Gout is a treatable chronic inflammatory condition that can cause severe joint pain and disfigurement. Commonly, it affects the first metatarsophalangeal (MTP) joint as a monoarticular process. It is typically caused by hyperuricemia and genetic mutations of the uricase gene. Gout is definitively diagnosed with the presence of negative birefringent needle-shaped crystals in the synovial fluid of the affected joint. Classically, seafood and alcohol are risk factors for developing gout. However, given current population trends, obesity is another major risk factor. The purpose of this study was to investigate the frequency of Achilles and plantar spurs and related factors in gout patients. Matherial and methods: We performed a retrospective review of gout patients, treated at Hacettepe University hospitals between 2014 and 2019. We identified patients from the hospital records using the ICD-10 code (M10). Demographic and clinical features, comorbidities, and foot radiographies were collected. A wild captured, male Monocellate cobra, Naja kaouthia of unknown age was observed with anorexia, imbalance of swimming and mild swelling in the distal fourth of the body. Physical examination revealed a palpable firm mass in the coelomic cavity, of which radiographic examination was suspicious as an obstruction of large intestine by a foreign body. Removal of the foreign body through its cloaca was attempted by enema and digital manipulation, but was not successful. Thereafter, an exploratory surgery was performed and revealed an abnormality in the left kidney that was compressing the large intestine. Ankylosing spondylitis (AS) rarely coexists with tophaceous gout in the spine. We present a case of a 57-year-old businessman with AS who did not have previous history of hyperuricemia or gouty arthritis. He had a three-month history of progressive bilateral lower limb weakness caused by spinal cord compression (rooted on a tophaceous gout), leading to incomplete paraplegia. He received surgical decompressive laminectomy, which removed the hypertrophic ligamentum flavum and tophus in the level of thoracolumbar junction. Viscosupplementation by intraarticular administration of exogenous hyaluronic acid has been a favorable treatment option to alleviate the symptoms of knee osteoarthritis. The potential but uncommon adverse reactions associated with intraarticular hyaluronic acid injection are local pain, swelling, and allergic response. These reactions are typically mild, transient, and self-limiting. The focus of this report is an 80-year-old man who presented with acute painful swelling of his knee joint after an injection of naturally derived hyaluronic acid. Aspiration of knee joint effusion yielded 40 mL of turbid straw-colored fluid. Synovial fluid cell counts revealed numerous leukocytes with a marked predominance of neutrophils. …
