BACKGROUND: Cardiogenic Shock (CS) remains the most common cause of death in hospitalized acute ST-segment elevation myocardial infarction (STEMI) patients. Predictors of outcomes in those patients include clinical, laboratory, radiologic variables, and management strategies. Objective To assess serum zinc level in patients presented with acute coronary syndrome and its relation to cardiac enzymes in acute myocardial infarction (AMI). Background Atherosclerosis is a well-known cause of myocardial infarction owing to accumulation of lipids and fibrous elements in arteries. It was found that there are highly efficient protective mechanisms in the cell, including metal-binding proteins as antioxidants and manganese superoxide dismutase and copper–zinc superoxide dismutase as enzymes. We present the case of a 31-year-old patient, without cardiovascular risk factors, without significant pathological or family history of cardiovascular disease, who was diagnosed with severe coronary artery disease, left main bifurcation localization, which required surgical coronary revascularization. The angiographic and intraoperative aspect excluded the most common causes of non-atherosclerotic causes of coronary lesions. Antiplatelet therapy reduces the risk of subsequent ischemic events in patients suffering from acute myocardial infarction (AMI). In this respect, the multi-agent therapy to be used along with aspirin remains to be determined. In this study, ticlopidine was tried in AMI for its potential additive effects over aspirin. This study, designed to investigate the blood lactate and pyruvate levels in AMI, was carried out on 41 patients. Based upon the localization of AMI, the patients were studied under anterior AMI (n=17), anteroseptal AMI (n=9) and inferior AMI (n=15) groups. Left ventricular failure developed in five patients with anterior AMI and in one patient with anteroseptal AMI. Cardiogenic shock developed in two patients with anterior AMI. Its detection in the circulation has been shown to be a sensitive and specific marker for myocardial cell damage.
