Introduction: An abdominal aortic aneurysm (AAA) is typically defined as aortic enlargement with a diameter of 3.0 cm or larger. The prevalence of AAA has declined over the past 2 decades among screened men 65 years or older in various European countries. The current prevalence of AAA in the United States is unclear because of the low uptake of screening. Inconsistent access to reliable transportation is a significant barrier to medical care in rural New York. Lack of transportation results in missed medical appointments, poor management of chronic conditions, and increased preventable emergency room visits. Increasing routine screening for this social determinant of health can reveal underlying barriers to care, promote referrals to case workers and increase resource allocation. This project sought to increase routine screening via the promotion of smart phrases to be included in routine wellness documentation in the EMR, in conjunction with a didactic session for current family medicine residents.

In the U.S., excluding melanoma, lung cancer is the leading cause of cancer-related deaths.¹
Cigarette smoking is the number one risk factor and is linked to 80-90% of lung cancer deaths.²
Low-dose Computerized Tomography has been shown to reduce lung cancer deaths by 20% in randomized control trials and observational studies.⁴
The U.S. Preventative Task Force recommends annual screening for lung cancer with low-dose computerized tomography (LDCT) in adults ages 55-80 years of age who:

• Have a 30 pack-year smoking history and
• Currently smoke or
• Have quit within the past 15 years.

A case control study of 118 patients from Oak Hill Hospital between the months of August 2019 and January 2020 was recently completed for a hospital research quality improvement study. The objective was to determine if the use of either Hemorrhagic or Ischemic Stroke Order Set had an effect on the length of stay (LOS) or adherence to current stroke guideline recommendations when compared to its disuse. Results indicate use of either the Hemorrhagic or Ischemic Stroke Order Set led to a decreased length of stay and an increased adherence to current stroke guideline recommendations. Clinical sites for family medicine clerkship have accumulated several years of student community health projects, many of which are written patient education materials. Anonymous surveys elicited the perspectives of medical professionals and faculty on the utilization of these projects in their medical practice. Medical professionals thought the community health projects were valuable educational experiences but cited lack of time as a barrier to utilization in patient care.

With the rise of the COVID-19 Pandemic many healthcare providers are emphasizing video messaging software to maintain ongoing patient care while respecting health guidelines that recommend social distancing and isolation measures. It is a known issue that while some patients may face barriers to healthcare in the form of transportation and timing of appointments, others might encounter barriers in the form of lack of internet access. This project seeks to identify the level of patient access to this model of healthcare and assess overall satisfaction and patient attitudes towards this adaptation.

According to the American Cancer Society, colorectal cancers are diagnosed in 1 in 22 men and 1 in 24 women in their lifetime. Despite the wide range of effective CRC screening options available in the US, the Hudson Headwaters Health Network (HHHN) has seen little improvement in its CRC screening rates of approximately 60-63% in recent years. This places HHHN, an FQHC in Upstate New York, well below the National Colorectal Cancer Round Table 2018 goal of 80% screening in eligible, average-risk adults. Background/Introduction: With congenital heart disease no two hearts are exactly the same. Even with the same lesion, the anatomy of the heart can be completely different. In response to a bedside nursing idea following a cardiac arrest, advanced practice nurses (APN) initiated a program to provide personalized cardiac anatomy diagrams at the bedside of every patient on the cardiology service.
