Inpatient Palliative Care Utilization In Sickle Cell Disease: Temporal Trends And Factors Associated With Usage, Lois C. Akpati

Background: Sickle cell disease (SCD) is a highly morbid condition notable for recurrent hospitalizations due to vaso-occlusive crises and complications of end-organ damage. Palliative care (PC) aims to provide holistic care to patients with serious chronic illnesses. Little is known about the use of inpatient palliative care services in adult patients with SCD.

Methods: We conducted a retrospective cross-sectional study utilizing data from the National Inpatient Sample (NIS) from 2008-2017. Patients >18 years old hospitalized with a primary or secondary ICD-9-CM and ICD-10-CM diagnosis of SCD were included. Background: Home health is the fastest-growing healthcare setting in the country. Through Home Health Value-Based Purchasing (HHVBP), the Center for Medicare and Medicaid Services (CMS) provides incentives or penalties to HHAs based on outcomes. Hospitalization and emergency department use are weighted heaviest as poor outcomes. HHVBP requires HHAs to report on whether they are engaging in advance care planning (ACP) conversations. For this study, ACP was defined as a conversation held in advance of a medical crisis with a loved one and/or a health care provider about goals; values; preferences for future medical treatments; and choice of a surrogate decision-maker. …

Physician-assisted suicide, legalized in many states is becoming an option for patients diagnosed with a terminal illness. Nurse participation in physician-assisted suicide is not supported through state nurse practice acts or national nursing organizations, causing potential contradictions in practice rights for advanced practice nurses. The purpose of this study was to examine the attitudes of registered and advanced practice nurses who work with the terminally ill regarding the patient option of physician-assisted suicide. Touch of Grace is a video-based training program on palliative and end-of-life care for nursing home and assisted living staff, including all non-licensed support staff.

Pre- and Post-test data and focus group feedback indicate that the Touch of Grace training program improved understanding of palliative and end of life concepts and also provided tools to improve staff interactions with families and residents during the final days of a resident’s life. As predicted, the largest shifts in understanding were seen in the non-licensed staff, although gains were seen in all staff groups. In a neurobehavioral lens, a human is defined as the balance of the mind, body, and brain. These systems function on their own and influence each other in a dynamic equilibrium. Interactions between these systems are observed in every aspect of life. A pathological example of this relationship is that depression, a state of mind, can increase the risk of developing cardiovascular diseases. By studying a neurobehavioral model of health, empirical links between psychology and neurobiology can be uncovered. For over 2000 years, Confucianism, Daoism, and Buddhism, three pillars of Chinese traditional culture, have guided Chinese people as they approach questions of how to age and how to die. Although these traditions are still woven in the fabric of everyday life, massive economic and political transformations in recent decades have drastically reshaped Chinese people’s beliefs and practices toward aging and end-of-life care.

In this presentation, Dr. Xu Li and Dr. Baozhen Luo will share insights and stories about cultural practices in the care of the elders in China from the past to the present.
