Background: Rural areas have higher percentages of older adults with multiple chronic illnesses yet disparities exist with access to palliative care (PC) in rural areas. Palliative care can improve quality measures that positively impact care and health outcomes.
Objective: The evidence-based project’s (EBP) objective was to implement a community-based PC program in a rural primary care clinic in rural Minnesota, US and evaluate quality metrics to further support program sustainability.
Design: The project developed and implemented a community-based PC program in rural Minnesota. A tool kit was created for use for the site’s care providers and leaders.

Purpose and Rationale: To implement a delirium prevention (DP) sleep protocol to at risk intensive care unit (ICU) patients to reduce delirium incidence and duration.
Synthesis of Evidence: A review of one clinical practice guideline, eight systematic reviews and meta-analysis, four randomized controlled trials, nine quasi-experimental trials, and one qualitative study provided evidence that interventions targeted to improve patients’ sleep may decrease delirium incidence and duration in adult patients in the ICU.

Objectives: The purpose of this project was to increase nurses’ knowledge and comfort level regarding advance care planning (ACP) in patients with a diagnosis of heart failure (HF) in a clinical setting.
Methods: A quasi-experimental design was used with a pre-and-post test administered to nurses prior to the beginning of a four-hour interactive class and immediately following the class. The data was analyzed using a 5-point Likert scale and paired t-tests were used to determine statistical significance. Statistical significance was set at 0.05.

Problem: One of the most important steps in infection prevention is the use of personal protective equipment (PPE) to protect patients and staff from infectious agents; yet, research indicates that PPE compliance remains suboptimal in many healthcare institutions.

Abstract
The desired outcome for this project is to improve nurse’s self-perceived confidence, competence and knowledge in recognizing the early symptoms of sepsis in adult medical patients. Evidence indicates that early recognition and intervention are key to survival.
Overall, the literature regarding sepsis education indicates that education in combination with a screening tool is the most effective in assisting nurses with early recognition of sepsis in hospitalized patients. All studies reviewed on escape rooms focused on nursing knowledge and self-perceived confidence or competence and used a pre/posttest design.

Objective: To improve self-efficacy in patients with type 2 diabetes and a Hemoglobin A1c (HbA1c) of 8% (64 mmol/mol) or greater using an innovative education multidisciplinary approach in a group setting.
Research Design and Methods: Out of 248 patients that were contacted with type 2 diabetes, who had a HbA1c of 8% or greater, five patients from a large clinic in the Midwest agreed to participate in group education. The organization determined there was a lack of a standardized progress review tool to objectively evaluate nurses’ clinical judgment during orientation. Further assessment determined gaps in communication between the preceptors and unit-based nursing education specialists (NESs), and a perceived lack of value in progress reviews. Tanner’s (2006) review of nearly 200 nursing research articles determined a nurse’s clinical judgments impact patient outcomes, making it imperative that organizations ensure their nurses have adequate clinical judgment to protect patients. Lasater’s clinical judgment rubric (LCJR), based on Tanner’s (2006) clinical judgment model was the only published tool found for assessing nurses’ clinical judgment. …
