Multiple Dimensions Of The Symptom Experience In Patients With Advanced Cancer And Their Impact On Quality Of Life, Stephanie Gilbertson-White

Many people with advanced cancer experience multiple severe symptoms as their disease progresses such as pain, sleep problems, fatigue, and depression. These symptoms can be a result of the cancer itself, cancer treatment or an interaction of the two. The studies reported in this dissertation uses the patients' own responses to survey questions to describe the multiple dimensions of the symptom experience; the factors that predict the total number of symptoms; as well as the optimal cutpoint between a low and a high number of symptoms and the between group differences in patient outcomes (i.e., depression, anxiety, quality-of-life).

OBJECTIVE: To describe a case of supernumerary minute ring chromosome (SMRC) in an infertile man, with the concurrent finding of a left varicocele. DESIGN: Case report. SETTING: Urologic infertility clinic in a university hospital. PATIENT(S): Male patient presenting with primary infertility. INTERVENTION(S): Karyotyping, genetic counseling, and microsurgical left varicocelectomy. MAIN OUTCOME MEASURE(S): Sperm concentration and motility, chromosomal number and structure. RESULT(S): Fluorescence in situ hybridization revealing SMRC 14. Sperm concentration and motility improved after left varicocele repair. Pregnancy achieved with sperm from a patient with globozoospermia is rare, even after ICSI, since the activation of the oocyte may not occur in this disorder. Therefore, activation of the oocytes by piezoelectricity or calcium ionophores has been suggested, although spontaneous activation of the oocyte after ICSI has been reported in some cases. We report a successful pregnancy in a couple in which the male partner had globozoospermia with microdeletions in the Y chromosome with no further assisted activation after ICSI. The surface doses to patients during chest, abdomen and pelvis radiography were measured over a period of 3 years, during which time computed radiography (CR) and digital radiography (DR) systems were introduced to replace film-screen systems. For film-screen and CR the surface doses were measured with thermoluminescent dosimeters. For DR the surface doses were calculated from the dose-area product (DAP) meter readings. Measurements were made for each type of examination and detector type on 10 average-size patients. OBJECTIVE: Heparin-induced thrombocytopenia (HIT) is a life threatening complication of heparin therapy, causing thrombosis. The aim of our study was to find out the frequencies of HIT antibody seroconversion and clinical HIT in Turkish medical patients on different forms of heparins. METHODS: Our study included 61 patients who were on unfractionated heparin (UFH) (n: 37) and low molecular weight heparin (LMWH) (n: 24) therapies. The frequency of HIT antibody formation was determined by means of antigenic (ELISA), and functional assays (serotonin release assay-SRA). OBJECTIVE: Prenatal maternal anxiety has detrimental effects on the offspring's neurocognitive development, including impaired attentional function. Antidepressants are commonly used during pregnancy, yet their impact on offspring attention and their interaction with maternal anxiety has not been assessed. The authors used P50 auditory sensory gating, a putative marker of early attentional processes measurable in young infants, to assess the impact of maternal anxiety and antidepressant use. Food has a number of functional properties that can support the balance between energy intake and energy expenditure and, theoretically, one of these is the thermic effect of food. Including high-protein foods in meals may be advantageous in contributing to energy expenditure but, in practice, the evidence needs to relate to specific foods and normal dietary conditions. Children's distress during medical procedures can be decreased when professionals or parents provide distraction coaching, but the quality of distraction may contribute to the variation in the results. This article describes the development of the Distraction Coaching Index (DCI), a tool for measuring the quality and frequency of distraction coaching. Nominal group technique and consensus agreement were used for concept clarification and development of behavioral indicators and descriptors. This study evaluates the impact of parent-provided distraction on children's responses (behavioral, physiological, parent, and self-report) during an IV insertion. Participants were 542 children, 4 to 10 years old, randomized to an experimental group that received a parent distraction coaching intervention or to routine care. Experimental group children had significantly less cortisol responsivity (p = .026). Children that received the highest level of distraction coaching had the lowest distress on behavioral, parent report, and cortisol measures. When parents provide a higher frequency and quality of distraction, children have lower distress responses on most measures.

OBJECTIVE: To report a case of stage IIIc poorly differentiated endometrial adenocarcinoma with trophoblastic differentiation and to review previously reported cases. METHODS: The clinical course and histopathology of the case were reviewed, and a literature search for other reported cases was performed. RESULTS: The tumor contained syncytiotrophoblast-like giant cells that stained positively for the beta subunit of human chorionic gonadotropin (beta-hCG), and the patient's serum beta-hCG level was elevated (95 mIU/ml), but became undetectable after treatment. Beta-hCG was used as a tumor marker during further therapy. At 16 months' survival, she remains without evidence of disease and with a beta-hCG …

Australia’s oldest human remains, found at Lake Mungo, include the world’s oldest ritual ochre burial (Mungo III) and the first recorded cremation (Mungo I). Until now, the importance of these finds has been constrained by limited chronologies and palaeoenvironmental information. Mungo III, the source of the world’s oldest human mitochondrial DNA, has been variously estimated at 30 thousand years (kyr) old, 42–45 kyr old and 62 +/- 6 kyr old. while radiocarbon estimates placed theMungo I cremation near 20–26 kyr ago. Even by archaeological standards, Blombos Cave is a modestly sized shelter. Yet artifacts recovered from just 13 cubic meters of deposit inside transformed our understanding of when our species developed behavioral attributes we associate with “modern” humans. From this cramped hole in a sandstone cliff on the Southern Cape coast of South Africa, Christopher Henshilwood and his colleagues unearthed evidence of symbolic expression, in the form of abstract designs (carved ochre bars) and personal ornaments (shell beads) at least 70,000 years old. That is more than 35,000 years before anything comparable emerged in Europe.

Previous experiments by our group in normal gravity (1 G) have revealed spatial relationships between postural and focal components of whole-body reaching and pointing movements. We suggested that these relationships could be explained partly through the use of gravity to displace the CoM and attain the object or target position. In this study we compared human whole-body reaching in 1 G and microgravity (0 G) in order to more fully investigate how gravity contributes to strategies adopted for task execution and to determine possible invariant temporal relationships between multiple segments.
