Many people in Androscoggin county are not able to consistently achieve a diet with the recommended food variety for improving health. Adequate nutrition for improving health outcomes requires access to healthy food at affordable prices, health literacy regarding diet and nutrition concepts, and the skills and access to equipment required to prepare and store food. A lack of resources in any one of these areas can limit a person’s ability to engage in a healthy diet, which increases their risk of developing preventable non-communicable diseases later in life.

This project aimed to build on a prior family medicine clerkship project which designed a pamphlet containing high-yield facts to help patients interpret common lab results. An Epic SmartPhrase was created from this information and made available for all users at Colchester Family Medicine. Fewer messages from patients inquiring about lab values would be expected to be routed to providers if this intervention were successful upon further evaluation. Future projects could create SmartPhrases which cover other frequently ordered tests, such as complete blood counts with differential, thryoid cascade, iron studies and urinalysis.

This project examines ways to increase access to HIV pre-exposure prophylaxis (PrEP) in Plattsburgh, New York (located in Clinton County). After a review of the current literature on the subject and interviews with community members, an intervention was conducted at CVPH Family Medicine. EHR tools were generated and distributed to providers in the practice, with the goal of increasing the distribution of patient education materials about PrEP.

This project aimed to develop a better understanding of patient and provider needs at all UVMMC family medicine and urgent care offices in regards to the new Cures Act. Data was gathered via a 8 question multiple choice survey and we had 24 provider responses. Most providers have found that they are receiving more phone calls from patients with questions in regards to their medical notes or results since the enactment of the Cures Act. At Milton Family Practice in Milton, VT, individuals diagnosed with pre-diabetes are referred to a clinical nutritionist and sent home with patient instructions about blood sugar and carbohydrates. For those who decline a meeting with the clinical nutritionist, the patient instructions are not an adequate substitute. This project focused on the creation of a SmartPhrase to be added to patient instructions outlining common high-carbohydrate foods and options for lower-carb alternatives.

HPV is a very common infection and can cause multiple types of cancers. The HPV vaccination is effective at preventing these infections and subsequent cancers. The HPV vaccine is most effective when given to adolescents; however, the vaccination rates for girls and especially boys remain low. Many reasons exist for parent's choice not to have their kids' vaccinated against HPV. Identifying and addressing these barriers through educational materials for parents will hopefully help improve HPV vaccination rates and reduce rates of these preventable cancers.

The goal of this project is to implement the creation of parent educational handouts distributed at routine visits at CHCRR Family Practice in Rutland, VT, to provide anticipatory guidance and improve attendance of pediatric well visits from birth to 5 years old.

As the popularity of cannabidiol (CBD) increases, more patients are interested in trying a "natural" adjunct to more standard medications. But with no current FDA oversight, an estimated 70% of CBD products could be mislabeled. In Vermont, federal regulation requires hemp products to contain less than 0.3% THC, but this has only been tested one time at one location since the 2018 Farm Bill was passed. Patient Education On Sweetened Beverages: An Interactive And Module-Based Approach, Matt Tsai

As sugary beverages continue to contribute to negative health outcomes, there remains a need for efficient, effective patient education in this area. A custom, interactive and reflective web-based module on sugary beverages for adult participants (n=42) was introduced at a rural Vermont outpatient clinic. Built-in quizzes evaluated learning through a pre and post-intervention design. On average, participants improved significantly from a pre-module mean score of 3.00 to a post-module score of 4.33 (p<0.0001). Additionally, participants' average rating of the module's effectiveness was 8.3 out of 10. A module-based approach may be one efficacious solution to improve patient education on sugary beverage consumption.

Most patients are willing to have advanced care planning (ACP) discussions with their medical team, even if they may be young and healthy. Yet, time constraints, perceived stigma of conversations surrounding death, and the educational requirement of documentation are known barriers for care providers to have these discussions. Identifying a health care agent (HCA) can be an important first step to continued ACP in the primary care setting. The purpose of this project is to propose a systemic approach to having HCA discussions in a time-efficient manner that can be easily implemented as part of a routine medical interview.

Diabetic Retinopathy is the leading cause of blindness in patient's age 25-74 in the world. Onset of disease is insidious and early intervention is the only way to reduce morbidity. On average only 60% of diabetic's get an annual eye exam, but in the Western Connecticut Health Network (WCHN) that number is closer to 32%. This project sought to educate and encourage patients to take advantage of a new tele-medicine, primary care based diabetic retinopathy screening program starting at Brookfield Family Medicine (part of the WCHN).

A large proportion of major depression in Vermont has a seasonal pattern. The most common form of Seasonal Affective Disorder is winter depression. Several evidence-based treatments exist for SAD including light therapy, CBT, and antidepressant medications. Adjunct therapies, like exercise and social interaction, can also help patients feel better during the winter.
