Tactical operators are required to carry loads as part of their occupations. Carriage of these loads have been associated with causing physical injuries to the carrier and impairing their ability to perform occupational tasks. One potential means of negating these risks associated with load carriage tasks is through physically conditioning the carrier. Through use of the Frequency, Intensity, Time and Type (F.I.T.T.) formula this review explored the literature to determine the optimal conditioning stimulus to enhance the resilience of tactical operators required to perform load carriage tasks. Background and Purpose. The Dynamic Gait Index (DGI) is a standardized clinical assessment that aids in evaluating a subject’s ability to modify gait in response to changing demands. The purpose of this study was to use Rasch measurement theory to examine whether the DGI rating scale meets suggested psychometric guidelines, whether the hierarchical order of DGI tasks is consistent with a clinically logical testing procedure, and whether the DGI represents a unidimensional construct. Subjects. Subjects were 84 community-dwelling male veterans (age range=64–88 years; mean±SD=75±6.47 years). Methods. Data were retrieved retrospectively from the participants’ clinical records. Background and Purpose. The purpose of this case report is to explore the feasibility of electromyography-triggered neuromuscular stimulation (EMG-stim) as an adjunct to constraint-induced movement therapy (CIMT). Case Description. The patient was a 72-year-old man, 10 years poststroke, who did not meet traditional CIMT criteria. The EMG-stim was applied to the wrist extensors of the patient's weaker arm for one half of the CIMT training hours. Outcomes. The intervention was feasible for this individual. Improvements were observed in motor behavior, quality and amount of use, muscle activity, wrist range of motion, and reaction time of the more-affected extremity. These improvements …

Rehabilitation is a specialty area with defined competencies and discrete nursing knowledge. Unless nurses have been educated in the basic competencies of rehabilitation, they are not prepared to provide safe, quality rehabilitative care to patients with chronic illnesses and disabilities. An evidence-based practice project was designed to address the need for education of the nursing staff working on an inpatient rehabilitation unit in Northwest Indiana. Roger's Diffusion of Innovations provided the theoretical framework for the project. A critical appraisal of the literature was conducted to determine best practice. Anterior cervical discectomy and fusion (ACDF) is a common surgical intervention for radiculopathy resulting from degenerative cervical spine conditions. Post-surgical cervical collar use is believed to reduce post-operative pain, provide the patient with a sense of security during activities of daily living and even reduce rates of non-fusion. This prospective randomized controlled pilot trial investigates trial design feasibility in relation to prospective physical, functional, and quality of life-related outcomes of patients undergoing ACDF with interbody cage, with (n = 17) and without (n = 16) post-operative cervical collar usage. This study explored post-migration experiences of recently migrated Pakistani Muslim adolescent females residing in the United States. In-depth, semi-structured interviews were conducted with thirty Pakistani Muslim adolescent females between the ages of 15 and 18 years living with their families in Houston, Texas. Data obtained from the interviews were evaluated using discourse analysis to identify major reoccurring themes. Participants discussed factors associated with the process of adaptation to the American culture. The results revealed that the main factors associated with adaptation process included positive motivation for migration, family bonding, social support networks, inter-familial communication, aspiration of adolescents to learn other …

Context: Students in health care often describe their feelings of self-inadequacy in clinical skills and report these feelings abate with experience.
Objective: To determine whether early contextual learning experiences can improve entry-level student physical therapists’ self-perceived level of clinical preparedness.
Design: A pair-wise comparison of pre vs. post intervention. Survey of students self-perceived level of clinical preparedness before and after a series of three planned small group contextual learning experiences involving the assessment of actual hospital patients.

To clarify the mechanisms of compensation in subjects with anterior cruciate ligament deficiency (ACL-D), we examined proprioception, quadriceps strength, somatosensory evoked potentials (SEPs) and muscle responses during gait in varied combinations of speed and incline. Seventeen subjects with ACL-D were grouped by functional level and report of giving way. Seven subjects without ACL-D served as a control sample for muscle response measures. ACL-D subjects with quadriceps weakness experienced giving way and could not resume sport activities. Those without weakness fell into one of two groups: (1) copers, who had full return to previous activity and no giving way despite proprioceptive …

Abstract: It is not known if manipulating velocity within a prescribed resistance training mode will improve muscle activation. Muscle activations of the Rectus Femoris (RF), Vastus Lateralis (VL), Vastus Medialis (VM) and Bicep Femoris (BF) were examined during a leg extension exercise at 3 different velocities on 15 subjects (10men, 5 female, Age = 21.5 ± 1.8 yrs, Height = 171.2 ± 12.5 cm, Mass = 75.5 ± 16.3 kg). Objective: Chronic pain is a common problem that is associated with mood disorders such as depression. The Depression, Anxiety, and Stress Scales (DASS-21) questionnaire is commonly used to help measure disordered mood. In this study, we used Rasch analysis to analyze the clinimetric properties of the DASS-21 in a chronic low back pain sample. Study Design and Setting: A Rasch analysis was conducted on data collected as a part of a randomized hospital-based placebo-controlled trial. DASS-21 questionnaires were completed by the 154 enrolled participants. Results: The DASS-21 subscales fit the Rasch model. No differential item functioning was detected for age, …

Extract:Sarcopenia is a little acknowledged geriatric syndrome associated with significant health care and personal burden (Janssen et al 2004). Defined by an age associated loss in muscle mass and function (either muscle strength or physical performance), sarcopenia is a known precursor to later life disability (Cruz-Jentoft et al 2010). Across the lifespan, an adult can expect a 35% loss in muscle size and 50% loss in muscle strength that will significantly reduce their capacity to undertake activities of daily living, and increase their risk of falls due to reduced balance capacity and core strength (Bean et al 2002).

Objectives: To investigate physiotherapist agreement in rating movement quality during lower extremity functional tests using two visual rating methods and physiotherapists with differing clinical experience.
Design: Clinical measurement.
Participants: Six healthy individuals were rated by 44 physiotherapists. These raters were in three groups (inexperienced, novice, experienced).
Main measures: Video recordings of all six individuals performing four lower extremity functional tests were visually rated (dichotomous or ordinal scale) using two rating methods (overall or segment) on two occasions separated by 3e4 weeks. Previous research reports positive effects of yoga on health. The physical and psychological outcomes of participation in Bikram yoga are under-researched despite its increasing popularity, and this type of yoga may be significant with regards to stress management due to its unique method of practice. This study was designed to assess changes in levels of mindfulness, perceived stress, and physical fitness after participation in an 8-week Bikram yoga program. In light of the increasing number of girls participating in sports, the pressures associated with "big time sports," and interest in preventing alcohol-related problems in female athletes, the purpose of this viewpoint is to examine the prevalence of alcohol abuse in female athletes and to provide coaches and teachers with suggestions for education and prevention. Some of these include a continual increase in the competition, negotiating relationships with teammates and coaches, performance anxiety or fear of failure, decreased social interaction due to competition, time management issues, burnout, and coping with injuries (Storch, Storch, Killiany, & Roberti, 2005).

In higher education in the United States, women are often underrepresented in leadership positions. When women try administration, they face a higher rate of attrition than their male counterparts. Ultrarunners participate in running events that exceed the 26.2 mile marathon distance (e.g., 50 K, 50-100 miles). Very little research exists on ultrarunners, especially women. The present study is a descriptive study detailing the motivation, goal orientation, demographic characteristics (e.g., age, job demands, family structure), training habits (e.g., hours per week of training), and coach utilization of women ultrarunners. Currently, little is known about strength and conditioning programs at the high school level. Therefore, the purpose of this research was to explore current practices in strength and conditioning for varsity high school athletes in selected sports. Specifically examined were: who administers programs for these athletes, what kinds of training activities are done, and whether the responsible party or emphasis changes depending on the gender of the athletes. Coaches of varsity soccer, basketball, and softball/baseball in three large Idaho school districts were asked to complete an online survey. Examines the marketing strategy of the short-lived Iowa Cornets, part of the Women's Professional Basketball League (WBL) in the late 1970's. Hoping to capitalize on the popularity of Iowa girls' high school basketball, the Cornets emphasized everything Iowa, signing home-grown players, hiring coaches with Iowa connections, and employing "cornography" - green and gold team colors, an ear-of-corn mascot named Shucks, and a bus called the Corn Dog. After a year, however, the team adopted some of the WBL's apologetic strategies, overemphasizing players' femininity and heterosexuality.

This session presents a unique insight into the Physical Conditioning Optimisation Review; a four year project to reduce musculoskeletal injuries at the Royal Military College. It will highlight the benefits and potential pitfalls of an injury surveillance program and the importance of identifying cultural and contextual factors that influence risks and risk controls. Lessons learnt and subsequent actions taken by the review will be explored and the impact of dedicated managerial support and effective leadership discussed.
