Irrational use of antibiotics lead to antibiotic resistance. Implementation of the policy on the use of antibiotics in Sanglah Hospital in Denpasar has never been evaluated. The purpose of this study to know the implementation of policy on the use of antibiotics, preventing antibiotics resistance and determine the application ofthe WHO interventions to improve rational drug use. This research is aqualitative descriptive study, with interviews and document traces. Data analysis through content analysis. In the present study, sensitive and mutant colonies of some ruminal bacterial species isolated from sheep, cattle, and buffalo were detected. We counted and considered \"mutant colonies\" the bacterial colonies grown in the clear inhibition zone in the Kirby-Bauer disk diffusion susceptibility test. Detected mutant colonies were higher in buffalo than in cattle and sheep. Duricef and metronidazole caused no mutations in any species. The others formed mutant colonies, where roxithromycin = polymyxin = chloramphenicol = gentamicin < erythromycin < vancomycin < piperacillin = cefotaxime < streptomycin < cefoperazone < ciprofloxacin < amikacin. Sheep had the highest number of sensitive isolates, and the number of sensitive isolates was dramatically lower in cattle and buffalo. There were no sensitive isolates with the antibiotic metronidazole, and there was a low number of sensitive isolates with duricef. The other antibiotics had more sensitive isolates (gentamicin = ciprofloxacin = amikacin > streptomycin = piperacillin > erythromycin > vancomycin = cefoperazone = cefotaxime > roxithromycin > polymyxin > chloramphenicol). Antibiotic resistance genes (ARGs) among bacterial populations are causing increasing concern with medical and agricultural implications. While the effluent of wastewater treatment plants (WWTPs) is treated with a variety of antimicrobial methods, bacteria and the genetic material that is able to pass on antibiotic resistance to environmental populations are not completely destroyed. Ampicillin (amp), tetracycline (tet), and sulfonamide (sul) antibiotics have been detected in Northwest Arkansas (NWA) streams, and IncP plasmids—which are especially notorious for containing antibiotic resistance genes and have been detected after disinfection in NWA WWTPs—are known to carry ARGs for those antibiotics.
