The introduction of a large number of new antibiotics has made selecting the one most appropriate for treatment a confusing task for the practicing physician. One problem is that the differences in pharmacokinetics, in vitro activity, and clinical indications between some of these agents may be relatively minor; this is especially true of the new cephalosporins. Another problem is that the generic names of the cephalosporins are so similar that it is impractical, even for the infectious diseases specialist, to be familiar with all of them. In Vitro And Clinical Studies Of Cefaclor, A New Cephalosporin, Daria Kiani, Tom Madhavan, Keith Burch, Don Pohlod, Evelyn Fisher, Edward L. Quinn

In vitro susceptibility studies of 246 clinical isolates demonstrated that this antibiotic was effective against Group A beta hemolytic streptococci, alpha hemolytic streptococci, S. pneumoniae, both penicillinase producing and nonpenicillinase producing Staphylococci, Klebsiella pneumoniae, and Proteus mirabilis. Cephalothin susceptible E. coii were also susceptible to cefaclor. Pseudomonas aeruginosa, Serratia sp., Enterobacter Sp., and Streptococcus faecalis were uniformly resistant to cefaclor. The efficacy and safety of this antibiotic were studied in 27 patients with urinary tract, soft tissue, and respiratory infections. Amikacin was evaluated in vitro against S aureus and a variety of gram negative bacilli. In concentrations of 12.5 μg/ml or less, it suppressed the growth of all organisms except 15% of Proteus mirabilis. Peak serum levels in patients, following a 7.5 mg/kg IM dose, averaged 21 μg/ml at one hour. Most of the drug appeared in the urine during the first six hours after administration. Of six patients receiving an adequate course of treatment, all but one were cured. Treatment in that patient also failed with a subsequent course of gentamicin and carbenicillin and he was cured only by …

Promethazine-Chlorpromazine Combination In The Treatment Of Unmanageable Psychotic Patients, Armando R. Favazza

Administering a combination of promethazine and chlorpromazine to patients with a "galloping psychosis" has an antipsychotic and tranquilizing effect which calms them down to a more manageable and less aggressive state. The drugs are chemically similar; promethazine's actions are strongly potentiated in the combination, so large doses must be given under careful supervision. Case histories demonstrate successful short term management of acutely psychotic, aggressive patients who were a danger to themselves and others.

A Preliminary Evaluation Of Azathioprine (Imuran®) In The Treatment Of Multiple Sclerosis, William G. Tucker, K. H. Kapphahn

A theoretical approach to the unknown etiology of multiple sclerosis is presented in this preliminary study which reports the results of empiric treatment with azathioprine. Imuran® therapy produced stabilization of the disease, neurological improvement, and no progress of neurological dysfunction or loss of any regained function. Evidence points toward a viral-induced immunological defect. Abnormal gamma globulins found in the cerebral spinal fluid suggest an immunological etiology.

This case history of a 45-year-old woman, who had been issued Dicumarol instead of Dronactin in a pharmacy-filled prescription, describes an emergency therapeutic program to control hemorrhage with vitamin K, adrenal corticosteroids, and the giving of fresh blood. In a patient with a bleeding disorder, unintentional ingestion of oral anticoagulants is to be suspected if the prothrombin time is markedly elevated, if the patient has been previotisly treated with an oral anticoagulant, or is engaged in a medical or paramedical occupation.
