The objective of this stud is to compare the efficacy between mobilization with movement (MWM) and end-range mobilization techniques (EMTs) in the management of adhesive capsulitis. Eighteen and twenty-two patients from MWM and EMT groups, respectively, who returned after two-week follow-up were evaluated. This was a randomized, evaluator-blind, parallel comparison study. Active and passive range of motion (ROM) for flexion, internal and external rotation, abduction and pain scale were assessed regularly by the same physical therapist. Postural hypotension is often observed in patients with cervical spinal cord injury. It may cause syncope, and interfere with sitting and standing trainings. Neuromuscular electrical stimulation (NMES) was reported to be able to alleviate and even prevent postural hypotension. The exact mechanism is not certain yet, but it may be related to vagal reflexes or muscle pumping actions. Little is known about the relation of planned exercise to insulin sensitivity in type 2 diabetes in Taiwan. Thus, the purpose of this study was to explore the effects of a 12-week aerobic exercise program on insulin sensitivity, serum lipids, cardiorespiratory fitness, and blood pressure in patients with type 2 diabetes. This study had a quasi-experimental design and used purposive sampling. The samples consisted of 13 eligible patients with type 2 diabetes. In the United States, the hospital-based fitness centers were rapidly developed in recent years and, in fact, they have already had over twenty years history. However, the concept is just at beginning stage in Taiwan. This investigation was directed against the managers of the hospital in Taiwan. To collect the interest of cognition and willingness of hospital managers on the issue of setting up a fitness center. One hundred and ninety-eight questionnaires were sent out and 138 reclaimed. A 55-year-old male patient suffered from left intracranial hemorrhage received craniotomy and removal of intracerebral hematoma. Ventriculoperitoneal shunt was implanted for the complication of hydrocephalus. The skull was put back later because of stable status. He developed scalp necrosis and, therefore several times of debridement was done. The skull was taken away under the suspicion of osteomyelitis. However, he had vomiting, somnolence, urinary incontinence and unable to walk two months after this procedure. Computed tomography showed slit ventricles in right hemisphere. Acute inflammatory demyelinating polyradiculoneuropathy (AIDP), also known as Guillain-Barre syndrome (GBS),the most common illness that causes acute and generalized paralyzing condition in the post-polio era, is diagnosed dependent on the base of clinical manifestations with supportive evidence of electrophysiologic study and cerebrospinal fluid examination. GBS often begins with the distal part of four limbs followed by a progressive course as ascending weakness and may even involve respiratory , facial or oropharyngeal muscles. Spinal fluid examination often reveals elevated protein concentration without associated increase in cells. This work describes motor, cognition and emotional problems in a child with Moyamoya disease before and after rehabilitation. Following comprehensive rehabilitation, the girl achieved improved gross motor, fine motor, cognition and emotional control. Moyamoya disease is an important cause of childhood ischemic stroke. Moyamoya disease has a bimodal age distribution, with one group of pediatric age (around first decade of life), and a second group of adults in the 30 to 40-year-old age range. Moyamoya disease involves progressive idiopathic stenosis and eventual occlusion of the large cerebral arteries at the circle of Willis. The pathogenesis of Moyamoya disease remains unclear. …

The subject patient was a case of ankylosing spondylitis (AS). Because of recurrent fine vertebral fracture combined with pseudoarthrosis and neurological deficiency, the patient had received several conventional surgical interventions over ten years. A year before the present admission, the patient suffered lower limbs weakness and received surgical intervention with thoracic corpectomy and fixation with vertebral prosthesis, anterior spinal instrumentation and posterior fusion with pedicle screw at Stanford hospital in the U.S.A. Previous studies have investigated the relationship between impaired physical fitness and the decline of living function in the elderly population. This study is to investigate the fitness profile in the middle and older persons, and to observe the effect of age on fitness.This study recruited 108 subjects, with independent ambulation skills in a Taipei community. There were 59 males and 49 females. The average age was 70.0±6.2 year-old in male and 65.9±8.9 year-old in female, respectively. The purposes of this study were to investigate the circumstances of occurrences in 53 patients with moderate to severe burns under 12 years of age, who were admitted to the burn center of the Armed Forces Tsoying Hospital in Kaohsiung from June 1998 to May 2000. The development of pelvic lymphocele is usually a complication following renal transplantation as well as pelvic lymphadenectomy for gynecologic or urologic cancers. Pelvic lymphocele after blunt trauma is rare described before. Asymptomatic patients are managed with observation and follow-up imaging studies. Symptomatic lymphoceles need to be treated by means of percutaneous needle aspiration or drainage. We reported a case of low back pain after a fall with delayed onset of ispilateral thigh swelling. Computed tomography revealed a fluid-filled mass in the pelvis. After treatment by CT-guided percutaneous aspiration, all symptoms rapidly resolved without recurrent symptoms in 6 months follow-up.

A patient with traumatic rotator cuff tear of right shoulder developed severe myofascial pain syndrome with reflex sympathetic dystrophy (RSD) involving the right upper extremity. She was unable to take any type of manual therapy or needle treatment due to severe allodynia in the whole right upper limb. She visited our pain clinic about 2 months after the onset of trauma. Hemineglect is a common symptom and sign in patients with right hemisphere lesion. These patients usually have problems of reduced awareness of stimuli from left side. This study aims to evaluate the correlation between presence of hemineglect and functional disability in patients with right hemispheric stroke or traumatic brain injury. Acute low back pain is a very common disease in Rehabilitation clinics. Although acute low back pain can be successfully treated with epidural block, there is few researches for the efficacy of caudal epidural block on acute low back pain. Seventeen patients with acute low back pain were included and managed with caudal epidural block. Spontaneous intracranial hypotension (SIH) is a syndrome with feature of low cerebrospinal fluid pressure (CSF) resulting from CSF leakage without any history of dural puncture, surgery, or penetrating trauma. The most characteristic feature of the syndrome is a postural headache. The headache is accentuated when the patient is in the upright position and relieved in the recumbent position. Associated symptoms include stiff neck, nausea, vomiting, tinnitus, hyperacusis, deafness, photophobia, diplopia, nystagmus, visual loss, vertigo, anorexia, malaise, and cranial nerves palsy. This study aimed to assess the prevalence, severity, and correlation between non-cerebral palsy mental retardation and scoliosis in a special education institute in Taipei. Adam's forward bending test was used to clinically screen 237 mental retardation non-cerebral palsy patients for scoliosis. Those patients with positive forward bending tests were referred for standing spinal radiographs, so that the Cobb angle could be quantified to allow evaluation of the severity of the scoliosis. Of the 237 students, 41 (17.3%) had positive signs in the screening test (female:16.19%, male:18.18%). ”The study aimed to depict the influence of bolus temperature, volume, and viscosity in swallowing function by a self-designed phagometer. Two micro switches were used to detect submental muscle activity and pharyngeal motion. They were anchored on a posterior-type cervicothoracic orthosis with multiple adjustable joints to ensure the well contact between the switch and the skin. We described a case of radiation myelopathy (RM). The correlations of evolutional changes in magnetic resonance image (MRI) findings in three years with clinical symptoms and treatment responses in different stages of the disease were discussed. We also describe some special rehabilitative consideration for RM patients. Cord enlargement with contrast-enhancement showed in MRI findings corresponding to rapid neurological deterioration. Some neurological improvement after anti-coagulation and I steroid therapy was observed in this stage. Cord atrophy without contrast-enhancement was showed in following MRI. Myofascial pain syndrome is very common in clinical practice. It is characterized by the existence of myofascial trigger points (MTrPs), which is a hypersensitive spot in a taut band of skeletal muscle fibers. In general, active MTrPs should be treated conservatively (non-invasive treatments including physical therapy) prior to the consideration of aggressive therapy (invasive treatments including injection and surgery). Allergic reactions to wasp stings range from large local reactions to life-threatening anaphylaxis. Of the anaphylactic shock occurring outside the hospital, wasp stings was concluded to be the second cause following drug allergy. Cerebral infarction and transient ischemic attack have been reported in the setting of acute anaphylactic shock due to wasp stings. However, there are fewer reports about the presentation of large vessels occlusion outside the brain after wasp stings. In this review article, we have described the clinical approach in detecting and confirming myofascial trigger points (MTrPs) that elicit clinical complaints. In the last decade, the pathophysiology of MTrPs has been much clarified based on clinical and basic science research work. An MTrP is a hyperirritable spot in a taut band of skeletal muscle fibers with characteristics related to central sensitization in the spinal cord. To evaluate the immediate effects of acupuncture therapy in myofascial pain syndrome (MPS), 13 subjects (11 females and 2 males), aged from 34 to 59 years old were recruited and all had suffered MPS anywhere from 1 to 180 months. The oblique insertion of an acupuncture needle was performed at an Ah's point (trigger point, TrP) and through the associated taut band without any twirling. Stretch reflex dynamics, including assessment of reflex threshold angle (RTA) and reflex threshold velocity (RTV), has been used for quantitative evaluation for spastic hypertonia. However, neither a reliable clinical scale nor objective instrumental measurement had been developed to quantify clinical hypotonia. Early diagnosis is difficult and thus treatment is often too late to prevent severe neuro- logical sequelae.A 46 years old male was admitted to our emergency department due to fever and general weakness. Under the impression of sepsis due to Staphylococcus aureus, he was admitted for further antibiotic treatment. Fever subsided one day after parenteral antibiotic usage. Myxoma is a rare cardiac tumor and also an unusual cause of cerebrovascular accident, which may result in significant permanent neurological deficit. There was no age preference for the development of myxoma. Most of the reported cases in the past were diagnosed between 20 and 50 years of age, and some of them were complicated by young (<45 y/o) stroke. We presented a 44 y/o man who had ischemic stroke with initial manifestation of left hemiparesis. A cardiac myxoma at left atrium was later detected by transthorasic echocardiography.
