HAI in the United States, causing almost half a million infections, with 29,000 deaths within 30 days of diagnosis, results from Clostridium difficile. Infection with this bacterium leads to deadly diarrhea, and disproportionately impacts patients 65 and older, representing 80% …

Decentralization has become one of the most important public-sector reform programs globally since the 1990s with mixed results. In this four-paper dissertation, I explored how decentralization reforms affect public goods and services delivery, by analyzing how post-decentralization institutional changes shape local expenditure decisions and health service utilizations in Indonesia. The main goal of this dissertation was to improve understanding on a better approach to studying public goods and services delivery under a decentralized governance. Before the advent of anti-retroviral therapy (ART), many states enacted and strictly enforced laws that criminalized HIV transmission. These laws were enacted to prevent HIV-infected individuals from knowingly transmitting the virus to the uninfected. Over the past two decades, however, questions have been raised regarding the effectiveness of HIV criminalization and its unintended consequences. For example, there is little evidence that the criminalization of HIV transmission has been effective in reducing the spread of the disease, particularly when compared to education and other prevention efforts. This thesis is a study of popular data resources in healthcare and their applications toward health information system design and development. The first section reviews the way data is collected with a list of websites where health datasets can be obtained; along with a summary of the recognized vocabulary standards that make it easier to understand and share this data. It is well established that adverse childhood experiences (ACEs) increase risk of developing mental health disorders later in life. It is unclear how early in childhood these effects can be seen. Executive functioning (EF) and emotion regulation (ER) have been proposed as potential mechanisms which mediate the effect of ACEs on mental health. Individual ACEs have been associated with poor EF and ER. It is unclear whether ACE scores impact EF and ER in a graded, dose-response manner. The Effects Of Pre-Pregnancy Tobacco Use On Food Cravings And Weight Gain In Pregnancy, Lauren E. Blau

Smoking cessation is associated with greater frequency of food cravings, even when controlling for body mass index. Recent studies have indicated similarities between obese women and women who smoke tobacco in mood states and food craving frequency. This is consistent with the common substrate hypothesis, which postulates that shared neural pathways mediate craving across multiple domains, and could explain why it is common for women attempting to quit smoking to gain weight. Smoking cessation prior to pregnancy is strongly encouraged due to the adverse effects of tobacco use on the developing fetus. Studies have shown that women who quit smoking …
